National Biosurveillance Strategy

The United States National Biosurveillance Strategy is the plan to implement a biosurveillance system that will monitor and interpret data that might relate to disease activity and threats to human or animal health – whether infectious, toxic, metabolic, and regardless of intentional or natural origin – in order to achieve early warning of health threats, early detection of health events and overall situational awareness of disease activity. Forty-eight states and the capital's federal district are contiguous in North America between Canada and Mexico. The State of Hawaii is an archipelago in the mid-Pacific Ocean. The U.S. territories are scattered about the Pacific Ocean and the Caribbean Sea, stretching across nine official time zones. The extremely diverse geography, climate, and wildlife of the United States make it one of the world's 17 megadiverse countries.

Biosurveillance is an aspect of biodefense relating to the detection of bioterrorism threats.

Types of data collection

It is debated whether is it better to engage in a “more data, faster is better” approach or whether it would be more effective to pursue fewer but more meaningful data streams. [2] Some of the things that will be monitored are counts of clinical diagnoses, sales of over-the-counter remedies, and school absentees among select age groups. [3]

Proposed plan

The Department of Homeland Security laid out the plan for the national biosurveillance system. They stated that an effect surveillance system relies on five key principles that are generally applied to public health and medical preparedness. The first is being prepared for all potential catastrophic health events; the second is having vertical and horizontal coordination among all levels of government; the third is having a regional approach to health preparedness; the fourth is the engagement of the private sector, academia, and other nongovernmental entities; the fifth is the important roles of individuals, families, and communities. [4] The biosurveillance program must be nationwide, robust, and integrated with international disease systems in order to provide early warning and ongoing characterization of disease outbreaks in real-time. [5] The system must be sufficiently able to identify a specific disease and its prevalence in varying populations and environments and must be able to tailor the analysis to new diseases. [1] The system should create a networked system to allow for two-way information flow between and among Federal, State, and local government public health authorities and clinical health care providers. [1] The system shall build upon existing Federal, State, and local surveillance systems where they exists and should enable and provide incentive for public health agencies to implement surveillance systems where they do not exist. [1]

APHL serves as a liaison between laboratories and federal and international agencies, and ensures that the network of laboratories has current and consistent scientific information in order to be ready for outbreaks and other public health emergencies. Membership consists of local, territorial, county and state public health laboratories; environmental, agricultural and veterinary laboratories; and corporations and individuals with an interest in public health and laboratory science. APHL is a non-profit, 501(c)(3) organization with a history of over fifty years.

The Public Health Information Network (PHIN) is a national initiative, developed by the Centers for Disease Control and Prevention (CDC), for advancing fully capable and interoperable information systems in public health organizations. The initiative involves establishing and implementing a framework for public health information systems.

The Health Threat Unit of the Directorate-General for Health and Consumer Protection, is responsible for terrorism surveillance and early warning of biological, chemical, and radiological threats within the European Union. The Health Threat Unit runs the Rapid Alert System, which conducts surveillance on communicable diseases and diseases caused by acts of bioterrorism. The surveillance data are coordinated and evaluated by the Health Emergency Operations Facility. Health threat information and warnings are sent to the member states by the Communication and Crisis Center (BICHAT) and the Security Office in Brussels, Belgium.

Project Bioshield Act

The Project Bioshield Act was an act passed by the United States Congress in 2004 calling for $5 billion for purchasing vaccines that would be used in the event of a bioterrorist attack. This was a ten-year program to acquire medical countermeasures to biological, chemical, radiological, and nuclear agents for civilian use. A key element of the Act was to allow stockpiling and distribution of vaccines which had not been tested for safety or efficacy in humans, due to ethical concerns. Efficacy of such agents cannot be directly tested in humans without also exposing humans to the chemical, biological, or radioactive threat being treated, so testing follows the FDA Animal Rule for pivotal animal efficacy.

Disaster medicine is the area of medical specialization serving the dual areas of providing health care to disaster survivors and providing medically related disaster preparation, disaster planning, disaster response and disaster recovery leadership throughout the disaster life cycle. Disaster medicine specialists provide insight, guidance and expertise on the principles and practice of medicine both in the disaster impact area and healthcare evacuation receiving facilities to emergency management professionals, hospitals, healthcare facilities, communities and governments. The disaster medicine specialist is the liaison between and partner to the medical contingency planner, the emergency management professional, the incident command system, government and policy makers.

DHS Chemical and Biological Defense Division

The Chemical and Biological Defense Division (CBD) is a division of the Science and Technology Directorate of the United States Department of Homeland Security. Within the Homeland Security Advanced Research Projects Agency, CBD develops technologies to increase the United States's preparedness and protect key National infrastructure against chemical, biological, and agricultural threats and disasters through improved threat awareness and advanced surveillance, detection, and protective countermeasures.

