Jurong Health Connect

Jurong Health Connect is a community health project started in Jurong, Singapore.

The project was initiated by then Acting Minister of Health Khaw Boon Wan, and was officially launched on 3 July 2004. Tan Cheng Bock served as the Chairman of the Jurong Health Connect Advisory Committee. [1] The aim is to develop a patient-centric, seamless, and accessible healthcare network for the residents in Jurong. Successful components of the pilot would then be replicated in other parts of Singapore.

One aspect of the initiative was a community web portal [2] that serves as a comprehensive hub for healthcare-related information in the Jurong area. [3] Beyond serving as a directory, the portal also provides self-administered questionnaires that recommend resources tailored to a resident's medical conditions and daily needs. [3] The web portal is supported by an information counter and a call centre hotline for residents who are less familiar with technology. [3]

In response to the problem of inadequate healthcare facilities for the elderly, a group of 8 churches and Christian organizations including the Graduate Christian Fellowship and the Singapore Nurses' Christian Fellowship came together to set up St Luke's Hospital for the Elderly Sick. St Luke's Hospital is headed by a board of directors and various committees that oversee the daily operations of the hospital.
