Comparative neuropsychology refers to an approach used for understanding human brain functions. It involves the direct evaluation of clinical neurological populations by employing experimental methods originally developed for use with nonhuman animals.

Over many decades of animal research, methods were perfected to study the effects of well-defined brain lesions on specific behaviors, and later the tasks were modified for human use. Generally the modifications involve changing the reward from food to money, but standard administration of the tasks in humans still involves minimal instructions, thus necessitating a degree of procedural learning in human and nonhuman animals alike.

Currently, comparative neuropsychology is used with neurological patients to link specific deficits with localized areas of the brain.

The comparative neuropsychological approach employs simple tasks that can be mastered without relying upon language skills. Precisely because these simple paradigms do not require linguistic strategies for solution, they are especially useful for working with patients whose language skills are compromised, or whose cognitive skills may be minimal.

Comparative neuropsychology contrasts with the traditional approach of using tasks that rely upon linguistic skills, and that were designed to study human cognition. Because important ambiguities about its heuristic value had not been addressed empirically, only recently has comparative neuropsychology become popular for implementation with brain-damaged patients.

Within the past decade, comparative neuropsychology has had prevalent use as a framework for comparing and contrasting the performances of disparate neurobehavioral populations on similar tasks.

History

Comparative neuropsychology involves the study of brain-behavior relationships by applying experimental paradigms, used extensively in animal laboratories, for testing human clinical populations. [1] Popular paradigms include delayed reaction tasks, discrimination and reversal learning tasks, and matching- and nonmatching-to-sample. [1] These tasks adapt behavioral paradigms used with nonhuman animals to measure cognitive function and dysfunction in humans. Such tasks were perfected on experimental animals having well defined brain lesions, and adapted for human neurological patients. [1] The comparative aspects of such approach resides in the analogy between animals with brain lesions and human patients with lesions in homologous areas of the brain. Examples are represented by the comparison between the brain of laboratory animals (primarily non human primates and mice) with the one of people with damages resulting from excessive alcohol use.

George Ettlinger

George Ettlinger was one of the few who actively combined human and animal research, and he did so consistently throughout his scientific career. [2] Ettinger work focused on the importance of the inferior temporal neocortex in visual discrimination learning and memory in macaque monkeys, [3] and on the importance of ventral temporal lobe in vision. Ettinger animals models carried inferotemporal or latero-ventral prestriate ablation. In 1966 George Ettlinger, together with the psychologist Colin Blakemore and the neurosurgeon Murray Falconer, described the results of a study on correlation between pre-operative intelligence and the severity of mesial temporal sclerosis in temporal lobe specimens excised to treat intractable epilepsy. [4] Such study It is known as a forerunner of what has become one of the potentially most interesting techniques for exploring the relationship between certain aspects of human memory and temporal lobe structures. Evidence is based on case studies of individual brain damaged patients who show deficits in brain areas and from patients who exhibit double dissociations. Double dissociations involve two patients and two tasks. One patient is impaired at one task but normal on the other, while the other patient is normal on the first task and impaired on the other. For example, patient A would be poor at reading printed words while still being normal at understanding spoken words, while the patient B would be normal at understanding written words and be poor at understanding spoken words. Scientists can interpret this information to explain how there is a single cognitive module for word comprehension. From studies like these, researchers infer that different areas of the brain are highly specialised. Cognitive neuropsychology can be distinguished from cognitive neuroscience, which is also interested in brain damaged patients, but is particularly focused on uncovering the neural mechanisms underlying cognitive processes.

Henry Gustav Molaison, known widely as H.M., was an American man who had a bilateral medial temporal lobectomy to surgically resect the anterior two thirds of his hippocampi, parahippocampal cortices, entorhinal cortices, piriform cortices, and amygdalae in an attempt to cure his epilepsy. Although the surgery was partially successful in controlling his epilepsy, a severe side effect was that he became unable to form new memories.

