In neurourology, post-micturition convulsion syndrome (PMCS), also known informally as pee shivers or piss shivers, is the experience of shivering during or after urination. [1] The syndrome seems to be experienced equally by men and women. [2]

The term "post-micturition convulsion syndrome" was coined in 1994 in the online question-and-answer newspaper column The Straight Dope , when a reader inquired about the phenomenon. [3]

Explanation

There has yet to be any peer-reviewed research on the topic. [2] The most plausible theory is that the shiver is a result of the autonomic nervous system getting its signals mixed up between its two main divisions: [4]

Part of the SNS response to a full bladder is the release of catecholamines (including epinephrine, norepinephrine and dopamine), which are dispatched to help restore or maintain blood pressure. [1] When urination begins, the PNS takes over, and catecholamine production changes. It may be the change in chemical production which causes the shiver, or the switch from SNS to PNS itself which is the cause. [4]

Shivering is a bodily function in response to cold and extreme fear in warm-blooded animals. When the core body temperature drops, the shivering reflex is triggered to maintain homeostasis. Skeletal muscles begin to shake in small movements, creating warmth by expending energy. Shivering can also be a response to fever, as a person may feel cold. During fever, the hypothalamic set point for temperature is raised. The increased set point causes the body temperature to rise (pyrexia), but also makes the patient feel cold until the new set point is reached. Severe chills with violent shivering are called rigors. Rigors occur because the patient's body is shivering in a physiological attempt to increase body temperature to the new set point.

Taking the piss is a colloquial term meaning to mock at the expense of others, or to be joking, without the element of offence. It is also sometimes phrased as a question, 'are they taking the piss?', when referring to an individual who takes above and beyond what is thought acceptable, similar to the expression, 'give them an inch and they take a mile.' It is a shortening of the idiom taking the piss out of, which is an expression meaning to mock, tease, joke, ridicule, or scoff. It is not to be confused with "taking a piss", which refers to the act of urinating. Taking the Mickey, taking the Mick or taking the Michael are additional terms for making fun of someone. These terms are most often used in the United Kingdom, Ireland, South Africa, New Zealand, and Australia.

Incontinence is another easily detectable symptom of the syndrome that is due to detrusor-sphincter discoordination.

In urology, voiding cystourethrography (VCUG) is a frequently performed technique for visualizing a person's urethra and urinary bladder while the person urinates (voids). It is used in the diagnosis of vesicoureteral reflux, among other disorders. The technique consists of catheterizing the person in order to fill the bladder with a radiocontrast agent, typically diatrizoic acid. Under fluoroscopy the radiologist watches the contrast enter the bladder and looks at the anatomy of the patient. If the contrast moves into the ureters and back into the kidneys, the radiologist makes the diagnosis of vesicoureteral reflux, and gives the degree of severity a score. The exam ends when the person voids while the radiologist is watching under fluoroscopy. Consumption of fluid promotes excretion of contrast media after the procedure. It is important to watch the contrast during voiding, because this is when the bladder has the most pressure, and it is most likely this is when reflux will occur. Despite this detailed description of the procedure, at least as of 2016 the technique had not been standardized across practices.

<span class="mw-page-title-main">Bladder sphincter dyssynergia</span> Medical condition

Bladder sphincter dyssynergia is a consequence of a neurological pathology such as spinal injury or multiple sclerosis which disrupts central nervous system regulation of the micturition (urination) reflex resulting in dyscoordination of the detrusor muscles of the bladder and the male or female external urethral sphincter muscles. In normal lower urinary tract function, these two separate muscle structures act in synergistic coordination. But in this neurogenic disorder, the urethral sphincter muscle, instead of relaxing completely during voiding, dyssynergically contracts causing the flow to be interrupted and the bladder pressure to rise.

Neurogenic bladder dysfunction, often called by the shortened term neurogenic bladder, refers to urinary bladder problems due to disease or injury of the central nervous system or peripheral nerves involved in the control of urination. There are multiple types of neurogenic bladder depending on the underlying cause and the symptoms. Symptoms include overactive bladder, urinary urgency, frequency, incontinence or difficulty passing urine. A range of diseases or conditions can cause neurogenic bladder including spinal cord injury, multiple sclerosis, stroke, brain injury, spina bifida, peripheral nerve damage, Parkinson's disease, multiple system atrophy or other neurodegenerative diseases. Neurogenic bladder can be diagnosed through a history and physical as well as imaging and more specialized testing. In addition to symptomatic treatment, treatment depends on the nature of the underlying disease and can be managed with behavioral changes, medications, surgeries, or other procedures. The symptoms of neurogenic bladder, especially incontinence, can severely degrade a person's quality of life.

A pissing contest, pissing duel, or pissing match, is a game in which participants compete to see who can urinate the highest, the farthest, for the longest, or the most accurately. Although the practice is usually associated with adolescent boys, women have been known to play the game, and there are literary depictions of adults competing in it. Since the 1940s, the term has been used as a slang idiomatic phrase describing contests that are "futile or purposeless", especially if waged in a "conspicuously aggressive manner". As a metaphor it is used figuratively to characterise futile ego-driven battling in a pejorative or facetious manner that is often considered vulgar. The image of two people urinating on each other has also been offered as a source of the phrase.

<span class="mw-page-title-main">Overflow incontinence</span> Medical condition

Overflow incontinence is a concept of urinary incontinence, characterized by the involuntary release of urine from an overfull urinary bladder, often in the absence of any urge to urinate. This condition occurs in people who have a blockage of the bladder outlet, or when the muscle that expels urine from the bladder is too weak to empty the bladder normally. Overflow incontinence may also be a side effect of certain medications.
