Cytotechnology is the microscopic interpretation of cells to detect cancer and other abnormalities. [1] This includes the examination of samples collected from the uterine cervix (Pap test), lung, gastrointestinal tract, or body cavities. [2]

A cytotechnologist is an allied health professional trained to evaluate specimens on glass slides using microscopes. [3] Two evaluations can be performed, starting with the initial evaluation, which can be performed by a computer, and points out areas that may be of particular interest for later examination. Then, the cytotechnologist performs a secondary evaluation and determines whether a specimen is normal or abnormal. Abnormal specimens are referred to a pathologist for final interpretation or medical diagnosis.

Much like with other medical fields, a Cytotechnologist's work must be completed with high fidelity and must be interpreted properly. The working relationship between the cytotechnologist and the pathologist provides quality control for their work. Discussions between the two and the severity of their disagreements are used as a measurement for quality assurance. [4] For example, when a cytotechnologist is working to construct a tissue microarray (TMA), they must check in with the supervising pathologist at multiple points in the process. The pathologist must first work with the cytotechnologist to make sure they know what to look for in the selection process and then review any selection that the cytotechnologist has concerns about. [5]  

Different countries have different certification requirements and standards for cytotechnologists. In the United States, there are currently two routes for certification: a person can first earn a bachelor's degree and then attend an accredited program in cytotechnology for one year, or they can attend a cytotechnology program that also awards a bachelor's degree. After successful completion of either route, the individual becomes eligible to take a certification exam offered by the American Society for Clinical Pathology (ASCP). [6] People who complete the requirements and pass the examination are entitled to designate themselves as "CT (ASCP)". People who reside outside the United States have the option to take the certification exam for the ASCPi instead. They must complete all requirements to be eligible, and upon successful completion, can practice in the USA. [7] The American Society for Cytotechnology (ASCT) sets U.S. professional standards, monitors legislative and regulatory issues, and provides education. Individual states regulate the licensure of cytotechnologists, usually following American Society of Cytopathology (ASC) guidelines. [8] Other countries have their own versions of the ASCT, including the British Association of Cytopathology (BAC) in the UK [9] and the European Federation of Cytology Societies (EFCS) in the EU. [10]  

The ASC is for cytopathologists but certain qualified cytotechnologists can join it as well.

History

ASCP certified the first cytotechnologist in 1957. In the 1970s, the number of schools peaked at 130, before dropping to 32 in 2011. [11]

Pathology is the study of disease and injury. The word pathology also refers to the study of disease in general, incorporating a wide range of biology research fields and medical practices. However, when used in the context of modern medical treatment, the term is often used in a narrower fashion to refer to processes and tests that fall within the contemporary medical field of "general pathology", an area that includes a number of distinct but inter-related medical specialties that diagnose disease, mostly through analysis of tissue and human cell samples. Idiomatically, "a pathology" may also refer to the predicted or actual progression of particular diseases, and the affix pathy is sometimes used to indicate a state of disease in cases of both physical ailment and psychological conditions. A physician practicing pathology is called a pathologist.

Anatomical pathology (Commonwealth) or anatomic pathology (U.S.) is a medical specialty that is concerned with the diagnosis of disease based on the macroscopic, microscopic, biochemical, immunologic and molecular examination of organs and tissues. Over the 20th century, surgical pathology has evolved tremendously: from historical examination of whole bodies (autopsy) to a more modernized practice, centered on the diagnosis and prognosis of cancer to guide treatment decision-making in oncology. The utility of clinical diagnostic testing relies squarely on the validity of test methodology. A koilocyte is a squamous epithelial cell that has undergone a number of structural changes, which occur as a result of infection of the cell by human papillomavirus (HPV). Identification of these cells by pathologists can be useful in diagnosing various HPV-associated lesions.

Diff-Quik is a commercial Romanowsky stain variant used to rapidly stain and differentiate a variety of pathology specimens. It is most frequently used for blood films and cytopathological smears, including fine needle aspirates. The Diff-Quik procedure is based on a modification of the Wright-Giemsa stain pioneered by Harleco in the 1970s, and has advantages over the routine Wright-Giemsa staining technique in that it reduces the 4-minute process into a much shorter operation and allows for selective increased eosinophilic or basophilic staining depending upon the time the smear is left in the staining solutions.

Surgical pathology is the most significant and time-consuming area of practice for most anatomical pathologists. Surgical pathology involves gross and microscopic examination of surgical specimens, as well as biopsies submitted by surgeons and non-surgeons such as general internists, medical subspecialists, dermatologists, and interventional radiologists.

The American Society of Cytopathology (ASC), founded in 1951, is a national professional society of physicians, cytotechnologists and scientists who are dedicated to cytopathology, which involves the cytologic method of diagnostic pathology. They have more than 3000 members including representatives for other countries. The ASC provides a forum where physicians and cytotechnologists can interact with one another. Although this society is involved with guidelines for cytotechnologists and admits certain qualified cytotechnologists, it should not be confused with the American Society for Cytotechnology (ASCT).

The American Society for Clinical Pathology (ASCP), formerly known as the American Society of Clinical Pathologists is a professional association based in Chicago, Illinois encompassing 130,000 pathologists and laboratory professionals.

The Bethesda system (TBS), officially called The Bethesda System for Reporting Cervical Cytology, is a system for reporting cervical or vaginal cytologic diagnoses, used for reporting Pap smear results. It was introduced in 1988 and revised in 1991, 2001, and 2014. The name comes from the location of the conference, sponsored by the National Institutes of Health, that established the system.

<span class="mw-page-title-main">Cervical screening</span> Type of medical screening

Cervical cancer screening is a medical screening test designed to identify risk of cervical cancer. Cervical screening may involve looking for viral DNA, and/or to identify abnormal, potentially precancerous cells within the cervix as well as cells that have progressed to early stages of cervical cancer. One goal of cervical screening is to allow for intervention and treatment so abnormal lesions can be removed prior to progression to cancer. An additional goal is to decrease mortality from cervical cancer by identifying cancerous lesions in their early stages and providing treatment prior to progression to more invasive disease.

A pathologists’ assistant (PA) is a physician extender whose expertise lies in gross examination of surgical specimens as well as performing forensic, medicolegal, and hospital autopsies.

John Kingsbury Frost was an American physician specializing in the field of cytopathology - the microscopic study of individual body cells to detect cancer and other diseases. The first area of the body to be studied in this way was the female genital tract, using the Pap smear invented by Georgios Papanikolaou. Frost and other physicians expanded the field to allow for cytopathologic evaluation of the lung, bladder, and many other body sites. Frost was best known as a teacher of cytopathology. He organized and directed a school of cytotechnology and created and led a postgraduate Institute to teach the techniques to physicians.

The American Osteopathic Board of Pathology (AOBPa) is an organization that provides board certification to qualified Doctors of Osteopathic Medicine (D.O.) who specialize in the diagnosis and characterization of disease in patients following thorough examination of biopsies and/or bodily fluids (pathologists).

Leopold George Koss was an American physician, pathologist, and professor at the Albert Einstein College of Medicine. He has been called "one of the founding fathers for the field of cytopathology".

The American Board of Pathology (ABPath) is one of 24 member boards of the American Board of Medical Specialties. This organization was assembled in May 1936, under the approval of the Advisory Board for Medical Specialties (ABMS) and the American Medical Association (AMA) Council on Medical Education and Hospitals. It is the duty of the ABPath to grant certification in Anatomic Pathology, Clinical Pathology and/or Anatomic/Neuropathology to qualified Doctors of Medicine and Doctors of Osteopathic Medicine (M.D./D.O.).
