Antibacterial soap is a soap which contains chemical ingredients that purportedly assist in killing bacteria. [1] The majority of antibacterial soaps contain triclosan, though other chemical additives are also common. [2] The effectiveness of products branded as being antibacterial has been disputed by some academics as well as the U.S. Food and Drug Administration (FDA). [3]

History

The earliest antibacterial soap was carbolic soap, which used up to 5% phenols (carbolic acid). Fears about the safety of carbolic soaps chemical components on the skin brought about a ban on some of these chemical components. [4] Triclosan and other antibacterial agents have long been used in commercial cleaning products for hospitals and other healthcare settings, however they began to be used in home cleaning products during the 1990s. [3]

Ingredients

Triclosan and triclocarban are the most common compounds used as antibacterials in soaps. [5] However, other common antibacterial ingredients in soaps include benzalkonium chloride, benzethonium chloride, and chloroxylenol. [6]

Effectiveness

Claims that antibacterial soap is effective stem from the long-standing knowledge that triclosan can inhibit the growth of various bacteria, as well as some fungi. [2] However, more recent reviews have suggested that antibacterial soaps are no better than regular soaps at preventing illness or reducing bacteria on the hands of users. [2] [7]

In September 2016, the U.S. Food and Drug Administration banned the use of the common antibacterial ingredients triclosan and triclocarban, and 17 other ingredients frequently used in "antibacterial" soaps and washes, due to insufficient information on the long-term health effects of their use and a lack of evidence on their effectiveness. The FDA stated "There is no data demonstrating that over-the-counter antibacterial soaps are better at preventing illness than washing with plain soap and water". [6] The agency also asserted that despite requests for such information, the FDA did not receive sufficient data from manufacturers on the long-term health effects of these chemicals. This ban does not apply to hand sanitizer. [5] This is due to the fact that hand sanitizer typically utilizes alcohol to kill microbes rather than triclosan or similar ingredients.[ citation needed ]

A 2017 statement by 200 scientists and medics published in the scientific journal Environmental Health Perspectives warns that anti-bacterial soaps and gels are useless and may cause harm. [8] The statement also cautioned against the use of antimicrobial agents in food contact materials, textiles, and paints. [8] British firm Unilever claimed in 2017 to be phasing triclosan and triclocarban out of their products by the end of the year, adding they would be replaced by “a range of alternatives, including natural and nature-inspired antibacterial ingredients”. [9]

Claims have been made in the media that antibacterial soap is more effective than plain soap in the prevention of the SARS-CoV-2 virus. The CDC and the Food and Drug Administration both recommend plain soap; there is no evidence that antibacterial soaps are any better, and limited evidence that they might be worse long-term. [10]

Owing to differences in composition and fluoride content, not all toothpastes are equally effective in maintaining oral health. The decline of tooth decay during the 20th century has been attributed to the introduction and regular use of fluoride-containing toothpastes worldwide. Large amounts of swallowed toothpaste can be poisonous. Common colors for toothpaste include white and blue.

<span class="mw-page-title-main">Medical uses of silver</span>

The medical uses of silver include its use in wound dressings, creams, and as an antibiotic coating on medical devices. Wound dressings containing silver sulfadiazine or silver nanomaterials may be used to treat external infections. The limited evidence available shows that silver coatings on endotracheal breathing tubes may reduce the incidence of ventilator-associated pneumonia. There is tentative evidence that using silver-alloy indwelling catheters for short-term catheterizing will reduce the risk of catheter-acquired urinary tract infections.

A deodorant is a substance applied to the body to prevent or mask body odor caused by bacterial breakdown of perspiration, for example in the armpits, groin, or feet. A subclass of deodorants, called antiperspirants, prevents sweating itself, typically by blocking sweat glands. Antiperspirants are used on a wider range of body parts, at any place where sweat would be inconvenient or unsafe, since unwanted sweating can interfere with comfort, vision, and grip. Other types of deodorant allow sweating but prevent bacterial action on sweat, since human sweat only has a noticeable smell when it is decomposed by bacteria.

