Functional spinal unit

A functional spinal unit (FSU) (or motion segment) is the smallest physiological motion unit of the spine to exhibit biomechanical [1] characteristics similar to those of the entire spine. [2]

A FSU consists of two adjacent vertebrae, the intervertebral disc and all adjoining ligaments between them and excludes other connecting tissues such as muscles. The three-joint complex that results is sometimes referred to as the "articular triad".

In vitro studies of isolated or multiple FSU's are often used to measure biomechanical properties of the spine. The typical load-displacement behavior of a cadaveric FSU specimen is nonlinear. Within the total range of passive motion of any FSU, the typical load-displacement curve consists of 2 regions or 'zones' that exhibit very different biomechanical behavior. In the vicinity of the resting neutral position of the FSU, this load-displacement behavior is highly flexible. This is the region known as the 'neutral zone', which is the motion region of the joint where the passive osteoligamentous stability mechanisms exert little or no influence. During passive physiological movement of the FSU, motion occurs in this region against minimal internal resistance. It is a region in which a small load causes a relatively large displacement. The 'elastic zone' is the remaining region of FSU motion that continues from the end of the neutral zone to the point of maximum resistance (provided by the passive osteoligamentous stability mechanism), thus limiting the range of motion. Spinal manipulation is an intervention performed on spinal articulations, synovial joints, which is asserted to be therapeutic. These articulations in the spine that are amenable to spinal manipulative therapy include the z-joints, the atlanto-occipital, atlanto-axial, lumbosacral, sacroiliac, costotransverse and costovertebral joints. National guidelines come to different conclusions with respect to spinal manipulation with some not recommending it, and others recommending a short course in those who do not improve with other treatments.

A burst fracture is a type of traumatic spinal injury in which a vertebra breaks from a high-energy axial load, with shards of vertebra penetrating surrounding tissues and sometimes the spinal canal. The burst fracture is categorized by the "severity of the deformity, the severity of (spinal) canal compromise, the degree of loss of vertebral body height, and the degree of neurologic deficit." Burst fractures are considered more severe than compression fractures because long-term neurological damage can follow. The neurologic deficits can reach their full extent immediately, or can progress for a prolonged time.

Joint stability refers to the resistance offered by various musculoskeletal tissues that surround a skeletal joint. Several subsystems ensure the stability of a joint. These are the passive, active and neural subsystems. It is believed that one or more of the subsystems must have failed if joint instability occurs, usually a torn or overstretched ligament. Instability of joints can cause unhealthy ranges of movement in your joints which can result in the joints fracturing.

Good posture refers to the "three natural curves [that] are present in a healthy spine.". It is also called neutral spine. Looking directly at the front or back of the body, the 33 vertebrae in the spinal column should appear completely vertical. From a side view, the cervical (neck) region of the spine (C1–C7) is bent inward, the thoracic region (T1–T12) bends outward, and the lumbar region (L1–L5) bends inward. The sacrum and coccyx rest between the pelvic bones. A neutral pelvis is in fact slightly anteriorly rotated which means the anterior superior iliac spines should be just in front of the pubic symphysis not in the same vertical line.

The Golgi tendon reflex (also called inverse stretch reflex, autogenic inhibition, tendon reflex) is an inhibitory effect on the muscle resulting from the muscle tension stimulating Golgi tendon organs (GTO) of the muscle, and hence it is self-induced. The reflex arc is a negative feedback mechanism preventing too much tension on the muscle and tendon. When the tension is extreme, the inhibition can be so great it overcomes the excitatory effects on the muscle's alpha motoneurons causing the muscle to suddenly relax. This reflex is also called the inverse myotatic reflex, because it is the inverse of the stretch reflex.

Typically the integumentary system can be thought of as skin, however the integumentary system also includes the segmented exoskeleton in arthropods and feathers of birds. Soft bodied animals such as starfish rely on the arrangement of the fibers in their tube feet for movement. Vertebrates that fly, glide, and parachute also have a characteristic fiber arrangements of their flight membranes that allows for the skin to maintain its structural integrity during the stress and strain experienced during flight.

Denticulate ligaments

The denticulate ligaments, also known as dentate ligaments, are triangular shaped ligaments that anchor the spinal cord along its length, at each side, to the dura mater. The bases of the ligaments arise in the pia mater and they are firmly attached to the arachnoid mater and dura mater at the apex. They have 21 attachments per side. Their tooth-like appearance originates the word which derives from Latin denticulatus, from denticulus ‘small tooth’. The denticulate ligaments are traditionally believed to provide stability for the spinal cord against motion within the vertebral column.

Fracture of biological materials may occur in biological tissues making up the musculoskeletal system, commonly called orthopedic tissues: bone, cartilage, ligaments, and tendons. Bone and cartilage, as load-bearing biological materials, are of interest to both a medical and academic setting for their propensity to fracture. For example, a large health concern is in preventing bone fractures in an aging population, especially since fracture risk increases ten fold with aging. Cartilage damage and fracture can contribute to osteoarthritis, a joint disease that results in joint stiffness and reduced range of motion.
