The Australian Commission on Safety and Quality in Health Care (ACSQHC) is the national health care standards agency for all health services, general practices, pathology and medical imaging providers in Australia. The commission is part of the Health and Aged Care portfolio of the federal government, which is accountable to the minister for health, currently Mark Butler.
The commission is jointly funded by the Australian Government, and state and territory governments.
The commission was first formed in 2006 and formalised in 2011 to delivery programs to improve health care received in Australia, primarily in the form of standards. [2] [3] The commission has developed national care standards for all health services (National Safety and Quality Health Service Standards), as well as more specific standards for general practices and allied health (Primary and Community Healthcare Standards), mental health services (Mental Health Standards for Community Managed Organisations), and clinical care delivered in aged care settings (Aged Care Quality Standards). [4]
The commission also develops and oversees two mandatory accreditation schemes for medical imaging services and pathology services, and is involved in the national digital mental health standards and the cosmetic surgery reform efforts. [4]
The National Safety and Quality Health Service Standards (NSQHS Standards) are Australia's principal health care standards, and apply to all health services including inpatient, outpatient, and community care. [5] There are 8 standards:
Within each standard are a sub-set of rules and expectations. Health services are required to be assessed by an independent accreditor against these standards, and cannot deliver services without accreditation in most circumstances. [6]
The Australian Health Service Safety and Quality Accreditation Scheme (AHSSQA) Scheme operationalises the standards by way of mandatory inspections, accreditation and reporting of health services in each state and territory. Independent organisations can apply to the commission to be an approved accrediting agency, and complete accreditations when engaged by health services as required. [7] Services must receive initial accreditation to begin services, and receive full accreditation within 18 months. [8] They will then be reaccredited at least once every three years. [9]
From 1 July 2023, short notice assessment began which reduced the notice period for hospitals, day procedure facilities and publicly funded or contracted community services to 24 hours. [8]

Pharmacists often serve as primary care providers in the community, and may offer other services such as health screenings and immunizations.
A dietitian, medical dietitian, or dietician is an expert in identifying and treating disease-related malnutrition and in conducting medical nutrition therapy, for example designing an enteral tube feeding regimen or mitigating the effects of cancer cachexia. Many dietitians work in hospitals and usually see specific patients where a nutritional assessment and intervention has been requested by a doctor or nurse, for example if a patient has lost their ability to swallow or requires artificial nutrition due to intestinal failure. Dietitians are regulated healthcare professionals licensed to assess, diagnose, and treat such problems. In the United Kingdom, dietitian is a 'protected title', meaning identifying yourself as a dietitian without appropriate education and registration is prohibited by law.
Clinical governance is a systematic approach to maintaining and improving the quality of patient care within the National Health Service (NHS) and private sector health care. Clinical governance became important in health care after the Bristol heart scandal in 1995, during which an anaesthetist, Dr Stephen Bolsin, exposed the high mortality rate for paediatric cardiac surgery at the Bristol Royal Infirmary. A medical laboratory scientist (MLS) or clinical laboratory scientist (CLS) or medical technologist (MT) performs diagnostic testing of blood and body fluids in clinical laboratories. The scope of a medical laboratory scientist's work begins with the receipt of patient or client specimens and terminates with the delivery of test results to physicians and other healthcare providers. The utility of clinical diagnostic testing relies squarely on the validity of test methodology. To this end, much of the work done by medical laboratory scientists involves ensuring specimen quality, interpreting test results, data-logging, testing control products, performing calibration, maintenance, validation, and troubleshooting of instrumentation as well as performing statistical analyses to verify the accuracy and repeatability of testing. Medical laboratory scientists may also assist healthcare providers with test selection and specimen collection and are responsible for prompt verbal delivery of critical lab results. Medical Laboratory Scientists in healthcare settings also play an important role in clinical diagnosis. An estimated 70% of medical decisions are based on laboratory test results and MLS contributions affect 95% of a health system's costs.

The RACGP is responsible for maintaining standards for quality clinical practice, education and training, and research in Australian general practice. The RACGP represents over 40,000 members across metropolitan, urban, rural and remote Australia.
