At the 1904 Summer Olympics, a water polo tournament was contested. Only American contestants participated; three teams of 7 players each entered. Water polo is mentioned in the games reports for the 1904 Summer Olympics, but currently water polo event is not included in the IOC's medal database for 1904 Olympics, and currently IOC consider water polo event as part of unofficial programme in 1904. The event took place in a pond in Forest Park, the location of both the Olympics and the World's Fair.

Water polo at the 1980 Summer Olympics as usual was a part of the swimming sport, other two parts were swimming and diving. They were not three separate sports, because they all were governed by one federation — FINA. Water Polo discipline consisted of one event: men's team. In the preliminary round 12 teams were divided into three groups. Two best teams from each group advanced to Group A of the final round to determine places 1 through 6. The rest of teams played in Group B of the final round to determine places 7 through 12.

The women’s league was added in 2004, based on growing interest in women's play.

The next time India won a medal was a bronze at the 1982 Asian Games.

The water polo tournaments at the 2016 Summer Olympics in Rio de Janeiro took place from 6 to 20 August at the Maria Lenk Aquatics Centre in Barra da Tijuca. Twenty teams competed in the tournament. Games that ended in ties in elimination rounds were decided by shootouts, as overtime has been abolished since 2013.

The 2014 FINA Men's Water Polo World League is played between November 2013 and June 2014 and open to all men's water polo national teams. After participating in a preliminary round, eight teams qualify to play in a final tournament, called the Super Final in Dubai, UAE from 16–21 June 2014.

Water polo competitions at the 2019 Pan American Games in Lima, Peru are scheduled to be held from July 28 to August 10. The venue for the competition is the Water polo pool located at the Villa María del Triunfo cluster. A total of eight men's and eight women's teams competed in each tournament. This means a total of 176 athletes are scheduled to compete.

The FINA Water Polo World Rankings is a ranking system for men's and women's national teams in water polo. The teams of the member nations of FINA, water polo's world governing body, are ranked based on their competitions results with the most successful teams being ranked highest.
