The Keogh Review into patient safety was carried out by Professor Sir Bruce Keogh in July 2013. [1]

Sir Bruce Edward Keogh, KBE, FRCS, FRCP is a British surgeon who specialises in cardiac surgery. He was Medical Director of the National Health Service in England from 2007 and National Medical Director of the NHS Commissioning Board from 2013 until his retirement early in 2018. He is Chair of Birmingham Women's and Children's NHS Foundation Trust.

This review was ordered by the Prime Minister in response to the Francis Inquiry into poor care at Mid Staffordshire Hospitals NHS Foundation Trust.

14 NHS Trusts which were persistent outliers in measures of hospital mortality were investigated:

Basildon and Thurrock University Hospitals NHS Foundation Trust provides healthcare for people in the south west Essex, in the East of England. There are two hospitals in the trust, a specialist cardiothoracic centre and one clinical centre: Basildon University Hospital, Orsett Hospital, The Essex Cardiothoracic Centre and Billericay St. Andrew's Centre. It became a Foundation Trust in 2004.

Blackpool Teaching Hospitals NHS Foundation Trust is an NHS Foundation Trust providing health services in North Lancashire, England. It runs Blackpool Victoria Hospital which is a large busy acute hospital; two smaller community hospitals - Clifton Hospital and Fleetwood Hospital; three elderly rehabilitation hospitals - Wesham Hospital Rehabilitation Unit, Rossall Hospital Rehabilitation Unit; the National Artificial Eye Service; Blenheim House Child Development Centre and community health services for North Lancashire.

Buckinghamshire Healthcare NHS Trust runs Wycombe Hospital, Stoke Mandeville Hospital, Amersham Hospital, Buckingham Community Hospital and Thame Community Hospital, in Buckinghamshire, England.

As a result of the review six NHS Foundation Trusts were put into special measures by Monitor and five by the NHS Trust Development Authority. Blackpool Teaching Hospitals NHS Foundation Trust, Colchester Hospital University NHS Foundation Trust and The Dudley Group NHS Foundation Trust were not put in special measures. [2]

Monitor was an executive non-departmental public body of the Department of Health, responsible for ensuring healthcare provision in NHS England was financially effective. It was the sector regulator for health services in England. Its chief executive was Ian Dalton and its Chair is Dido Harding. Monitor was merged with the NHS Trust Development Authority to form NHS Improvement on 1 April 2016.

The NHS Trust Development Authority (NHSTDA) was an executive non-departmental public body of the Department of Health. Its formation came as a result of reorganisation of the National Health Service (NHS) in England outlined in the Health and Social Care Act 2012. It is now part of NHS Improvement.

Colchester Hospital University NHS Foundation Trust was an NHS Foundation Trust which ran Colchester General Hospital in Colchester, Essex.

East Lancashire Hospitals NHS Trust is an NHS hospital trust in Lancashire, England. It was formed in April 2003, as the result of a locally controversial, cost saving merger of Blackburn Hyndburn & Ribble Valley (BHRV) NHS Trust and Burnley Health Care NHS Trust, first announced in September 1999.

The Medway NHS Trust is one of four hospital trusts in Kent, in southeast England. The trust employs over 3500 staff. The trust's main focus is running Medway Maritime Hospital in Gillingham, Medway.

Aintree University Hospital NHS Foundation Trust is an NHS Foundation Trust located in the suburb of Fazakerley, in the city of Liverpool, England. It was formerly known as University Hospital Aintree NHS Trust and became a Foundation Trust in 2006. It manages Aintree University Hospital.
