Laika is an open-source Electronic Health Record (EHR) testing framework that analyzes and reports on the interchangeability of EHR systems. This includes the testing for certification of EHR software products and networks. Laika is designed to verify the input and output of EHR data against the standards and criteria identified by the Certification Commission for Healthcare Information Technology (CCHIT). [1]

In June 2008, the CCHIT began using Laika to automate EHR interchangeability testing, [2] but by December 2011, the organization had suspended its use of the framework. [3]

Interoperability packages

Laika C32

The first tool to be created in the Laika framework suite, it supports testing of the HL7/ASTM Continuity of Care Document (CCD) constrained by the HITSP C32 version 2.1 specification. [4]

Laika ORU

Released in September 2008 to test the interoperability of HL7 2.5.1 lab messages. Laika ORU can be used with Mirth, an open-source health informatics messaging package, to manage the routing of HL7 2.5.1 lab messages with Laika.[ citation needed ]

Laika XDS

Scheduled to be released in March 2009 to test EHR systems and Health Information Exchange systems with XDS registries and repositories.[ citation needed ]

Development collaboration

Laika is an active collaborative effort between the CCHIT and the MITRE Corporation. The CCHIT is leading the functional requirements definition of the Laika testing framework. MITRE is leading the technical software design and is prototyping the software service.[ citation needed ]

The Certification Commission is a private, not-for-profit organization, whose mission is to accelerate adoption of health information technology in the United States. MITRE is a 501(c)(3) not-for-profit corporation that manages three Federally Funded Research and Development Centers (FFRDCs) and works in partnership with the US government, applying systems engineering and advanced technology to address issues of critical national importance in the USA.[ citation needed ]

Deployment in virtual environments

Laika has been deployed in virtual environments using, for example, the Amazon cloud environment. This allows centralized testing of multiple EHRs in segmented environments. It also allows portable implementations, so that field testing can be achieved.

Technical details

Laika is licensed under an Apache 2.0 open source license. Laika uses the Ruby on Rails framework, the Java programming language, the open source PostgreSQL database, and several Web 2.0 JavaScript libraries including Scriptaculous and Prototype.

In 2010, the core Laika software infrastructure, consisting of the Laika database and Rails controllers, was forked to support the open source popHealth project. The popHealth project was developed from resources provided by the Federal Health Architecture within the Office of the National Coordinator. popHealth integrates with a healthcare provider's electronic health record (EHR) system to produce summary quality measures on the provider's patient population. The MITRE Corporation was also tasked as the technical lead team for the popHealth activity.

Name

Laika is named after the dog and first living animal to enter earth orbit, paving the way for human space flight. [5]

Health Level Seven or HL7 refers to a set of international standards for transfer of clinical and administrative data between software applications used by various healthcare providers. These standards focus on the application layer, which is "layer 7" in the OSI model. The HL7 standards are produced by Health Level Seven International, an international standards organization, and are adopted by other standards issuing bodies such as American National Standards Institute and International Organization for Standardization.

Electronic health record

An electronic health record (EHR) is the systematized collection of patient and population electronically stored health information in a digital format. These records can be shared across different health care settings. Records are shared through network-connected, enterprise-wide information systems or other information networks and exchanges. EHRs may include a range of data, including demographics, medical history, medication and allergies, immunization status, laboratory test results, radiology images, vital signs, personal statistics like age and weight, and billing information.

The eHealth Exchange, formerly known as the Nationwide Health Information Network, is an initiative for the exchange of healthcare information. It was developed under the auspices of the U.S. Office of the National Coordinator for Health Information Technology (ONC), and now managed by a non-profit industry coalition called Sequoia Project. The exchange is a web-services based series of specifications designed to securely exchange healthcare related data. The NwHIN is related to the Direct Project which uses a secure email-based approach. One of the latest goals is to increase the amount of onboarding information about the NwHIN to prospective vendors of health care systems.

The HL7 Clinical Document Architecture (CDA) is an XML-based markup standard intended to specify the encoding, structure and semantics of clinical documents for exchange. In November 2000, HL7 published Release 1.0. The organization published Release 2.0 with its "2005 Normative Edition."

The Certification Commission for Health Information Technology (CCHIT) was an independent, 501(c)3 nonprofit organization with the public mission of accelerating the adoption of robust, interoperable health information technology. The Commission certified electronic health record technology from 2006 until 2014. It was approved by the Office of the National Coordinator for Health Information Technology (ONC) of the U.S. Department of Health and Human Services (HHS) as an Authorized Testing and Certification Body (ONC-ATCB). The CCHIT Certified program is an independently developed certification that includes a rigorous inspection of an EHR's integrated functionality, interoperability and security using criteria developed by CCHIT's broadly representative, expert work groups. These products may also be certified in the ONC-ATCB certification program.

HOSxP is a hospital information system, including Electronic health record (EHR), used in hospitals across Thailand. It serves over 300 hospitals in Thailand. The software aims to ease the healthcare workflow of health centers, for small sanatoriums to central hospitals.

European Institute for Health Records

The European Institute for Health Records or EuroRec Institute is a non-profit organization founded in 2002 as part of the ProRec initiative. On 13 May 2003, the institute was established as a non-profit organization under French law. Current President of EuroRec is Prof. Dipak Kalra. The institute is involved in the promotion of high quality Electronic Health Record systems in the European Union. One of the main missions of the institute is to support, as the European authorised certification body, EHRs certification development, testing and assessment by defining functional and other criteria.

The ISO/TC 215 is the International Organization for Standardization's (ISO) Technical Committee (TC) on health informatics. TC 215 works on the standardization of Health Information and Communications Technology (ICT), to allow for compatibility and interoperability between independent systems.

Health information technology (HIT) is health technology, particularly information technology, applied to health and health care. It supports health information management across computerized systems and the secure exchange of health information between consumers, providers, payers, and quality monitors. InterSystems Corporation is a privately held vendor of software systems and technology for high-performance database management, rapid application development, integration, and healthcare information systems. LSS Data Systems

LSS Data Systems (LSS), was a Minnesota-based medical software and service company that develops products for the physician practice community. LSS was founded in 1982, and since then was a partner of Medical Information Technology (MEDITECH), developing physician practice management and ambulatory electronic health record software. In 2000 and 2001 MEDITECH invested in LSS and in February 2011, acquired complete ownership. LSS became a wholly owned subsidiary of the Massachusetts based company, and announced completion of the merger on January 1, 2014.

NextGen Healthcare, Inc. is an American software and services company headquartered in Irvine, California, US. The company develops and sells electronic health record (EHR) software and practice management systems to the healthcare industry. NextGen Healthcare also provides population health, financial management, and clinical solutions for medical and dental practices. On September 7, 2018, Quality Systems Inc. changed its name to Nextgen Healthcare, Inc. and on September 10, their stock ticker symbol changed to NASDAQ: NXGN.

Fast Healthcare Interoperability Resources is a standard describing data formats and elements and an application programming interface (API) for exchanging electronic health records (EHR). The standard was created by the Health Level Seven International (HL7) health-care standards organization.

Medical device connectivity is the establishment and maintenance of a connection through which data is transferred between a medical device, such as a patient monitor, and an information system. The term is used interchangeably with biomedical device connectivity or biomedical device integration. By eliminating the need for manual data entry, potential benefits include faster and more frequent data updates, diminished human error, and improved workflow efficiency.

Health Level Seven International (HL7) is a non-profit ANSI-accredited standards development organization dedicated to providing standards and solutions that empower global health data interoperability.
