The Columbia Accident Investigation Board (CAIB) was an internal commission convened by NASA to investigate the destruction of the Space Shuttle Columbia during STS-107 upon atmospheric re-entry on February 1, 2003. The panel determined that the accident was caused by foam insulation breaking off from the external fuel tank, forming debris which damaged the orbiter's wing, and that the problem of "debris shedding" was well known but considered "acceptable" by management. The panel also recommended changes that should be made to increase the safety of future shuttle flights. The CAIB released its final report on August 26, 2003.

Roosevelt Mercer Jr., SES, is the director of the Interagency Planning Office (IPO) for the Next Generation Air Transportation System (NextGen) at the Federal Aviation Administration where he provides high-level leadership for interagency and international collaboration related to NextGen. He executes the collaborative processes needed to ensure efficient coordination among all federal partners whose decisions impact NextGen. The federal partner agencies include the National Aeronautics and Space Administration (NASA), Department of Defense (DoD), Department of Homeland Security (DHS), Department of Commerce (DOC), National Transportation Safety Board (NTSB), National Science Foundation (NSF), and Federal Aviation Administration (FAA), as well as the White House Office of Science and Technology Policy (OSTP) and the Office of the Director of National Intelligence (ODNI) as an ex officio participant. Mr. Mercer is charged with providing executive direction to a dynamic multi-agency and international partnering organization focused on future NextGen technology, policy, and collaborative activities.

Darren Wayne McDew is a retired United States Air Force general who served as the Commander of United States Transportation Command at Scott Air Force Base, Illinois. His previous senior command positions include Commander of Air Mobility Command as a general and Commander, Eighteenth Air Force (18AF) as a lieutenant general. In March, 2019, soon after General McDew's retirement, United States Secretary of Transportation Elaine Chao named him as Co-Chair of the Special Committee to Review the Federal Aviation Administration's Aircraft Certification Process. According to a letter from Secretary Chao, the Special Committee is "specifically tasked to review the 737 MAX 800 certification process from 2012 to 2017."
Ricky Lynn Waddell is a retired lieutenant general in the United States Army Reserve who served as a Deputy National Security Advisor to President Donald Trump from 2017 to 2018 and as the Assistant to the Chairman of the Joint Chiefs of Staff from 2018 to 2021. His promotion to lieutenant general was authorized by the United States Senate on 26 September 2019. He retired from active military service in October 2021.
Dorothy A. Hogg is a retired lieutenant general of the United States Air Force who last served as the twenty-third Surgeon General of the United States Air Force and the first Surgeon General of the United States Space Force. Hogg serves as functional manager of the U.S. Air Force Medical Service. In this capacity, she advises the Secretary of the Air Force and Air Force Chief of Staff, as well as the Assistant Secretary of Defense for Health Affairs on matters pertaining to the medical aspects of the air expeditionary force and the health of Airmen.

The Deputy Chief of Staff for Intelligence, Surveillance, Reconnaissance, and Cyber Effects Operations of the United States Air Force is a position in the United States Air Force tasked with the development and implementation of policy formulation, planning, evaluation, oversight and leadership of Air Force intelligence, surveillance, reconnaissance and cyber effects operations capabilities. Commonly referred to as the A26, it is held by a lieutenant general who also serves as the representative of the Air Force to the intelligence community. The position is one of among the ten same positions in the Headquarters of the U.S. Air Force. As such, the officeholder of this position serves in the Air Staff. The current holder of this position is Lieutenant General Leah G. Lauderback.

She most recently served as the director of operations of the United States Transportation Command. She previously served as the director of operations, strategic deterrence, and nuclear integration of the Air Mobility Command.