The Department of Health and Aged Care (DHAC), formerly the Department of Health, is a department of the Australian Government responsible for health research, funding, promotion and regulation in Australia. Primary health care and aged care services are overseen by DHAC, while tertiary health services are administered by state and territory governments. The department is responsible for programs such as Medicare, the Pharmaceutical Benefits Scheme, and agencies such as the Therapeutic Goods Administration and the National Health and Medical Research Council.
Patient safety is a discipline that emphasizes safety in health care through the prevention, reduction, reporting and analysis of error and other types of unnecessary harm that often lead to adverse patient events. The frequency and magnitude of avoidable adverse events, often known as patient safety incidents, experienced by patients was not well known until the 1990s, when multiple countries reported significant numbers of patients harmed and killed by medical errors. Recognizing that healthcare errors impact 1 in every 10 patients around the world, the World Health Organization (WHO) calls patient safety an endemic concern. Indeed, patient safety has emerged as a distinct healthcare discipline supported by an immature yet developing scientific framework. There is a significant transdisciplinary body of theoretical and research literature that informs the science of patient safety with mobile health apps being a growing area of research.
A Patient Safety Organization (PSO) is a group, institution, or association that improves medical care by reducing medical errors. Common functions of patient safety organizations are data collection, analysis, reporting, education, funding, and advocacy. A PSO differs from a Federally designed Patient Safety Organization (PSO), which provides health care providers in the U.S. privilege and confidentiality protections for efforts to improve patient safety and the quality of patient care delivery
A medical laboratory or clinical laboratory is a laboratory where tests are conducted out on clinical specimens to obtain information about the health of a patient to aid in diagnosis, treatment, and prevention of disease. Clinical medical laboratories are an example of applied science, as opposed to research laboratories that focus on basic science, such as found in some academic institutions.
Due to the near-universal desire for safe and good quality healthcare, there is a growing interest in international healthcare accreditation. Providing healthcare, especially of an adequate standard, is a complex and challenging process. Healthcare is a vital and emotive issue—its importance pervades all aspects of societies, and it has medical, social, political, ethical, business, and financial ramifications. In any part of the world healthcare services can be provided either by the public sector or by the private sector, or by a combination of both, and the site of delivery of healthcare can be located in hospitals or be accessed through practitioners working in the community, such as general medical practitioners and dental surgeons.
Hospital accreditation has been defined as “A self-assessment and external peer assessment process used by health care organizations to accurately assess their level of performance in relation to established standards and to implement ways to continuously improve”. Critically, accreditation is not just about standard-setting: there are analytical, counseling and self-improvement dimensions to the process. There are parallel issues in evidence-based medicine, quality assurance and medical ethics, and the reduction of medical error is a key role of the accreditation process. Hospital accreditation is therefore one component in the maintenance of patient safety. However, there is limited and contested evidence supporting the effectiveness of accreditation programs.
The United Kingdom Akkreditering Forum (UKAF), founded in June 1998 by a group of leading healthcare accreditation organisations, is a London-based network of healthcare accreditation organisations formed with the intention of sharing experience regarding good practice in accreditation, as well as sharing new ideas around improving the methodology for such programmes. It has an additional role in working to ensure that the general public develop a clearer idea about accreditation, its function, its implications and its execution.
The United Kingdom Accreditation Service (UKAS) is the sole national accreditation body recognised by the British government to assess the competence of organisations that provide certification, testing, inspection and calibration services. It evaluates these conformity assessment bodies and then accredits them where they are found to meet relevant internationally specified standards.
Health care quality is a level of value provided by any health care resource, as determined by some measurement. As with quality in other fields, it is an assessment of whether something is good enough and whether it is suitable for its purpose. The goal of health care is to provide medical resources of high quality to all who need them; that is, to ensure good quality of life, cure illnesses when possible, to extend life expectancy, and so on. Researchers use a variety of quality measures to attempt to determine health care quality, including counts of a therapy's reduction or lessening of diseases identified by medical diagnosis, a decrease in the number of risk factors which people have following preventive care, or a survey of health indicators in a population who are accessing certain kinds of care.
