Rating of perceived exertion

In sports, health, and exercise testing, the rating of perceived exertion (RPE), as measured by the Borg rating of perceived exertion scale, [1] [2] [3] is a quantitative measure of perceived exertion during physical activity. [3] [4] [5] [6]

In medicine, this is used to document the patient's exertion during a test for the severity of diseases. Sports coaches use the scale to assess the intensity of training and competition as well as endurance. The original scale introduced by Gunnar Borg rated exertion on a scale of 6-20. Borg then constructed a newer category-ratio scale, the Borg CR-10 scale, rated on a scale from 1-10. This is especially used in clinical diagnosis and severity assessment of breathlessness and dyspnea, chest pain, angina and musculo-skeletal pain. The CR-10 scale is best suited when there is an overriding sensation arising either from a specific area of the body rather than overall exertion, for example, muscle pain, ache or fatigue in the quadriceps or from pulmonary responses during exertion.

The Borg scale can be compared to other linear scales such as the Likert scale or a visual analogue scale. The sensitivity and reproducibility of the results are broadly very similar, although the Borg scale may outperform the Likert scale in some cases. [7]

Borg RPE scale

The Borg RPE scale is a numerical scale that ranges from 6 to 20, [8] where 6 means "no exertion at all" and 20 means "maximal exertion." When a measurement is taken, a number is chosen from the following scale by an individual that best describes their perceived level of exertion during physical activity.

The scale was constructed to roughly correlate to 10% of heart rate in a healthy 20-year-old. [8] In older individuals, the correlation becomes higher than 10% at the high-end of the scale, as maximum heart rate declines with age.

Borg RPE scale and the external environment

It appears that outdoor enternal environments can lower the perceived exertion that a given exercise is connected to in laboratory or other indoor conditions. [9]

Pilates is a type of mind-body exercise developed in the early 20th century by German physical trainer Joseph Pilates, after whom it was named. Pilates called his method "Contrology". It is practiced worldwide, especially in developed countries such as Australia, Canada, South Korea, the United Kingdom and the United States. Pilates uses a combination of around 50 repetitive exercises to spur muscle exertion. Each exercise flows from the "five essentials": breath, cervical alignment, rib and scapular stabilization, pelvic mobility, and utilization of the transversus abdominis. Each exercise is typically repeated three to five times. As of 2023, over 12 million people practice Pilates.

Delayed onset muscle soreness (DOMS) is the pain and stiffness felt in muscles after unaccustomed or strenuous exercise. The soreness is felt most strongly 24 to 72 hours after the exercise. It is thought to be caused by eccentric (lengthening) exercise, which causes small-scale damage (microtrauma) to the muscle fibers. After such exercise, the muscle adapts rapidly to prevent muscle damage, and thereby soreness, if the exercise is repeated.

V̇O2 max (also maximal oxygen consumption, maximal oxygen uptake or maximal aerobic capacity) is the maximum rate of oxygen consumption attainable during physical exertion. The name is derived from three abbreviations: "V̇" for volume (the dot over the V indicates "per unit of time" in Newton's notation), "O2" for oxygen, and "max" for maximum and usually normalized per kilogram of body mass. A similar measure is V̇O2 peak (peak oxygen consumption), which is the measurable value from a session of physical exercise, be it incremental or otherwise. It could match or underestimate the actual V̇O2 max. Confusion between the values in older and popular fitness literature is common. The capacity of the lung to exchange oxygen and carbon dioxide is constrained by the rate of blood oxygen transport to active tissue.

Overtraining occurs when a person exceeds their body's ability to recover from strenuous exercise. Overtraining can be described as a point where a person may have a decrease in performance and plateauing as a result of failure to consistently perform at a certain level or training load; a load which exceeds their recovery capacity. People who are overtrained cease making progress, and can even begin to lose strength and fitness. Overtraining is also known as chronic fatigue, burnout and overstress in athletes. It is suggested that there are different variations of overtraining, firstly monotonous program over training suggest that repetition of the same movement such as certain weight lifting and baseball batting can cause performance plateau due to an adaption of the central nervous system which results from a lack of stimulation. A second example of overtraining is described as chronic overwork type training where the subject may be training with too high intensity or high volume and not allowing sufficient recovery time for the body. Up to 10% of elite endurance athletes and 10% of American college swimmers are affected by overtraining syndrome.

