The play-off round of the 2027 AFC Asian Cup qualification will be played between 5 and 10 September 2024. [1]
When the play-off round was originally announced, a total of ten teams (the 2nd to 10th ranked losing teams from the first round plus the Northern Mariana Islands) would play home-and-away over two legs to determine the final five qualifiers for the third round. [2] [3] However, Guam and the Northern Mariana Islands were not included in the draw for the play-off round announced on 1 May 2024. This resulted in the top three losing teams (Bhutan, Maldives, and Laos) moving directly to the third round, and the remaining six teams entering the play-offs. [4] The play-off winners join the 21 teams which advance directly to the third round (three best ranked losing teams from the first round and 18 teams from the second round) to compete for the final six slots in the 2027 AFC Asian Cup.
The teams participating in this round were decided by their ranking from the preliminary round.

The draw for the play-off round was held on 9 May 2024 in Kuala Lumpur, Malaysia. [4] These six teams were divided into three pairings, with each pair playing home and away between 5 and 10 September 2024. The numbers in parentheses indicate the FIFA Men's World Ranking of April 2024.

The Kuwait national football team is the national team of Kuwait and is controlled by the Kuwait Football Association. Kuwait made one World Cup finals appearance, in 1982, managing one point in the group stages. In the Asian Cup, Kuwait reached the final in 1976 and won the tournament in 1980.
The Jordan national football team represents Jordan in international football and is controlled by the Jordan Football Association. Jordan have never qualified for the World Cup finals but have appeared five times in the Asian Cup and reached the final match of a major tournament for the first time in the 2023 edition, finishing as runners-up for the first time.
The Guam national football team represents Guam, an overseas territory of the United States, in international football and is controlled by the Guam Football Association. They are affiliated with the Asian Football Confederation's East Asian Football Federation region.

Pakistan became a member of FIFA in 1948 joining the Asian Football Confederation and its national team debuted in 1950.
The Northern Mariana Islands national football team represents the Commonwealth of the Northern Mariana Islands in international men's football. The team is controlled by the governing body for football in the Northern Mariana Islands, the Northern Mariana Islands Football Association, which is a member of the East Asian Football Federation (EAFF) and since December 2020 a full member of the Asian Football Confederation (AFC). The association is not a member of the world governing body FIFA, although it had applied for membership, and so while the national team is eligible to enter AFC and EAFF-run competitions, they are currently ineligible for global competitions such as the FIFA World Cup. As such, they do not have an official FIFA ranking. However, the team have been consistently ranked as one of the worst teams in the world on the Elo ratings and were in fact, in July 2016 rated as the worst men's senior international team in the world in a ratings system that also includes a number of other non-FIFA teams.
