The 16 teams were split into four groups of four teams. The tournament was played at a single venue with the group winners and the three best second-ranked teams qualified for the final tournament. [2]

The 2016 Men's Water Polo Olympic Qualification Tournament was held in Trieste, Italy, from 3 to 10 April 2016, at the Polo Natatorio "Bruno Bianchi". The top four teams advanced to the Olympics.

On 2 November 2023, it was announced that the tournament would not be hosted in Israel due to the 2023 Israel–Hamas war.