Triclosan is an antibacterial and antifungal agent present in some consumer products, including toothpaste, soaps, detergents, toys, and surgical cleaning treatments. It is similar in its uses and mechanism of action to triclocarban. A disinfectant is a chemical substance or compound used to inactivate or destroy microorganisms on inert surfaces. Disinfection does not necessarily kill all microorganisms, especially resistant bacterial spores; it is less effective than sterilization, which is an extreme physical or chemical process that kills all types of life. Disinfectants are generally distinguished from other antimicrobial agents such as antibiotics, which destroy microorganisms within the body, and antiseptics, which destroy microorganisms on living tissue. Disinfectants are also different from biocides—the latter are intended to destroy all forms of life, not just microorganisms. Disinfectants work by destroying the cell wall of microbes or interfering with their metabolism. An antimicrobial is an agent that kills microorganisms (microbicide) or stops their growth. Antimicrobial medicines can be grouped according to the microorganisms they act primarily against. For example, antibiotics are used against bacteria, and antifungals are used against fungi. They can also be classified according to their function. The use of antimicrobial medicines to treat infection is known as antimicrobial chemotherapy, while the use of antimicrobial medicines to prevent infection is known as antimicrobial prophylaxis.

Chloroxylenol, also known as para-chloro-meta-xylenol (PCMX), is a chlorine substituted phenol with a white to off-white appearance and a phenolic odor. The discovery of chloroxylenol was the result of efforts to produce improved antiseptics that began at the end of the 1800s. First synthesized in Germany in 1923, it was borne out of the study of coal tar components that began a decade earlier.

Hand sanitizer is a liquid, gel or foam generally used to kill many viruses/bacteria/microorganisms on the hands. It can also come in the form of a cream, spray, or wipe. In most settings, hand washing with soap and water is generally preferred. Hand sanitizer is less effective at killing certain kinds of germs, such as norovirus and Clostridium difficile, and unlike hand washing, it cannot physically remove harmful chemicals. People may incorrectly wipe off hand sanitizer before it has dried, and some are less effective because their alcohol concentrations are too low.

Cetylpyridinium chloride (CPC) is a cationic quaternary ammonium compound used in some types of mouthwashes, toothpastes, lozenges, throat sprays, breath sprays, and nasal sprays. It is an antiseptic that kills bacteria and other microorganisms. It has been shown to be effective in preventing dental plaque and reducing gingivitis. It has also been used as an ingredient in certain pesticides. Though one study seems to indicate cetylpyridinium chloride does not cause brown tooth stains, at least one mouthwash containing CPC as an active ingredient bears the warning label "In some cases, antimicrobial rinses may cause surface staining to teeth," following a failed class-action lawsuit brought by customers whose teeth were stained.

Benzethonium chloride, also known as hyamine is a synthetic quaternary ammonium salt. This compound is an odorless white solid, soluble in water. It has surfactant, antiseptic, and anti-infective properties, and it is used as a topical antimicrobial agent in first aid antiseptics. It is also found in cosmetics and toiletries such as soap, mouthwashes, anti-itch ointments, and antibacterial moist towelettes. Benzethonium chloride is also used in the food industry as a hard surface disinfectant.

Triclocarban is an antibacterial chemical once common in, but now phased out of, personal care products like soaps and lotions. It was originally developed for the medical field. Although the mode of action is unknown, TCC can be effective in fighting infections by targeting the growth of bacteria such as Staphylococcus aureus. Additional research seeks to understand its potential for causing antibacterial resistance and its effects on organismal and environmental health.

A soap substitute is a natural or synthetic cleaning product used in place of soap or other detergents, typically to reduce environmental impact or health harms or provide other benefits.

Antimicrobials destroy bacteria, viruses, fungi, algae, and other microbes. The cells of bacteria (prokaryotes), such as salmonella, differ from those of higher-level organisms (eukaryotes), such as fish. Antibiotics are chemicals designed to either kill or inhibit the growth of pathogenic bacteria while exploiting the differences between prokaryotes and eukaryotes in order to make them relatively harmless in higher-level organisms. Antibiotics are constructed to act in one of three ways: by disrupting cell membranes of bacteria, by impeding DNA or protein synthesis, or by hampering the activity of certain enzymes unique to bacteria.

<span class="mw-page-title-main">Ceftazidime/avibactam</span> Combination antibiotic medication

Ceftazidime/avibactam, sold under the brand name Avycaz among others, is a fixed-dose combination medication composed of ceftazidime, a cephalosporin antibiotic, and avibactam, a β-lactamase inhibitor. It is used to treat complicated intra-abdominal infections, urinary tract infections, and pneumonia. It is only recommended when other options are not appropriate. It is given by injection into a vein.
