The Self-Injurious Behavior Inhibiting System (SIBIS) is an apparatus designed to reduce self-injurious behavior (SIB) directed at the head, such as banging the head against walls and other objects or hitting oneself in the head. Invented by Dr. Robert E. Fischell, Glen H. Fountain, and Charles M. Blackburn in 1984, the device is able to detect instances of head-directed SIB, and delivers an aversive electric shock contingent on its occurrence. [1] The United States Food and Drug Administration banned the device in 2020 as part of a larger blanket ban on devices that use electric shocks to modify behavior without the consent of the user. Other devices covered by this ban include the Graduated Electronic Decelerator. [2]
The United States Food and Drug Administration (FDA) has concluded that aversive conditioning devices like the SIBIS pose an "unreasonable risk of harm" and are less effective than positive behavior support alone. While some studies appear to support their efficacy, these studies do no meet modern standards of evidence. Newer and better designed studies find more risks and fewer benefits. [3] The FDA states that such devices may cause both physical and psychological harm, including: depression, anxiety, worsening of self-injurious behavior, PTSD, burns, and pain. [4]
There are two models of SIBIS. The simpler model consists of an electrode and a radio transmitter wrapped around the arm or leg using Velcro. [5] When a child administers a blow to the head, the SIBIS device is used to recognize the self-injurious behavior. This is possible because the SIBIS device is composed of two wirelessly connected parts: the "sensor module" and the "stimulus module". [1] The impact monitor serves to both detect an impact to the head and to protect the head from the damage that the impact could potentially incur. The sensor module is placed on either the body part receiving the impact (such as the head) or on the body part delivering the impact (such as the arm or knee). Wherever it is placed, the sensor module senses the impact of the blow and sends out an electrical signal. This electrical signal triggers the stimulus module, allowing for the aversive stimulation, the shock, to be delivered.
SIBIS is designed to reduce SIB by immediately delivering positive punishment when head-directed SIB occurs. Only 5 cm × 3 cm × 1 cm in size, [6] the stimulus module delivers an 85 V electrical shock at 3.5 mA of current to the subject each time the patient strikes his or her head sufficiently hard enough to register on the velocity impact detector. [7] The delivered shock is designed not to be very painful, but rather an uncomfortable response to the SIB. The impact detector of the apparatus can be adjusted. [6]
The SIBIS was used at the Judge Rotenberg Center between 1988 and 1990 as part of the school's behavior modification program. Behaviorists at the center used the SIBIS not only for the purpose of reducing self-harming behavior, but to deter any behavior that they considered undesirable. The school's behaviorists were not satisfied with the SIBIS because its shock was not powerful enough to produce compliance some cases: The school's founder, Matthew Israel, reported that one student was shocked by the SIBIS over 5000 times in a day without the desired change in behavior. Israel asked the manufacturer of the SIBIS, Human Technologies, to build a device that delivered stronger shocks, but they refused. Israel then designed the Graduated Electronic Decelerator (GED), which delivered much more powerful shocks than the SIBIS that lasted ten times as long. The SIBIS was soon phased out at the Judge Rotenberg Center in favor of the GED. [8]
Behaviorists
Burrhus Frederic Skinner was an American psychologist, behaviorist, author, inventor, and social philosopher. Considered the father of Behaviorism, he was the Edgar Pierce Professor of Psychology at Harvard University from 1958 until his retirement in 1974.
Operant conditioning, also called instrumental conditioning, is a learning process where behaviors are modified through the association of stimuli with reinforcement or punishment. In it, operants—behaviors that affect one's environment—are conditioned to occur or not occur depending on the environmental consequences of the behavior.
Electrical injury is a physiological reaction caused by electric current passing through the body. The injury depends on the density of the current, tissue resistance and duration of contact. Very small currents may be imperceptible or produce a light tingling sensation. A shock caused by low and otherwise harmless current could startle an individual and cause injury due to jerking away or falling. Stronger currents may cause some degree of discomfort or pain, while more intense currents may induce involuntary muscle contractions, preventing the person from breaking free of the source of electricity. Still larger currents result in tissue damage and may trigger ventricular fibrillation or cardiac arrest. Consequences of injury from electricity may include amputations, bone fractures and orthopedic and musculoskeletal injuries. If death results from an electric shock the cause of death is generally referred to as electrocution.
