The HIV/AIDS Bureau is a part of the Health Resources and Services Administration (HRSA), an agency of the United States Department of Health and Human Services. The HIV/AIDS Bureau was established in 1990 as a part of the Ryan White CARE Act and is most notable for the Ryan White HIV/AIDS Program, which stands as the U.S. government's social response to the HIV/AIDS epidemic of the late 20th century.
The original Ryan White Program was enacted in 1990 to improve the quality and availability of care for people with HIV/AIDS and their families. It was reauthorized in 1996, 2000 and 2006. Another reauthorization was due in 2009.
The HIV/AIDS Bureau was created in 1997, consolidating functions that had previously been dispersed among four other bureaus of HRSA. [1]
The Ryan White HIV/AIDS Program provides primary medical care—including dentistry—and support services to individuals living with the disease who lack health insurance or personal financial resources to pay for their own care. For most, it is the "program of last resort", covering those who do not qualify for any other health benefits program, including Medicare or Medicaid.
While clinical care and support services are the primary focus of the program, Ryan White also funds training and technical assistance for medical professionals as well as demonstration projects aimed at identifying and slowing the epidemic in high-risk populations. At present, young males of color are at greatest risk. But HIV/AIDS is also increasing among women: in 1985 women made up only 8% of total cases; by 2006 that number had increased to 23%.
HRSA-funded services are intended to reduce the use of costly inpatient care, extend screening and treatment into medically underserved populations, and improve the quality of life for those affected by the epidemic. The program achieves these goals by funding HIV/AIDS care and services through grants to state and local governments, health care providers and community-based organizations.
Through Part A of the program, HRSA provides funding to Eligible Metropolitan Areas and Transitional Grant Areas (those hardest hit by the HIV/AIDS epidemic) for a wide range of community-based efforts, such as outpatient health care and case management.
Through Part B, HRSA provides formula funding to states and territories to strengthen the quality, availability, and organizational structures of their delivery networks. This funding includes the AIDS Drug Assistance Program (ADAP), which supports the provision of prescription medications. About a third of the Ryan White budget is spent on ADAP.
Through Part C, HRSA grants support public and private non-profit providers (such as community and migrant health centers, faith-based organizations, hospitals and university medical centers, and city and county health departments) for outpatient early-intervention services and capacity-building.
As an example of what is possible, in 1989 Pernessa C. Seele worked with major religious leaders, such as Dr. Frederick B. Williams and Dr. Wyatt Tee Walker and their congregations in Harlem, New York, to create the first Harlem Week of Prayer for Healing of AIDS. She doubled the number of congregations the next year, and incorporated as "The Balm in Gilead, Inc." The non-profit now provides educational and technical assistance as a center for public health through faith communities, with some funding from the US Centers for Disease Control in Atlanta. Beginning with organizing congregations in prayers, Seele and her staff work with them to provide education and work to prevent HIV/AIDS, as well as to provide faith community support to patients and families. They have involved 10,000 congregations across the United States, as well as establishing model programs in the Caribbean and some nations in Africa.
Through Part D, HRSA funds public agencies and private non-profits to build community- and faith-based programs for children, youth, women and families. Part D also provides for a Youth Services Initiative to identify HIV-infected adolescents; bring them into comprehensive, culturally and linguistically sensitive care; and retain them in treatment.
Part F includes the Special Projects of National Significance Program, which provides funding to public and private nonprofit entities to develop innovative models of HIV care for underserved populations. Part F also includes the Dental Reimbursement Program, the Community-Based Dental Partnership Program and the AIDS Education and Training Centers.
Part A funds eligible metropolitan areas and transitional grant areas. 75% must be spent for core services.

Part D grants support services for women, infants, children and youth.

The United States President's Emergency Plan For AIDS Relief (PEPFAR) is a United States governmental initiative to address the global HIV/AIDS epidemic and help save the lives of those suffering from the disease. Launched by U.S. President George W. Bush in 2003, as of May 2020, PEPFAR has provided about $90 billion in cumulative funding for HIV/AIDS treatment, prevention, and research since its inception, making it the largest global health program focused on a single disease in history until the COVID-19 pandemic. PEPFAR is implemented by a combination of U.S. government agencies in over 50 countries and overseen by the Global AIDS Coordinator at the United States Department of State. As of 2023, PEPFAR has saved over 25 million lives, primarily in sub-Saharan Africa.

The Ryan White Comprehensive AIDS Resources Emergency Act was an act of the United States Congress and is the largest federally funded program in the United States for people living with HIV/AIDS. In exchange for States adopting harsh criminal laws regulating the conduct of HIV-positive individuals and providing for their public felony prosecution, the act made federal funding available through contingency grants to states for low-income, uninsured, and under-insured people to be treated with the chemotherapeutic drug AZT. The act is named in honor of Ryan White, an Indiana teenager who contracted HIV through a tainted blood transfusion. He was diagnosed with AIDS in 1984 at age 13 and was subsequently expelled from school because of the disease. White became a well-known advocate for AIDS research and awareness until his death in 1990 at age 18.
Whitman-Walker Health (WWH), formerly Whitman-Walker Clinic, is a non-profit community health center in the Washington, D.C. metropolitan area with a special expertise in HIV/AIDS healthcare and LGBT healthcare. Chartered as an affirming health center for the gay and lesbian community in 1978, Whitman-Walker was one of the first responders to the HIV/AIDS epidemic in D.C. and became a leader in HIV/AIDS education, prevention, diagnosis and treatment. In recent years, Whitman-Walker has expanded its services to include primary healthcare services, a stronger focus on queer women's care and youth services.

The AETCs were established in 1987 through federal funding from the Health Resources and Services Administration (HRSA) Bureau of Health Professions. In 1997, the AETCs became a component of the Ryan White HIV/AIDS Program.
Healthy Way LA (HWLA) was a free public health care program available to underinsured or uninsured, low-income residents of Los Angeles County from 2007 until 2014. The program, administered by the Los Angeles County Department of Health Services (LADHS), was a Low Income Health Program (LIHP) approved under the Section 1115 Medicaid Waiver. AIDS amendments of 1988, better known as the Health Omnibus Programs Extension (HOPE) Act of 1988, is a United States statute amending the Public Health Service Act. The Acquired Immune Deficiency Syndrome amendments were compiled as Title II - Programs with Respect to Acquired Immune Deficiency Syndrome within the HOPE Act of 1988. The Title II Act appropriated federal funding for Acquired Immune Deficiency Syndrome (AIDS) education, prevention, research, and testing. The U.S. legislative title provisioned the establishment of the presidentially appointed National Commission on AIDS. The S. 2889 legislation was passed by the 100th U.S. Congressional session and signed by President Ronald Reagan on November 4, 1988.

He has also been director of health for Wichita, Kansas; medical director of Planned Parenthood of Kansas; director of the Robert Wood Johnson AIDS Health Services Program; director of the Office of Consumer Affairs for the Food and Drug Administration; and director of the East Asia and Pacific Regional Medical Office of the Peace Corps.

AIHA is contributing to improve the worldwide health conditions. This organisation has been associated and largely contributed in the HIV-related area since 2000.
ADAP Advocacy is an American national 501(c)(3) non-profit organization based in Washington, D.C., that is dedicated to promoting and enhancing the AIDS Drug Assistance Programs (ADAPs) and improving access to care and treatment for Persons Living with HIV/AIDS (PLWHA) in the United States and the U.S. Territories.
