Holby City is a British medical drama television series that aired weekly on BBC One. It was created by Tony McHale and Mal Young as a spin-off from the established BBC medical drama Casualty, and premiered on 12 January 1999; the show ran until 29 March 2022. It follows the lives of medical and ancillary staff at the fictional Holby City Hospital, the same hospital as Casualty, in the fictional city of Holby, and features occasional crossovers of characters and plots with both Casualty and the show's 2007 police procedural spin-off HolbyBlue. It began with eleven main characters in its first series, all of whom subsequently left the show. New main characters were then periodically written in and out, with a core of around fifteen main actors employed at any given time. In casting the first series, Young sought actors who were already well known in the television industry, something which has continued throughout its history, with cast members including Patsy Kensit, Jane Asher, Robert Powell, Ade Edmondson and John Michie.
Jac Naylor is a fictional character from the BBC medical drama Holby City, portrayed by Rosie Marcel. She made her first screen appearance during the series eight episode "Bird on a Wire", which was originally broadcast on 22 November 2005. The character is introduced alongside Luke Roberts and Tom Chambers as Joseph Byrne and Sam Strachan, respectively. Jac is characterised as a highly ambitious, forthright surgeon who is career-oriented. Her backstory states that her mother placed her in foster care aged twelve.

Sacha Levy is a fictional character from the BBC One medical drama Holby City, played by Bob Barrett. He made his first appearance in the twelfth series episode "Talk to Me", broadcast on 5 January 2010. Sacha joined the Keller ward staff as a specialist registrar in general surgery. Sacha is the father of long-serving character Chrissie Williams's infant son, Daniel, as a result of a one-night stand, and has since held unreciprocated feelings for her. Since Chrissie's departure in 2013, Sacha's storylines have seen a relationship with Essie Harrison and a battle with depression. Barrett has reprised the role of Sacha in sister show Casualty in 2019 and 2023.

The character was one of multiple characters introduced following a spate of cast exits. Sahira is employed at Holby City Hospital as a Cardiothoracic surgical registrar. Kate Oates is a British television producer, who was born in Nottingham and graduated from Warwick University. She began her career working as a researcher and editorial assistant for Germaine Greer, before becoming a script editor for the soap opera Crossroads. In 2003, she joined the radio soap The Archers as a producer, a position which Oates credits as the majority of her training. After resigning from this job, Oates began working on the ITV soap opera Emmerdale as a script editor, during which time she assisted with the soap's fortieth anniversary celebrations and worked on British Soap Award-winning storylines. She remained in the position until 2012 when she joined Coronation Street as the assistant producer.

Writers worked alongside a consultant oncologist for medical advice to provide an accurate depiction of terminal cancer. The episode was written by Joe Ainsworth and directed by Jan Bauer, and premiered on BBC One on 19 September 2017. The episode features the wedding of established characters Zosia March and Oliver Valentine, who both work on the show's fictitious Darwin ward. It formed the first of a two-part collective of episodes leading up to Arfwedson's departure from the series. The episode also featured Zosia's father Guy Self ruining her wedding day, getting drunk and falling on a shard of glass. Other characters battle in the operating theatre to save his life. While registrar Dominic Copeland and nurse Ben "Lofty" Chiltern sleep together forming part of their ongoing gay storyline.

She is known for playing the role of Rachel Breckle in Emmerdale from 2011 to 2015, for which she received a nomination for the British Soap Award for Best Newcomer. Since leaving Emmerdale, Oaten has made appearances in various television series and films, including Rise of the Footsoldier: The Pat Tate Story, Holby City and Coronation Street. As well as her onscreen roles, Oaten has also starred in various theatre productions across the UK, including stints in Bedroom Farce and Up 'n' Under.