Aversion therapy is a form of psychological treatment in which the patient is exposed to a stimulus while simultaneously being subjected to some form of discomfort. This conditioning is intended to cause the patient to associate the stimulus with unpleasant sensations with the intention of quelling the targeted behavior.
Cranial electrotherapy stimulation (CES) is a form of neurostimulation that delivers a small, pulsed, alternating current via electrodes on the head. CES is used with the intention of treating a variety of conditions such as anxiety, depression and insomnia. CES has been suggested as a possible treatment for headaches, fibromyalgia, smoking cessation, and opiate withdrawal, but there is little evidence of effectiveness for many of these conditions and the evidence for use in acute depression is not sufficient to justify it.
In psychology, aversives are unpleasant stimuli that induce changes in behavior via negative reinforcement or positive punishment. By applying an aversive immediately before or after a behavior the likelihood of the target behavior occurring in the future is reduced. Aversives can vary from being slightly unpleasant or irritating to physically, psychologically and/or emotionally damaging. It is not the level of unpleasantness or intention that defines something as an aversive, but rather the level of effectiveness the unpleasant event has on changing (decreasing) behavior.
Dog training is a kind of animal training, the application of behavior analysis which uses the environmental events of antecedents and consequences to modify the dog behavior, either for it to assist in specific activities or undertake particular tasks, or for it to participate effectively in contemporary domestic life. While training dogs for specific roles dates back to Roman times at least, the training of dogs to be compatible household pets developed with suburbanization in the 1950s.
Applied behavior analysis (ABA), also called behavioral engineering, is a psychological intervention that applies empirical approaches based upon the principles of respondent and operant conditioning to change behavior of social significance. It is the applied form of behavior analysis; the other two forms are radical behaviorism and the experimental analysis of behavior.
The Association for Behavior Analysis International (ABAI) is a nonprofit organization dedicated to promoting behavior analysis. The organization has over 9,000 members. The group organizes conferences and publishes journals on the topic of applied behavior analysis (ABA). ABAI has issued detailed, specific position papers intended to guide practitioners of ABA. The ABAI publishes six scholarly journals including The Psychological Record and their primary organ, Perspectives on Behavior Science, formerly The Behavior Analyst. They also publish an informational journal, Education and Treatment of Children, describing practical treatment of children with behavioral problems.
In operant conditioning, punishment is any change in a human or animal's surroundings which, occurring after a given behavior or response, reduces the likelihood of that behavior occurring again in the future. As with reinforcement, it is the behavior, not the human/animal, that is punished. Whether a change is or is not punishing is determined by its effect on the rate that the behavior occurs. This is called motivating operations (MO), because they alter the effectiveness of a stimulus. MO can be categorized in abolishing operations, decrease the effectiveness of the stimuli and establishing, increase the effectiveness of the stimuli. For example, a painful stimulus which would act as a punisher for most people may actually reinforce some behaviors of masochistic individuals.
A shock collar or remote training collar, also known as an e-collar, Ecollar, or electronic collar) is a type of training collar that utilizes electrical shocks to induce pain to the neck of a dog or other parts of its body, depending on the desired outcome. These collars incorporate a radio-controlled electronic device and are worn around the dog's neck. Many European countries view shock collars as animal cruelty and have banned their use. The mechanism behind shock collars involve inflicting varying levels and duration of pain, which generates fear and serves as a deterrent for undesirable behaviors. Some models of shock collar models offer additional features such as a tone or vibrational setting that can be used as an alternative or in combination with the shock. Certain advanced collars include Internet mapping capabilities and GPS functionality to track the dog's location or notify the owner about its whereabouts.
Conditioned place preference (CPP) is a form of Pavlovian conditioning used to measure the motivational effects of objects or experiences. This motivation comes from the pleasurable aspect of the experience, so that the brain can be reminded of the context that surrounded the "encounter". By measuring the amount of time an animal spends in an area that has been associated with a stimulus, researchers can infer the animal's liking for the stimulus. This paradigm can also be used to measure conditioned place aversion with an identical procedure involving aversive stimuli instead. Both procedures usually involve mice or rats as subjects. This procedure can be used to measure extinction and reinstatement of the conditioned stimulus. Certain drugs are used in this paradigm to measure their reinforcing properties. Two different methods are used to choose the compartments to be conditioned, and these are biased vs. unbiased. The biased method allows the animal to explore the apparatus, and the compartment they least prefer is the one that the drug is administered in and the one they most prefer is the one where the vehicle is injected. This method allows the animal to choose the compartment they get the drug and vehicle in. In comparison, the unbiased method does not allow the animal to choose what compartment they get the drug and vehicle in and instead the researcher chooses the compartments.
