Coastal Hospice and Palliative Care is a non-profit health care organization founded in 1980. [1] It is located in Salisbury, Maryland, and serves Dorchester, Wicomico, Somerset and Worcester counties.

Its mission as a hospice is to serve the needs of the terminally ill in their own homes throughout the four Lower Shore counties. [2] [3] Coastal Hospice also provides palliative care, or medical care for managing pain and other symptoms of cancer. Coastal Hospice uses a team approach, sending registered nurses, counselors, home health aides, social workers, and volunteers to patients' homes. Working with the patients' own physicians, the health care workers strive to help patients live fully and as pain-free as possible. Charitable contributions and donations support charity care, helping to offset costs of care for those who need help but cannot pay either privately or by insurance.

Coastal Hospice at the Lake

Coastal Hospice at the Lake opened in 2004. It is a 14-bed inpatient facility located within the Deer's Head Center. [4] It is designed to give patients short-term care, usually less than five days, when a patient needs intensified medical care to treat symptoms that cannot be managed at home. The facility is also used for respite care, when a caregiver needs a break.

Coastal Hospice at the Ocean

Recently renamed the Macky & Pam Stansell House of Coastal Hospice at the Ocean, the new facility is expected to open in fall 2018. [5] The facility will be home for hospice patients who have no one to care for them in their own homes. It will also be a community outreach center focusing on palliative care, volunteers, grief support, and community education such as advance directives. Located at The Point in Ocean Pines, most patient rooms and gathering areas will have a view of the Isle of Wight Bay and the Ocean City skyline. [6]

The Delmarva Peninsula, or simply Delmarva, is a large peninsula on the East Coast of the United States, occupied by the vast majority of the state of Delaware and parts of the Eastern Shore regions of Maryland and Virginia. The peninsula is 170 miles (274 km) long. Palliative care is an interdisciplinary medical caregiving approach aimed at optimizing quality of life and mitigating suffering among people with serious, complex, and often terminal illnesses, with death as the outcome. Within the published literature, many definitions of palliative care exist. The World Health Organization (WHO) describes palliative care as "an approach that improves the quality of life of patients and their families facing the problems associated with life-threatening illness, through the prevention and relief of suffering by means of early identification and impeccable assessment and treatment of pain and other problems, physical, psychosocial, and spiritual." In the past, palliative care was a disease specific approach, but today the WHO takes a more broad approach, that the principles of palliative care should be applied as early as possible to any chronic and ultimately fatal illness.

In medicine, specifically in end-of-life care, palliative sedation is the palliative practice of relieving distress in a terminally ill person in the last hours or days of a dying person's life, usually by means of a continuous intravenous or subcutaneous infusion of a sedative drug, or by means of a specialized catheter designed to provide comfortable and discreet administration of ongoing medications via the rectal route.

Terminal illness or end-stage disease is a disease that cannot be cured or adequately treated and is reasonably expected to result in the death of the patient. This term is more commonly used for progressive diseases such as cancer, dementia or advanced heart disease than for injury. In popular use, it indicates a disease that will progress until death with near absolute certainty, regardless of treatment. A patient who has such an illness may be referred to as a terminal patient, terminally ill or simply as being terminal. There is no standardized life expectancy for a patient to be considered terminal, although it is generally months or less. Life expectancy for terminal patients is a rough estimate given by the physician based on previous data and does not always reflect true longevity. An illness which is lifelong but not fatal is a chronic condition.

Royal Trinity Hospice Hospital in London, England

Royal Trinity Hospice is the oldest hospice in the United Kingdom, founded in 1891 by a member of the Hoare banking family. It is located in Clapham Common, London, England, and provides specialist palliative care. In 2019 Royal Trinity Hospice was rated "Outstanding" by the Care Quality Commission, the highest rating that can be awarded. The hospice provides palliative and end of life care for patients in an inpatient unit at their Clapham Common headquarters and in the community, wherever patients may be living. Florence Wald American nurse (1917–2008)

Florence Wald was an American nurse, former Dean of Yale School of Nursing, and largely credited as "the mother of the American hospice movement". She led the founding of Connecticut Hospice, the first hospice program in the United States. Late in life, Wald became interested in the provision of hospice care within prisons. In 1998, Wald was inducted into the National Women's Hall of Fame.

The American Academy of Hospice and Palliative Medicine (AAHPM) is a professional organization for physicians specializing in Hospice and Palliative Medicine, headquartered in Chicago, Illinois. AAHPM has more than 5,200 members; 82 percent are physicians, 12 percent are nurses or other health care providers and 6 percent are residents or students.

Community Hospice & Palliative Care, also known simply as Community Hospice, is a not-for-profit hospice, which has served the Greater Jacksonville Metropolitan Area since its inception in 1979. The organization was the first hospice program in Northeast Florida and one of a few operating programs in the state when Florida began granting hospice licenses in 1981; Community Hospice received their license in 1983 and in 2008, assisted nearly 1,000 patients daily and more than 6,000 patients a year.

Hospice care in the United States

In the United States, hospice care is a type and philosophy of end-of-life care which focuses on the palliation of a terminally ill patient's symptoms. These symptoms can be physical, emotional, spiritual or social in nature. The concept of hospice as a place to treat the incurably ill has been evolving since the 11th century. Hospice care was introduced to the United States in the 1970s in response to the work of Cicely Saunders in the United Kingdom. This part of health care has expanded as people face a variety of issues with terminal illness. In the United States, it is distinguished by extensive use of volunteers and a greater emphasis on the patient's psychological needs in coming to terms with dying.

Hospice care is a type of health care that focuses on the palliation of a terminally ill patient's pain and symptoms and attending to their emotional and spiritual needs at the end of life. Hospice care prioritizes comfort and quality of life by reducing pain and suffering. Hospice care provides an alternative to therapies focused on life-prolonging measures that may be arduous, likely to cause more symptoms, or are not aligned with a person's goals.

The Community Foundation of the Eastern Shore (CFES) was established in 1984, and is located in Salisbury, Maryland. Its mission is to "To strengthen our community by connecting people who care with causes that matter to improve quality of life in our region" and its vision is for "...a community where all can live, learn, work, create and prosper". It serves the three lower counties of the Eastern Shore of Maryland: Somerset, Wicomico, and Worcester.
