Plot

On October 31, 2018, Cursed Spirits led by Suguru Geto place veils over Shibuya, trapping all the people inside, forcing Satoru Gojo to enter. During the encounter, Geto succeeds in tricking Gojo, trapping him in the Prison Realm. Before he's sealed away, Gojo confirms that "Geto" is in fact an imposter, a mysterious brain controlling the body.

The rest of the Jujutsu Sorcerers enter Shibuya, facing off against the curse spirits to reclaim the Prison Realm and free Gojo. Yuji Itadori fights against the cursed womb Choso, whose seeking revenge for Yuji having killed his brothers. Choso nearly kills Yuji, but traumatically leaves when he sees a vision of them as brothers. Megumi Fushiguro briefly encounters his resurrected father, Toji Zen'in, who contently kills himself when he learns Megumi took the "Fushiguro" name. Kento Nanami, Maki Zen'in, and Naobito Zen'in are burned by the Cursed Spirit Jogo, who then force feeds 10 fingers into Yuji, transforming him into King of Curses Ryomen Sukuna.

Sukuna fights and kills Jogo, when he notices Megumi activate the most dangerous of his Ten Shadows shikigami, Mahoraga, in a suicide ploy to defeat another Curse User. Not wanting Megumi to die, Sukuna battles Mahoraga, successfully defeating it by activating his Domain Expansion, which also decimates a large portion of Shibuya and its inhabitants. Yuji breaks down emotionally blaming himself for the death and destruction.

He shortly afterwards witnesses the Cursed Spirit Mahito kill Nanami, and angrily starts fighting him. Nobara Kugisaki soon joins in to fight Mahito, only to be severely wounded. A distraught Yuji resigns himself, but is saved by Aoi Todo, who convinces him to regain his resolve, and they resume the fight. Yuji eventually gains the upperhand and defeats Mahito in battle, however Pseudo-Geto appears and absorbs Mahito for himself using his Curse Technique.

Choso returns and reveals Pseudo-Geto is Noritoshi Kamo, a Curse User from the past (and one of his many aliases) responsible for his creation and goes to fight him. They are then assisted by Yuki Tsukumo, intending to challenge Pseudo-Geto's ideology. Pseudo-Geto proceeds to use Mahito's Idle Transfiguration on Japan, activating two sets of people: those with latent potential to become Jujutsu sorcerers, and reincarnated sorcerers from the past. He soon escapes with the Prison Realm, while taunting Yuji, unleashing a hoard of cursed spirits on Tokyo to keep everyone busy.

In the aftermath of the Shibuya Incident, Gojo is labelled as an accomplice to Geto, and the jujutsu higher-ups resume Yuji's execution, with Yuta Okkotsu selected to be the executioner.

At the Zen'in Clan's headquarters, Naobito's death is confirmed and his will is shared with key members of the clan, including Naobito's misogynistic son, Naoya. While he normally would become the head of the clan, an arrangement by Gojo and Toji states that if the former were to be indisposed in any way, Megumi would become the head. An infuriated Naoya goes to hunt Megumi and Yuji to claim his spot as clan head.

Meanwhile, Yuji has remained in the city with Choso fighting cursed spirits, discussing his place in Jujutsu society, believing he doesn't deserve to return to Jujutsu High. Naoya appears before the two, but before their fight can begin, Yuta arrives onto the scene; Yuta gives chase to Yuji while Choso stays behind to fight Naoya.

Yuji surprises Yuta with his strength and speed, so Yuta is forced to use his cursed spirit partner, Rika, to subdue and execute him. Meanwhile, Choso is initially overwhelmed by Naoya's immense speed, but is able to turn the tide using his poisonous blood. Yuta arrives and knocks Choso out, requesting Naoya to inform the higher-ups about Yuji's death in exchange for healing him.

Yuji has a vision of himself as a baby, in which his grandfather, Wasuke, warns his son, Jin, about his mysteriously resurrected wife, Kaori, who walks in with stitches on her forehead. Yuji wakes up in front of Yuta, who reveals that his death was staged by him as per Gojo's request to protect Yuji. Having been in a similar position to him, he also tries to reassure Yuji that he's not to blame for the deaths at Sukuna's hands.

Megumi arrives to ask Yuji to help him save his step-sister, Tsumiki, who has been ensnared in a "Culling Game" by Pseudo-Geto, who he is planning to force all of the awakened sorcerers to fight in. Yuji agrees, informing Yuta that Sukuna has some plan for Megumi and to kill him if he ever retakes control of his body again. The three and Choso go to Jujutsu High where they reunite with Yuki and a recovering Maki. Using Choso's connection to his cursed womb siblings, they are able to enter the correct paths through the Tombs of the Star Corridor in order to confront the immortal sorcerer Master Tengen on information for how to deal with the Culling Games and rescue Gojo. They enter a white void room, where Tengen reveals themself to them.