Einstein was originally an intrusion detection system that monitors the network gateways of government departments and agencies in the United States for unauthorized traffic. The software was developed by the United States Computer Emergency Readiness Team (US-CERT), which is the operational arm of the National Cyber Security Division (NCSD) of the United States Department of Homeland Security (DHS). The program was originally developed to provide "situational awareness" for the civilian agencies. While the first version examined network traffic and subsequent versions examined content, the current version of Einstein is significantly more advanced.

Pandemic and All-Hazards Preparedness Act

On December 19, 2006, the Pandemic and All-Hazards Preparedness Act (PAHPA), Public Law No. 109-417, was signed into law by President George W. Bush. PAHPA has broad implications for the United States Department of Health and Human Services's (HHS) preparedness and response activities. Among other things, the act amended the Public Health Service Act to establish within the department a new Assistant Secretary for Preparedness and Response (ASPR); provided new authorities for a number of programs, including the advanced development and acquisitions of medical countermeasures; and called for the establishment of a quadrennial National Health Security Strategy.

Homeland Security Centers of Excellence (HS-Centers) are sponsored by the U.S. Department of Homeland Security. These Centers develop technology and training for police and domestic military units. The Centers were created under the Homeland Security Act in 2002 in order to establish a coordinated, university-based system to increase domestic security measures.

The Center for Infectious Disease Research and Policy (CIDRAP) is a center within the University of Minnesota that focuses on addressing public health preparedness and emerging infectious disease response. It was founded in 2001 by Michael Osterholm, PhD, MPH, in order to "prevent illness and death from infectious diseases through epidemiological research and rapid translation of scientific information into real-world practical applications and solutions".

BioSense is a program of the Centers for Disease Control and Prevention (CDC) that tracks health problems as they evolve and provides public health officials with the data, information and tools they need to better prepare for and coordinate responses to safeguard and improve the health of the American people.

By integrating local and state-level information, CDC will provide a timely and cohesive picture at the regional and national levels and improve BioSense's utility. The key components of the BioSense program redesign are to:

Real-time outbreak and disease surveillance system (RODS) is a syndromic surveillance system developed by the University of Pittsburgh, Department of Biomedical Informatics. It is "prototype developed at the University of Pittsburgh where real-time clinical data from emergency departments within a geographic region can be integrated to provide an instantaneous picture of symptom patterns and early detection of epidemic events."

The International Society for Disease Surveillance (ISDS) is a 501(c) 3 nonprofit organization, based in Boston, Massachusetts and dedicated to the improvement of population health by advancing the science and practice of disease surveillance. ISDS facilitates interdisciplinary collaboration and promote and conduct research, education and advocacy. ISDS's 400+ membership represents professional and academic subject matter experts in the fields of public health surveillance, clinical practice, health informatics, health policy, and other areas related to national and global health surveillance. ISDS holds an annual national conference for the public health surveillance community.

Pandemic and All-Hazards Preparedness Reauthorization Act of 2013

The Pandemic and All-Hazards Preparedness Reauthorization Act of 2013 is a law enacted by the 113th United States Congress. The Act amends the Public Health Service Act in order to extend, fund, and improve several programs designed to prepare the United States and health professionals in the event of a pandemic, epidemic, or biological, chemical, radiological, or nuclear accident or attack. The Act clarifies the authority of different American officials, makes it easier to temporarily reassign personnel to respond to emergency situations, and alters the process for testing and producing medical countermeasures. The Act is focused on improving preparedness for any public health emergency.

The Blue Ribbon Study Panel on Biodefense is a panel of former high-ranking government officials that analyzes US capabilities and capacity to defend against biological threats. According to the Panel's mission statement, the organization was formed to "provide for a comprehensive assessment of the state of U.S. biodefense efforts, and to issue recommendations that will foster change."

A Foreign animal disease (FAD) is an animal disease or pest, whether terrestrial or aquatic, not known to exist in the United States or its territories. When these diseases can significantly affect human health or animal production and when there is significant economic cost for disease control and eradication efforts, they are considered a threat to the United States. Another term gaining preference to be used is Transboundary Animal Disease (TAD), which is defined as those epidemic diseases which are highly contagious or transmissible and have the potential for very rapid spread, irrespective of national borders, causing serious socio-economic and possibly public health consequences. An Emerging Animal Disease "may be defined as any terrestrial animal, aquatic animal, or zoonotic disease not yet known or characterized, or any known or characterized terrestrial animal or aquatic animal disease in the United States or its territories that changes or mutates in pathogenicity, communicability, or zoonotic potential to become a threat to terrestrial animals, aquatic animals, or humans."