Clinical neuropsychology

Clinical neuropsychology is a sub-field of psychology concerned with the applied science of brain-behaviour relationships. Clinical neuropsychologists use this knowledge in the assessment, diagnosis, treatment, and or rehabilitation of patients across the lifespan with neurological, medical, neurodevelopmental and psychiatric conditions, as well as other cognitive and learning disorders. The branch of neuropsychology associated with children and young people is pediatric neuropsychology.

Brenda Milner British-Canadian neuroscientist and neuropsychologist

Brenda Milner is a British-Canadian neuropsychologist who has contributed extensively to the research literature on various topics in the field of clinical neuropsychology. As of 2010, Milner is a professor in the Department of Neurology and Neurosurgery at McGill University and a professor of Psychology at the Montreal Neurological Institute. As of 2005, she holds more than 20 degrees and continues to work in her nineties. Her current work covers many aspects of neuropsychology including her lifelong interest in the involvement of the temporal lobes in episodic memory. She is sometimes referred to as "the founder of neuropsychology" and has proven to be an essential key in its development. She received the Balzan Prize for Cognitive Neuroscience, in 2009, and the Kavli Prize in Neuroscience, together with John O'Keefe, and Marcus E. Raichle, in 2014. She turned 100 in July 2018 and at the time was still overseeing the work of researchers.

The two-streams hypothesis is a model of the neural processing of vision as well as hearing. The hypothesis, given its initial characterisation in a paper by David Milner and Melvyn A. Goodale in 1992, argues that humans possess two distinct visual systems. Recently there seems to be evidence of two distinct auditory systems as well. As visual information exits the occipital lobe, and as sound leaves the phonological network, it follows two main pathways, or "streams". The ventral stream leads to the temporal lobe, which is involved with object and visual identification and recognition. The dorsal stream leads to the parietal lobe, which is involved with processing the object's spatial location relative to the viewer and with speech repetition.

Executive functions are a set of cognitive processes that are necessary for the cognitive control of behavior: selecting and successfully monitoring behaviors that facilitate the attainment of chosen goals. Executive functions include basic cognitive processes such as attentional control, cognitive inhibition, inhibitory control, working memory, and cognitive flexibility. Higher-order executive functions require the simultaneous use of multiple basic executive functions and include planning and fluid intelligence.

Edith F. Kaplan was an American psychologist. She was a pioneer of neuropsychological tests and did most of her work at the Boston VA Hospital. Kaplan is known for her promotion of clinical neuropsychology as a specialty area in psychology. She examined brain-behavioral relationships in aphasia, apraxia, developmental issues in clinical neuropsychology, as well as normal and abnormal aging. Kaplan helped develop a new method of assessing brain function with neuropsychological assessment, called "The Boston Process Approach."

Foix-Chavany-Marie Syndrome (FCMS), also known as Bilateral Opercular Syndrome, is a neuropathological disorder characterized by paralysis of the facial, tongue, pharynx, and masticatory muscles of the mouth that aid in chewing. The disorder is primarily caused by thrombotic and embolic strokes, which cause a deficiency of oxygen in the brain. As a result, bilateral lesions may form in the junctions between the frontal lobe and temporal lobe, the parietal lobe and cortical lobe, or the subcortical region of the brain. FCMS may also arise from defects existing at birth that may be inherited or nonhereditary. Symptoms of FCMS can be present in a person of any age and it is diagnosed using automatic-voluntary dissociation assessment, psycholinguistic testing, neuropsychological testing, and brain scanning. Treatment for FCMS depends on the onset, as well as on the severity of symptoms, and it involves a multidisciplinary approach.

Verbal fluency tests are a kind of psychological test in which participants have to produce as many words as possible from a category in a given time. This category can be semantic, including objects such as animals or fruits, or phonemic, including words beginning with a specified letter, such as p, for example. The semantic fluency test is sometimes described as the category fluency test or simply as "freelisting", while letter fluency is also referred to as phonemic test fluency. The COWAT is the most employed phonemic variant. Although the most common performance measure is the total number of words, other analyses such as number of repetitions, number and length of clusters of words from the same semantic or phonemic subcategory, or number of switches to other categories can be carried out.

