The sitting-rising test is a clinical test which provides a significant and efficient prediction of mortality risk in the elderly. It was initially developed by Brazilian researchers in exercise physiology and sports medicine in the 1990s. The test involves sitting on the floor, then returning to a standing position from the floor. Results are scored out of ten total points, divided between the two tasks. [1]

A 2012 study with sex- and age-reference sitting-rising test (SRT) scores derived from 6141 adults appeared in the European Journal of Preventive Cardiology with other evaluation charts in the supplemental materials. [2]

Procedure

The test itself is simple. Subjects are instructed by the evaluator to, "without worrying about the speed of movement, try to sit and then to rise from the floor, using the minimum support that you believe is needed." [2]

Scoring

The maximum possible score on the SRT is 10 points: a possible total of 5 points for sitting down, and 5 points for rising from the floor to a standing position. Use of a hand, forearm, knee, or the side of the leg to press up from the ground—or bracing a hand on the knee—each result in a deduction of one point. The minimum possible score is 0 points. An additional 0.5 points is deducted if the evaluator perceives an unsteady execution or partial loss of balance. If the subject loses points on the first few attempts, the evaluator provides advice to help them improve their score on subsequent tries. The best scores for sitting and rising are used to determine the final score. [2]

Outcomes

A 2016 study, which aimed to assess the utility of the test for patients recovering from strokes, found that age was strongly associated with particular scores. This same study suggested 7.8 as a general cutoff score when differentiating between healthy older adults and those struggling with chronic stroke symptoms. [3]

In another study of subjects between the ages of 51 and 80, those with scores in the lowest range (0 to 3) were 5–6 times more likely to die within the study period (about 6 years) than those in the group with the highest scores (8 to 10). [2] Contrary to the suggestions of some headlines which imply that the test can predict mortality across all age groups in this way, [4] that broad of a claim has not been suggested in, nor is it supported by, existing literature. [1] Rather, the test is described as a potentially useful measure of "a physical function construct not captured by the other tests." [5]

The CHADS2 score and its updated version, the CHA2DS2-VASc score, are clinical prediction rules for estimating the risk of stroke in people with non-rheumatic atrial fibrillation (AF), a common and serious heart arrhythmia associated with thromboembolic stroke. Such a score is used to determine whether or not treatment is required with anticoagulation therapy or antiplatelet therapy, since AF can cause stasis of blood in the upper heart chambers, leading to the formation of a mural thrombus that can dislodge into the blood flow, reach the brain, cut off supply to the brain, and cause a stroke.

Cardiac rehabilitation (CR) is defined by the World Health Organization (WHO) as "the sum of activity and interventions required to ensure the best possible physical, mental, and social conditions so that patients with chronic or post-acute cardiovascular disease may, by their own efforts, preserve or resume their proper place in society and lead an active life". CR is a comprehensive model of care delivering established core components, including structured exercise, patient education, psychosocial counselling, risk factor reduction and behaviour modification, with a goal of optimizing patient's quality of life and reducing the risk of future heart problems.

Balance in biomechanics, is an ability to maintain the line of gravity of a body within the base of support with minimal postural sway. Sway is the horizontal movement of the centre of gravity even when a person is standing still. A certain amount of sway is essential and inevitable due to small perturbations within the body or from external triggers. An increase in sway is not necessarily an indicator of dysfunctional balance so much as it is an indicator of decreased sensorimotor control.

Occupational cardiovascular diseases (CVD) are diseases of the heart or blood vessels caused by working conditions, making them a form of occupational illness. These diseases include coronary heart disease, stroke, cardiomyopathy, arrythmia, and heart valve or heart chamber problems. Cardiovascular disease is the leading cause of death in the United States and worldwide. In the United States, cardiovascular diseases account for one out of four deaths. The 6th International Conference on Work Environment and Cardiovascular Diseases found that within the working age population about 10-20% of cardiovascular disease deaths can be attributed to work. Ten workplace stressors and risk factors were estimated to be associated with 120,000 U.S. deaths each year and account for 5-8% of health care costs.

Fugl-Meyer Assessment (FMA) scale is an index to assess the sensorimotor impairment in individuals who have had stroke. This scale was first proposed by Axel Fugl-Meyer and his colleagues as a standardized assessment test for post-stroke recovery in their paper titled The post-stroke hemiplegic patient: A method for evaluation of physical performance. It is now widely used for clinical assessment of motor function. The Fugl-Meyer Assessment score has been tested several times, and is found to have excellent consistency, responsivity and good accuracy. The maximum possible score in Fugl-Meyer scale is 226, which corresponds to full sensory-motor recovery. The minimal clinically important difference of Fugl-Meyer assessment scale is 6 for lower limb in chronic stroke and 9-10 for upper limb in sub-acute stroke.

Age-related mobility disability is a self-reported inability to walk due to impairments, limited mobility, dexterity or stamina. It has been found mostly in older adults with decreased strength in lower extremities.
