The Division II tournaments made up the third level of competition at the 2003 IIHF World U18 Championships. The Group A tournament took place between 17 and 23 March 2003 in Tallinn, Estonia and the Group B tournament took place between 5 and 11 March 2003 in Belgrade, Serbia and Montenegro. South Korea and Romania won the Group A and Group B tournaments respectively and gained promotion to Division I for the 2004 IIHF World U18 Championships. While Bulgaria finished last in Group A and South Africa last in Group B and were both relegated to Division III for 2004.

Konstantin Mihailov is a Bulgarian former professional ice hockey and roller in-line hockey goaltender. He played in 28 Ice Hockey World Championships with the Bulgaria men's national ice hockey team, and three IIHF Inline Hockey World Championships with the Bulgaria men's national inline hockey team, then retired from international play at age 51. His lengthy international career was recognized by the International Ice Hockey Federation (IIHF) with receipt of the Torriani Award, which inducted him into the IIHF Hall of Fame in 2019.