Ari Daniel Ne'eman is an American disability rights activist and researcher who co-founded the Autistic Self Advocacy Network in 2006. On December 16, 2009, President Barack Obama announced that Ne'eman would be appointed to the National Council on Disability. After an anonymous hold was lifted, Ne'eman was unanimously confirmed by the United States Senate to serve on the Council on June 22, 2010. He chaired the council's Policy & Program Evaluation Committee making him the first autistic person to serve on the council. In 2015, Ne'eman left the National Council on Disability at the end of his second term. He currently serves as a consultant to the American Civil Liberties Union. As of 2019, he also is a Ph.D. candidate in Health Policy at Harvard University.
Pain in invertebrates is a contentious issue. Although there are numerous definitions of pain, almost all involve two key components. First, nociception is required. This is the ability to detect noxious stimuli which evokes a reflex response that moves the entire animal, or the affected part of its body, away from the source of the stimulus. The concept of nociception does not necessarily imply any adverse, subjective feeling; it is a reflex action. The second component is the experience of "pain" itself, or suffering—i.e., the internal, emotional interpretation of the nociceptive experience. Pain is therefore a private, emotional experience. Pain cannot be directly measured in other animals, including other humans; responses to putatively painful stimuli can be measured, but not the experience itself. To address this problem when assessing the capacity of other species to experience pain, argument-by-analogy is used. This is based on the principle that if a non-human animal's responses to stimuli are similar to those of humans, it is likely to have had an analogous experience. It has been argued that if a pin is stuck in a chimpanzee's finger and they rapidly withdraw their hand, then argument-by-analogy implies that like humans, they felt pain. It has been questioned why the inference does not then follow that a cockroach experiences pain when it writhes after being stuck with a pin. This argument-by-analogy approach to the concept of pain in invertebrates has been followed by others.
Functional Behavior Assessment (FBA) is an ongoing process of collecting information with a goal of identifying the environmental variables that control a problem or target behavior. The purpose of the assessment is to prove and aid the effectiveness of the interventions or treatments used to help eliminate the problem behavior. Through functional behavior assessments, we have learned that there are complex patterns to people's seemingly unproductive behaviors. It is important to not only pay attention to consequences that follow the behavior but also the antecedent that evokes the behavior. More work needs to be done in the future with functional assessment including balancing precision and efficiency, being more specific with variables involved and a more smooth transition from assessment to intervention.

They are the chairperson of the American Bar Association Civil Rights & Social Justice Disability Rights Committee. They are also Policy Counsel for Privacy & Data at the Center for Democracy & Technology, and Director of Policy, Advocacy, & External Affairs at the Autistic Women & Nonbinary Network. In 2022, they unsuccessfully ran for the Maryland House of Delegates in District 7A, losing to state delegate Kathy Szeliga and delegate-elect Ryan Nawrocki.

The graduated electronic decelerator (GED) is a device that delivers powerful electric shocks to the skin, described by the United Nations as torture, created by Matthew Israel for use on people at the Judge Rotenberg Center as part of the institution's behavior modification program. The school has since been condemned for torture by the United Nations special rapporteur on torture for its use of the GED and other inhumane punishments. In 2020, the device was banned in the United States by the Food and Drug Administration, however the ban was overturned in federal court a year later. In response, congress amended the Food, Drug, and Cosmetic Act to expand FDA’s authority to ban such devices.
Matthew Israel is a controversial American behavioral psychologist who is known for founding the Judge Rotenberg Center and inventing the Graduated Electronic Decelerator. As a freshman in college, Israel read B. F. Skinner's novel, Walden Two, in which the heroes build a utopia by conditioning the residents of a commune through the behaviorist principles of reward and punishment. Israel said that reading Walden Two was like a "religious conversion" for him. After reading the book, Israel devoted himself to making the utopia described in it a reality. Israel said that the period between reading the book and his first attempts to start the utopia was a very difficult time of his life. "I thought about committing suicide," he said. "If I couldn't bring a community into existence, [in] what sense was life worth living?" Israel went on to study behaviorism under B. F. Skinner and to receive his Ph.D. from Harvard University in 1960.