The California Verbal Learning Test (CVLT) is one of the most widely used neuropsychological tests in North America. As an instrument, it represents a relatively new approach to clinical psychology and the cognitive science of memory. It measures episodic verbal learning and memory, and demonstrates sensitivity to a range of clinical conditions. The test does this by attempting to link memory deficits with impaired performance on specific tasks. It assesses encoding, recall and recognition in a single modality of item presentation (auditory-verbal). The CVLT is considered to be a more sensitive measure of episodic memory than other verbal learning tests. It was designed to not only measure how much a subject learned, but also reveal strategies employed and the types of errors made. The CVLT indexes free and cued recall, serial position effects, semantic clustering, intrusions, interference and recognition. Delis et al. (1994) released the California Verbal Learning Test for Children (CVLT-C). The California Verbal Learning Test-II (CVLT-II) is an updated version of the original CVLT, which has been standardized and provides normative data.

Retrospective memory is the memory of people, words, and events encountered or experienced in the past. It includes all other types of memory including episodic, semantic and procedural. It can be either implicit or explicit. In contrast, prospective memory involves remembering something or remembering to do something after a delay, such as buying groceries on the way home from work. However, it is very closely linked to retrospective memory, since certain aspects of retrospective memory are required for prospective memory.

In psychology and neuroscience, executive dysfunction, or executive function deficit, is a disruption to the efficacy of the executive functions, which is a group of cognitive processes that regulate, control, and manage other cognitive processes. Executive dysfunction can refer to both neurocognitive deficits and behavioural symptoms. It is implicated in numerous psychopathologies and mental disorders, as well as short-term and long-term changes in non-clinical executive control.

Some of the research that is conducted in the field of psychology is more "fundamental" than the research conducted in the applied psychological disciplines, and does not necessarily have a direct application. The subdisciplines within psychology that can be thought to reflect a basic-science orientation include biological psychology, cognitive psychology, neuropsychology, and so on. Research in these subdisciplines is characterized by methodological rigor. The concern of psychology as a basic science is in understanding the laws and processes that underlie behavior, cognition, and emotion. Psychology as a basic science provides a foundation for applied psychology. Applied psychology, by contrast, involves the application of psychological principles and theories yielded up by the basic psychological sciences; these applications are aimed at overcoming problems or promoting well-being in areas such as mental and physical health and education.

The Halstead-Reitan Neuropsychological Test Battery (HRNB) and allied procedures is a comprehensive suite of neuropsychological tests used to assess the condition and functioning of the brain, including etiology, type, localization and lateralization of brain injury. The HRNB was first constructed by Ward C. Halstead, who was chairman of the Psychology Department at the University of Chicago, together with his doctoral student, Ralph Reitan. A major aim of administering the HRNB to patients was if possible to lateralize a lesion to either the left or right cerebral hemisphere by comparing the functioning on the both sides of the body on a variety of tests such as the Suppression or Sensory Imperception Test, the Finger Agnosia Test, Finger Tip Writing, the Finger Tapping Test, and the Tactual Performance Test. One difficulty with the HRNB was its excessive administration time. In particular, administration of the Halstead Category Test was lengthy, so subsequent attempts were made to construct reliable and valid short-forms.

Topographical disorientation is the inability to orient oneself in one's surroundings, sometimes as a result of focal brain damage. This disability may result from the inability to make use of selective spatial information or to orient by means of specific cognitive strategies such as the ability to form a mental representation of the environment, also known as a cognitive map. It may be part of a syndrome known as visuospatial dysgnosia.

Ralph M. Reitan was an American neuropsychologist and one of the fathers of American clinical neuropsychology having brought the notion of brain-behavior relationships to the forefront of the field. He is best known for his role in developing the Halstead-Reitan Neuropsychological Battery and his strong belief in empiricism and evidence-based practice. He was a strong advocate of use of a fixed battery in neuropsychological assessment, published prolifically, and mentored many students who also became prominent in the field. As an author, he has been collected by libraries.
