EuroSCORE (European System for Cardiac Operative Risk Evaluation) is a risk model which allows the calculation of the risk of death after a heart operation. The model asks for 17 items of information about the patient, the state of the heart and the proposed operation, [1] and uses logistic regression to calculate the risk of death. [2] It is free to use online.

First published in 1999, [3] the model has been adopted worldwide, becoming the most widely used risk index for cardiac surgery, [4] and its use is believed to have contributed substantially to the improvement in the results of heart surgery seen at the beginning of the millennium. The original models are now aging, [5] and a new model - EuroSCORE II - was announced at the EACTS meeting in Lisbon in 2011 and published in the European Journal of Cardiothoracic Surgery in April 2012. The updated calculator is available online at the official EuroSCORE website.

EuroSCORE II has substantially improved the calibration of the older models and at least maintained and perhaps further improved on their discriminatory power. It has largely replaced the two older models of EuroSCORE and is now the predominant worldwide heart surgery risk model.[ citation needed ]

Risk models are used for two main reasons. The first is that a risk model such as EuroSCORE allows the calculation of the risk of death before undertaking a heart operation. This is important as it serves to guide the clinician and the patient about the advisability of an operation by helping to weigh the risk against the benefits. The second is as a method of quality control. By calculating the expected risk of death for a population of patients having heart operations, this can be compared with the number of actual deaths. This comparison can be used as a measure of the quality of the performance of the hospital, unit or surgeon in question.[ citation needed ]

Statistics

The statistical basis for EuroSCORE and EuroSCORE II is the well-established logistic regression. The available EuroSCORE papers supply logistic regression coefficients but without any measure of uncertainty and the raw data from which the calculations have been made are not available to the public.

Postperfusion syndrome, also known as "pumphead", is a constellation of neurocognitive impairments attributed to cardiopulmonary bypass (CPB) during cardiac surgery. Symptoms of postperfusion syndrome are subtle and include defects associated with attention, concentration, short-term memory, fine motor function, and speed of mental and motor responses. Studies have shown a high incidence of neurocognitive deficit soon after surgery, but the deficits are often transient with no permanent neurological impairment.

A valve area of less than 1.0 cm2 is considered to be severe aortic stenosis.

He is also the Director of Quality Outcomes and Process Improvement for the Department of Thoracic and Cardiovascular Surgery and Affiliate Cardiac Surgery Program at Cleveland Clinic.


The Revised Cardiac Risk Index (RCRI) is a tool used to estimate a patient's risk of perioperative cardiac complications. The RCRI and similar clinical prediction tools are derived by looking for an association between preoperative variables (e.g., patient's age, type of surgery, comorbid diagnoses, or laboratory data) and the risk for cardiac complications in a cohort of surgical patients (the "derivation cohort"). Variables that have independent predictive value in a logistic regression analysis are incorporated into the risk index. Ideally, the accuracy and validity of the risk index is then tested in a separate cohort (the "validation cohort"). In 1977 Goldman, et al., developed the first cardiac risk index, which included nine variables associated with an increased risk of perioperative cardiac complications. This became known as the Original Cardiac Risk Index (or alternatively the Goldman Index). In 1999, Lee et al. published a cardiac risk index derived from 2893 patients and validated in 1422 patients aged ≥ 50 undergoing major noncardiac surgery, which became known as the Revised Cardiac Risk Index (RCRI). Lee identified six independent variables that predicted an increased risk for cardiac complications. A patient's risk for perioperative cardiac complications increased with number of variables that were present.

Bridge therapy is therapy intended, in transportation metaphor, to serve as a figurative bridge to another stage of therapy or health, helping a patient past a challenging period caused by particular severe illness. There are various types of bridge therapy, such as bridge to transplant, bridge to candidacy, bridge to decision, bridge to recovery, and anticoagulation bridge. Bridge therapy exists in contrast to destination therapy, which is the figurative destination rather than a bridge to something else.

She serves as Chair of the Royal College of Surgeons of England Women in Surgery Forum. She was appointed an Order of the British Empire in 2020 for services to diversity in the National Health Service in Wales.

Linda Sharples is a British statistician who is Professor of Medical Statistics at the London School of Hygiene & Tropical Medicine. Her research considers statistical analysis of medical interventions. She has provided expert advice to clinical trials on cardiovascular disease, diabetes and cancer.
