The Haaksbergen monster truck accident happened on 28 September 2014 in Haaksbergen, the Netherlands, when a monster truck crashed into the attending crowd. [1] Three visitors were reported dead, amongst them one child. [2] [3] According to Hans Gerritsen, the mayor of Haaksbergen, twelve people were injured. [3]

The Monster Truck Racing Association has stated that spectators were situated at event level and in close proximity to the performance area, separated by only bike rack barricades. It also stated that multiple layers of safety precaution, such as an on-board Remote Ignition Interrupter (RII) to allow event staff to instantly disable the vehicle, would have undoubtedly prevented the accident. On March 15, 1999, Amtrak's southbound City of New Orleans passenger train collided with a semi-trailer truck in the village of Bourbonnais, Illinois, United States. Most of the train derailed, killing eleven people. A National Transportation Safety Board (NTSB) investigation into the accident attributed the cause to the truck driver trying to beat the train across a grade crossing. The NTSB's recommendations from the accident included increased enforcement of grade crossing signals, the installation of train event recorders at all new or improved grade crossings, and procedures to provide emergency responders with accurate lists of all crew members and passengers aboard trains. The city of Bourbonnais erected a memorial near the site to commemorate those killed in the accident.

The Sierre coach crash occurred on 13 March 2012 near Sierre, Switzerland, when a coach carrying school teachers and pupils crashed into a wall in the Sierre Tunnel. The Oss railway accident happened on 20 September 2018 when a passenger train collided with a Stint cart on a level crossing at Oss, North Brabant, Netherlands. Four children on the cart were killed, and two were seriously injured. Stint carts were subsequently banned from use on public roads in the Netherlands. An investigation into the cause of the accident is ongoing.

The Chihuahua monster truck accident occurred on 5 October 2013 in Chihuahua City, Mexico, when a monster truck crashed into spectators at an air show. Eight people were reported killed, including children, and 79 were injured. Officials and event organizers were blamed for the disaster. To date, it is the deadliest monster truck incident to have occurred.
