He has also appeared in Telugu, Kannada, Hindi, and English-language productions. In a career spanning five decades, he has acted in more than 400 films. He is the recipient of several accolades, including three National Film Awards, eight Kerala State Film Awards, eleven Kerala Film Critics Awards and thirteen Filmfare Awards South. For his contribution to film, the Government of India awarded him the Padma Shri in 1998. In 2022, he was honoured with Kerala Prabha Award, the second-highest honour given by the Government of Kerala.

After his debut, he appeared in an uncredited role in Kaalachakram (1973). In 1979, while he was practising law in Manjeri he was offered a pivotal role in M. T. Vasudevan Nair's Devalokam. However, this film was never released.

Kadha Paranja Kadha is a 2018 Indian Malayalam-language film written and directed by Siju Jawahar, and starring Siddharth Menon, Tharushi, Shaheen Siddique, Renji Panicker, and Dileesh Pothan. The film features music composed by Sithara Krishnakumar and Jaison J. Nair. The film was released on 9 February 2018 in theatres and digital platform in 2021 by Baburaj Asariya through his film production & distribution house Collective Frames. Oru Yamandan Premakadha is a 2019 Indian Malayalam-language comedy-drama spoof film directed by B. C. Noufal and written by Bibin George and Vishnu Unnikrishnan. It stars Dulquer Salmaan, Nikhila Vimal, Soubin Shahir, Salim Kumar and Vishnu Unnikrishnan in the lead roles. The film's production began on 3 July 2017 and was completely shot in Kochi. It was released on 25 April 2019.

The film was released on 3 February 2023 in theatres and streaming on the OTT ZEE5 from 16 April 2023. Film received mixed reviews from both critics and audience alike praising the performance of Vishnu Unnikrishnan and Bibin George.
