The corduroy sign is a radiological finding observed on spinal imaging in cases of vertebral hemangiomas. It refers to a striated or vertically oriented linear pattern seen on imaging, resembling the appearance of corduroy fabric. [1] This sign is most commonly identified on lateral radiographs or magnetic resonance imaging (MRI) of the spine and is an important diagnostic marker for benign conditions such as vertebral hemangiomas. Patients with the typical ‘corduroy appearance’ is extremely rare clinically. [2]
Vertebral hemangiomas are benign vascular tumors of the vertebral bodies. They arise due to a proliferation of thin-walled blood vessels within the bone and are often asymptomatic. The corduroy sign reflects the characteristic trabecular changes caused by the hemangioma, namely, thinning of trabeculae as well as thickened vertical struts, resulting from compensatory hypertrophy of the remaining trabeculae to maintain structural integrity. These vertical trabecular striations give rise to the "corduroy" appearance on imaging. [3]
On lateral radiographs and sagittal CT of the spine, the corduroy sign appears as vertical linear striations, which are alternating bands of radiolucency (representing vascular spaces) and radiodensity (representing thickened trabeculae) within the vertebral body. The vertebral body may retain its normal shape and size unless the lesion becomes aggressive. [4]
An upper gastrointestinal series, also called a barium swallow, barium study, or barium meal, is a series of radiographs used to examine the gastrointestinal tract for abnormalities. A contrast medium, usually a radiocontrast agent such as barium sulfate mixed with water, is ingested or instilled into the gastrointestinal tract, and X-rays are used to create radiographs of the regions of interest. The barium enhances the visibility of the relevant parts of the gastrointestinal tract by coating the inside wall of the tract and appearing white on the film. This in combination with other plain radiographs allows for the imaging of parts of the upper gastrointestinal tract such as the pharynx, larynx, esophagus, stomach, and small intestine such that the inside wall lining, size, shape, contour, and patency are visible to the examiner. With fluoroscopy, it is also possible to visualize the functional movement of examined organs such as swallowing, peristalsis, or sphincter closure. Depending on the organs to be examined, barium radiographs can be classified into "barium swallow", "barium meal", "barium follow-through", and "enteroclysis". To further enhance the quality of images, air or gas is sometimes introduced into the gastrointestinal tract in addition to barium, and this procedure is called double-contrast imaging. In this case the gas is referred to as the negative contrast medium. Traditionally the images produced with barium contrast are made with plain-film radiography, but computed tomography is also used in combination with barium contrast, in which case the procedure is called "CT enterography".
A trabecula is a small, often microscopic, tissue element in the form of a small beam, strut or rod that supports or anchors a framework of parts within a body or organ. A trabecula generally has a mechanical function, and is usually composed of dense collagenous tissue. It can be composed of other material such as muscle and bone. In the heart, muscles form trabeculae carneae and septomarginal trabeculae, and the left atrial appendage has a tubular trabeculated structure.

A compression fracture is a collapse of a vertebra. It may be due to trauma or due to a weakening of the vertebra. This weakening is seen in patients with osteoporosis or osteogenesis imperfecta, lytic lesions from metastatic or primary tumors, or infection. In healthy patients, it is most often seen in individuals suffering extreme vertical shocks, such as ejecting from an ejection seat. Seen in lateral views in plain x-ray films, compression fractures of the spine characteristically appear as wedge deformities, with greater loss of height anteriorly than posteriorly and intact pedicles in the anteroposterior view.
An osteoid osteoma is a benign (non-cancerous) bone tumor that arises from osteoblasts and some components of osteoclasts. It was originally thought to be a smaller version of an osteoblastoma. Osteoid osteomas tend to be less than 1.5 cm in size. The tumor can be in any bone in the body but are most common in long bones, such as the femur and tibia. They account for 10 to 12 percent of all benign bone tumors and 2 to 3 percent of all abnormal bone growths. A hemangioma or haemangioma is a usually benign vascular tumor derived from blood vessel cell types. The most common form, seen in infants, is an infantile hemangioma, known colloquially as a "strawberry mark", most commonly presenting on the skin at birth or in the first weeks of life. A hemangioma can occur anywhere on the body, but most commonly appears on the face, scalp, chest or back. They tend to grow for up to a year before gradually shrinking as the child gets older. A hemangioma may need to be treated if it interferes with vision or breathing or is likely to cause long-term disfigurement. In rare cases internal hemangiomas can cause or contribute to other medical problems. They usually disappear by 10 years of age. The first line treatment option is beta blockers, which are highly effective in the majority of cases. Hemangiomas present at birth are called congenital hemangiomas, while those that form later in life are called infantile hemangiomas.
An occult fracture is a fracture that is not readily visible, generally in regard to projectional radiography ("X-ray"). Radiographically, occult and subtle fractures are a diagnostic challenge. They may be divided into 1) high energy trauma fracture, 2) fatigue fracture from cyclical and sustained mechanical stress, and 3) insufficiency fracture occurring in weakened bone. Independently of the cause, the initial radiographic examination can be negative either because the findings seem normal or are too subtle. Advanced imaging tools such as computed tomography, magnetic resonance imaging (MRI), and scintigraphy are highly valuable in the early detection of these fractures.
Vertebral hemangiomas or haemangiomas (VHs) are a common vascular lesion found within the vertebral body of the thoracic and lumbar spine. These are predominantly benign lesions that are often found incidentally during radiology studies for other indications and can involve one or multiple vertebrae. Vertebral hemangiomas are a common etiology estimated to be found in 10-12% of humans at autopsy. They are benign in nature and frequently asymptomatic. Symptoms, if they do occur, are usually related to large hemangiomas, trauma, the hormonal and hemodynamic changes of pregnancy (causing intra-spinal bleeding), or osseous expansion and extra-osseous extension into surround soft tissues or epidural region of the spinal canal.
Codfish vertebra refers to the biconcave appearance of the vertebra in sagittal radiographs due to pathological changes, such as demineralisation. Codfish appearance of the vertebra is seen in several conditions such as osteoporosis, steroid or heparin therapy, Cushing syndrome, idiopathic, sickle cell disease, leukemia, Duchenne muscular dystrophy, and homo-cystinuria. Codfish vertebra sign is usually first seen in lumbar vertebrae.