High-intensity interval training (HIIT) is a training protocol alternating short periods of intense or explosive anaerobic exercise with brief recovery periods until the point of exhaustion. HIIT involves exercises performed in repeated quick bursts at maximum or near maximal effort with periods of rest or low activity between bouts. The very high level of intensity, the interval duration, and number of bouts distinguish it from aerobic (cardiovascular) activity, because the body significantly recruits anaerobic energy systems. Excess post-exercise oxygen consumption is a measurably increased rate of oxygen intake following strenuous activity. In historical contexts the term "oxygen debt" was popularized to explain or perhaps attempt to quantify anaerobic energy expenditure, particularly as regards lactic acid/lactate metabolism; in fact, the term "oxygen debt" is still widely used to this day. However, direct and indirect calorimeter experiments have definitively disproven any association of lactate metabolism as causal to an elevated oxygen uptake.

The metabolic equivalent of task (MET) is the objective measure of the ratio of the rate at which a person expends energy, relative to the mass of that person, while performing some specific physical activity compared to a reference, currently set by convention at an absolute 3.5 mL of oxygen per kg per minute, which is the energy expended when sitting quietly by a reference individual, chosen to be roughly representative of the general population, and thereby suited to epidemiological surveys. Exercise-induced bronchoconstriction (EIB) occurs when the airways narrow as a result of exercise. This condition has been referred to as exercise-induced asthma (EIA); however, this term is no longer preferred. While exercise does not cause asthma, it is frequently an asthma trigger.

Exertion is the physical or perceived use of energy. Exertion traditionally connotes a strenuous or costly effort, resulting in generation of force, initiation of motion, or in the performance of work. It often relates to muscular activity and can be quantified, empirically and by measurable metabolic response.

The central governor is a proposed process in the brain that regulates exercise in regard to a neurally calculated safe exertion by the body. In particular, physical activity is controlled so that its intensity cannot threaten the body’s homeostasis by causing anoxic damage to the heart muscle. The central governor limits exercise by reducing the neural recruitment of muscle fibers. This reduced recruitment causes the sensation of fatigue. The existence of a central governor was suggested to explain fatigue after prolonged strenuous exercise in long-distance running and other endurance sports, but its ideas could also apply to other causes of exertion-induced fatigue.

Human locomotion is considered to take two primary forms: walking and running. In contrast, many quadrupeds have three distinct forms of locomotion: walk, trot, and gallop. Walking is a form of locomotion defined by a double support phase when both feet are on the ground at the same time. Running is a form of locomotion that does not have this double support phase.

Fitness instructors rely heavily on the use of music during their class as a way of motivating their clients. In addition to making physical activity and exercise more enjoyable, athletes have used music as an ergogenic aid. Most of the studies that have explored the effects of music on performance was aerobic performance. Aerobic performance is assessed by measuring specific parameters of, such as maximal oxygen consumption (VO2 max), heart rate (HR), rate of perceived exertion (RPE), and blood lactate (mmol/ L), power output (W). There is conflicting data regarding the effects of music on aerobic performance. On one hand, studies have suggested that music does increase aerobic exercise performance by influencing certain parameters, such as rate of perceived exertion and time to exhaustion. However, there are studies that rebuke this notion, stating that music had no effect on aerobic performance. The reason for this disparity among data is the construction of the tests themselves. There are several factors that need to be taken into account when exploring the effects of music on exercise. Those include: exercise intensity, the subject experience in performing exercise, and type of music.

Central nervous system fatigue, or central fatigue, is a form of fatigue that is associated with changes in the synaptic concentration of neurotransmitters within the central nervous system which affects exercise performance and muscle function and cannot be explained by peripheral factors that affect muscle function. In healthy individuals, central fatigue can occur from prolonged exercise and is associated with neurochemical changes in the brain, involving serotonin (5-HT), noradrenaline, and dopamine. The roles of dopamine, noradrenaline, and serotonin in CNS fatigue are unclear, as pharmacological manipulation of these systems has yielded mixed results. Central fatigue plays an important role in endurance sports and also highlights the importance of proper nutrition in endurance athletes.

Exertional rhabdomyolysis (ER) is the breakdown of muscle from extreme physical exertion. It is one of many types of rhabdomyolysis that can occur, and because of this, the exact prevalence and incidence are unclear.
