Forensic medicine

Forensic medicine is a broad term used to describe a group of medical specialties which deal with the examination and diagnosis of individuals who have been injured by or who have died because of external or unnatural causes such as poisoning, assault, suicide and other forms of violence, and apply findings to law (i.e. court cases). Forensic medicine is a multi-disciplinary branch which includes the practice of forensic pathology, forensic psychiatry, forensic dentistry, forensic radiology and forensic toxicology. [1] [2] There are two main categories of forensic medicine; Clinical forensic medicine; Pathological forensics medicine, with the differing factor being the condition of the patients. In clinical forensic medicine it is the investigation of trauma to living patients, [3] whereas pathological forensic medicine involves the examination of traumas to the deceased to find the cause of death. [4]

History

The term clinical forensic medicine, coined by Thomas Stuart, dates back to the 19th century, referring to the connection between the usage of medical evidence for judiciary purposes. Although this form of forensics medicine has been used before this term was conceived. However, clinical forensics could not be considered a thing until both legal and medical systems were well developed. But, there has been evidence of some form of forensics as far back as 220B.C.E., in the Qin Dynasty, where evidence of linking medical and legal systems were written out. [3] Forensic medicine emerged as a discipline in France in the late 18th century. [5]

Pathological forensic medicine was not considered as its own subfield until 1819, when Joan Lobstein was appoint the position of Professorship of Pathology at the University of Strassburg. [6] However, forensic pathology has been used throughout history to determine cause all factors of a death (e.g. mechanism, etc.) by examining the body of the deceased. Autopsies of animals were conducted as early as 400 B.C.E. Until the 13th century the body of the deceased were considered holy and could not be operated on. However, around 1231 C.E. the first law that allowed for the dissection and observation of a human body. This led to further easing on the concept of human autopsies, as more and more occurred. This development led to many advancements in pathology as the human body was properly mapped for its structure and function, and studied for the causes of diseases. This led to the overall increase in health as ancient technique were rid of, and new scientific medical practices were implemented. [6]

Necrosis is caused by factors external to the cell or tissue, such as infection, or trauma which result in the unregulated digestion of cell components. In contrast, apoptosis is a naturally occurring programmed and targeted cause of cellular death. While apoptosis often provides beneficial effects to the organism, necrosis is almost always detrimental and can be fatal.

Pathology is the study of disease and injury. The word pathology also refers to the study of disease in general, incorporating a wide range of biology research fields and medical practices. However, when used in the context of modern medical treatment, the term is often used in a narrower fashion to refer to processes and tests that fall within the contemporary medical field of "general pathology", an area which includes a number of distinct but inter-related medical specialties that diagnose disease, mostly through analysis of tissue and human cell samples. Idiomatically, "a pathology" may also refer to the predicted or actual progression of particular diseases, and the affix pathy is sometimes used to indicate a state of disease in cases of both physical ailment and psychological conditions. A physician practicing pathology is called a pathologist.

Anatomical pathology (Commonwealth) or anatomic pathology (U.S.) is a medical specialty that is concerned with the diagnosis of disease based on the macroscopic, microscopic, biochemical, immunologic and molecular examination of organs and tissues. The practice of chemical preservation, whereby chemical preservatives and disinfectant solutions are pumped into a body to replace natural body fluids, have made the occurrence of "coffin birth" so rare that the topic is rarely mentioned in international medical discourse.

Forensic toxicology is the use of toxicology and disciplines such as analytical chemistry, pharmacology and clinical chemistry to aid medical or legal investigation of death, poisoning, and drug use. The primary concern for forensic toxicology is not the legal outcome of the toxicological investigation or the technology utilized, but rather the obtention and interpretation of results. A toxicological analysis can be done to various kinds of samples. A forensic toxicologist must consider the context of an investigation, in particular any physical symptoms recorded, and any evidence collected at a crime scene that may narrow the search, such as pill bottles, powders, trace residue, and any available chemicals. Provided with this information and samples with which to work, the forensic toxicologist must determine which toxic substances are present, in what concentrations, and the probable effect of those chemicals on the person.

Neuropathology is the study of disease of nervous system tissue, usually in the form of either small surgical biopsies or whole-body autopsies. Neuropathologists usually work in a department of anatomic pathology, but work closely with the clinical disciplines of neurology, and neurosurgery, which often depend on neuropathology for a diagnosis. Neuropathology also relates to forensic pathology because brain disease or brain injury can be related to cause of death. Neuropathology should not be confused with neuropathy, which refers to disorders of the nerves themselves rather than the tissues. In neuropathology, the branches of the specializations of nervous system as well as the tissues come together into one field of study.

Medical jurisprudence or legal medicine is the branch of science and medicine involving the study and application of scientific and medical knowledge to legal problems, such as inquests, and in the field of law. Virtopsy is a virtual alternative to a traditional autopsy, conducted with scanning and imaging technology. The name is a portmanteau of "virtual" and "autopsy" and is a trademark registered to Richard Dirnhofer (de), the former head of the Institute of Forensic Medicine of the University of Bern, Switzerland.

Forensic nursing is defined as the application of the nursing process to public or legal proceedings, and the application of forensic health care in the scientific investigation of trauma and/or death related to abuse, violence, criminal activity, liability, and accidents.

<span class="mw-page-title-main">Earl Rose (coroner)</span> American physician

Earl Forrest Rose was an American forensic pathologist, professor of medicine, and lecturer of law. Rose was the medical examiner for Dallas County, Texas, at the time of the assassination of United States President John F. Kennedy and he performed autopsies on J. D. Tippit, Lee Harvey Oswald, and Jack Ruby. After being shoved by Kennedy's aides, he stepped aside and allowed Kennedy's body to be removed from Parkland Memorial Hospital without performing an autopsy.

In many legal jurisdictions, the manner of death is a determination, typically made by the coroner, medical examiner, police, or similar officials, and recorded as a vital statistic. Within the United States and the United Kingdom, a distinction is made between the cause of death, which is a specific disease or injury, versus manner of death, which is primarily a legal determination versus the mechanism of death which does not explain why the person died or the underlying cause of death and can include cardiac arrest or exsanguination. Different categories are used in different jurisdictions, but manner of death determinations include everything from very broad categories like "natural" and "homicide" to specific manners like "traffic accident" or "gunshot wound". In some cases an autopsy is performed, either due to general legal requirements, because the medical cause of death is uncertain, upon the request of family members or guardians, or because the circumstances of death were suspicious.

A digital autopsy is a non-invasive autopsy in which digital imaging technology, such as with computerized tomography (CT) or magnetic resonance imaging (MRI) scans, is used to develop three-dimensional images for a virtual exploration of a human body.

The stages of death of a human being have medical, biochemical and legal aspects. The term taphonomy from palaeontology applies to the fate of all kinds of remains of organisms. Forensic taphonomy is concerned with remains of the human body.
