An infusion set is used with devices such as an insulin pump. The purpose of an infusion set is to deliver insulin under the skin, fulfilling a similar function like an intravenous line. [1] It is a complete tubing system to connect an insulin pump to the pump user and as such includes a subcutaneous cannula, adhesive mount, quick-disconnect, and a pump cartridge connector.[ citation needed ]

There are several types, styles, and sizes of infusion sets available, varying in cannula length, entry angle and in the length of tube connecting the cannula to the pump. The kind of choice of these options depends on a variety of factors, such as the patient's body fat percentage.

Functionality

Insertion devices

Some pump users prefer to use an insertion device to insert their sets, rather than pushing the needle in by hand. These are spring-loaded and are designed to push the needle into the skin quickly. The user simply loads the primed set into the insertion device, peels off the paper protecting the adhesive pad, places the device against the skin and presses a button. This may be an advantage for those with needle phobia.

Sites for infusion sets

Infusion sets are most commonly placed on the abdomen, in a roughly semicircular area around and below the navel. Other sites include the upper leg, upper buttocks, hips, upper arms and lower back. Insulin absorption may vary from site to site, and therefore medical advice is required.

Rotation of sites

The infusion set is replaced regularly, usually every 2–3 days. Insulin absorption becomes less effective the longer the set is left in place, leading to poorer control of blood glucose. For this reason, the site of the infusion set is moved when the set is changed. Often a number of favorite sites for the infusion set are used on a rotation basis.

Disconnecting the set

The set can be disconnected from the pump and tubing with a quick-release that leaves the cannula and adhesive pad in place. This is convenient when swimming or showering (since most pumps are not fully waterproof), or when engaging in any activity when it is not desirable to be attached to the pump.

A central venous catheter (CVC), also known as a central line(c-line), central venous line, or central venous access catheter, is a catheter placed into a large vein. It is a form of venous access. Placement of larger catheters in more centrally located veins is often needed in critically ill patients, or in those requiring prolonged intravenous therapies, for more reliable vascular access. These catheters are commonly placed in veins in the neck, chest, groin, or through veins in the arms.

A syringe is a simple reciprocating pump consisting of a plunger that fits tightly within a cylindrical tube called a barrel. The plunger can be linearly pulled and pushed along the inside of the tube, allowing the syringe to take in and expel liquid or gas through a discharge orifice at the front (open) end of the tube. The open end of the syringe may be fitted with a hypodermic needle, a nozzle or tubing to direct the flow into and out of the barrel. Intravenous therapy is a medical technique that administers fluids, medications and nutrients directly into a person's vein. The intravenous route of administration is commonly used for rehydration or to provide nutrients for those who cannot, or will not—due to reduced mental states or otherwise—consume food or water by mouth. It may also be used to administer medications or other medical therapy such as blood products or electrolytes to correct electrolyte imbalances. A winged infusion set—also known as "butterfly" or "scalp vein" set—is a device specialized for venipuncture: i.e. for accessing a superficial vein or artery for either intravenous injection or phlebotomy. An insulin port functions as a medication delivery channel directly into the subcutaneous tissue. When applying the injection port, an insertion needle guides a soft cannula under the skin. Once applied, the insertion needle is removed and only the soft cannula remains below the skin, acting as the gateway into the subcutaneous tissue.

An injection port is usually a disposable device applied by the patient and worn for period of 3–5 days. When giving shots via an injection port, the needle stays above the surface of the skin. Medication is delivered via a short soft cannula. An injection port can be used in conjunction with multiple daily injections of insulin by people with diabetes. <span class="mw-page-title-main">Minimed Paradigm</span> Insulin pumps

MiniMed Paradigm is a series of insulin pumps manufactured by Medtronic for patients with diabetes mellitus. The pump operates with a single AA battery and uses a piston-plunger pump to infuse a programmed amount of insulin into the patient through a length of tubing. The Paradigm uses a one-way wireless radio frequency link to receive blood sugar measurements from select glucose meters. The Paradigm RT series adds the ability to receive data from a mated continuous blood-glucose monitor. Although the pump can use these measurements to assist in calculating a dose of insulin, no actual change in insulin delivery occurs without manual user-intervention.

<span class="mw-page-title-main">Injector pen</span> Drug storage and delivery device

An injector pen is a device used for injecting medication under the skin. First introduced in the 1980s, injector pens are designed to make injectable medication easier and more convenient to use, thus increasing patient adherence. The primary difference between injector pens and traditional vial and syringe administration is the easier use of an injector pen by people with low dexterity, poor vision, or who need portability to administer medicine on time. Injector pens also decrease the fear or adversity towards self-injection of medications, which increases the likelihood that a person takes the medication.
