Wearing an electronic heart-rate counter and standing on a lighted circular platform known as "The Plate," a single contestant answers questions in order to win money over a total of seven rounds. While a round is in progress, the contestant's heartbeats are continuously counted and deducted from an initial total of 1,000. If the total reaches zero, the game ends immediately, the contestant is defeated and leaves empty-handed. During gameplay, the contestant's heart rate is displayed on a monitor and relayed to a live string quartet playing the show's musical score; they adjust their tempo (beats per minute) to match the heart rate and maximise tension.
The contestant must give seven correct answers to complete Round 1, and the required number decreases by one for each subsequent round (six in Round 2, five in Round 3, etc.). Each incorrect answer deducts 25 heartbeats. The contestant may change out a question by stepping off the Plate, at a cost of 100 beats (50 in Series 1). Any correct answers given prior to stepping off are counted toward the number required to complete each round once play resumes. If the contestants number of heartbeats reach 0 it's CONTESTANT DEFEATED and GAME OVER.
Each completed round increases the contestant's winnings to as follows:
- Round 1 – £250
- Round 2 – £500
- Round 3 – £1,000
- Round 4 – £2,500
- Round 5 – £5,000
- Round 6 – £10,000
- Round 7 – £25,000
At the start of each new round, the contestant is shown an example of the game to be played and may choose to "Cash Out" with the money banked to that point rather than play on. In order to win the money, they must complete one final "Cashout Round."

The show is broadcast at various times, spanning across six episodes for each set. The program was originally shown by TROS on NPO 2, but moved to creator John de Mol's channel Tien in 2005. After the channel was discontinued after its sale to the RTL Group, the program moved to RTL 4. In 2019, the program moved to SBS6 due to the transfer of Linda de Mol from RTL to SBS.

Adult contestants answer questions, as if they came from an elementary grade school quiz. The original U.S. version debuted on the Fox Broadcasting network on February 27, 2007, with host Jeff Foxworthy, airing on Fox until 2009, as a syndicated TV series, between 2009 and 2011, and then revived on Fox in 2015, and again on Nickelodeon in 2019, with new host, John Cena. The Are You Smarter than a 5th Grader? format, has since been replicated in several other countries, some versions under the same title, and some under modified ones.
Pointless is a British television quiz show produced by Banijay subsidiary Remarkable Entertainment for the BBC hosted by Alexander Armstrong. In each episode, four teams of two contestants attempt to find correct but obscure answers to four rounds of general knowledge questions, with the winning team eligible to compete for the show's cash jackpot.
Five Minutes to a Fortune is a British game show, which aired on Channel 4 from 6 April 2013 to 5 May 2013 and was hosted by Davina McCall. It offered pairs of contestants the chance to win a top prize of £50,000. The show aired weekdays at 5:00pm but was unable to generate high ratings to compete with ITV's The Chase and BBC's Pointless, both of which air roughly in the same timeslot.
Million Dollar Minute is an Australian quiz show which aired on the Seven Network. It premiered on 16 September 2013. The show was originally hosted by Grant Denyer, and later by Simon Reeve, and aired at 5:30 pm on weeknights. The show was cancelled in September 2015 and was replaced by The Chase Australia in its timeslot. Repeats are currently shown on 7TWO in place of Home and Away's Early Years on Hiatus.
L'eredità is an Italian Rai 1 game show currently hosted by Marco Liorni. It premiered on 29 July 2002. From 29 July 2002 to 10 June 2006, it was hosted by Italian presenter Amadeus. He was succeeded by Carlo Conti, who hosted the show until 14 April 2014, when he was replaced by Fabrizio Frizzi. Frizzi hosted until 23 October 2017, when he became ill during the taping of the next show and was taken to the hospital. Carlo Conti substituted for Frizzi from 30 October 2017 to 15 December 2017, and he hosted the special episode in tandem with Fabrizio Frizzi when the latter returned to host the quiz. On 16 December 2017, Fabrizio Frizzi resumed hosting the show by himself until 25 March 2018. The next day, Frizzi died of a brain haemorrhage. Following a hiatus due to Frizzi's death, the game returned from 3 April 2018 until the end of the season with the host Carlo Conti. Flavio Insinna presented the show from 24 September 2018 until 2023.

In March 2017, NBC ordered six hour-long episodes of the series. Ellen DeGeneres serves as host, while Stephen "tWitch" Boss appears as announcer/sidekick. The series is based on game segments from DeGeneres' daytime talk show, The Ellen DeGeneres Show. The series premiered on December 18, 2017. On February 18, 2020, DeGeneres announced on The Ellen DeGeneres Show that the series was renewed for a fourth season, which began airing on October 6, 2020. In January 2022, the series was canceled after four seasons.
