Risk of infection is a nursing diagnosis which is defined as the state in which an individual is at risk to be infected by an opportunistic or pathogenic agent (e.g., viruses, fungi, bacteria, protozoa, or other parasites) from endogenous or exogenous sources. [1] The diagnosis was approved by NANDA in 1986. Although anyone can become infected by a pathogen, patients with this diagnosis are at an elevated risk and extra infection controls should be considered.

Endogenous sources

The risk of infection depends on a number of endogenous sources. [2] Skin damage from incision as well as very young or old age can increase a patient's risk of infection. Examples of risk factors includes decreased immune system secondary to disease, compromised circulation secondary to peripheral vascular disease, compromised skin integrity secondary to surgery, or repeated contact with contagious agents.

Assessment

The patient should be asked about a history of repeated infections, symptoms of infection, recent travel to high-risk areas, and their immunization history. They should also be assessed for objective signs such as the presence of wounds, fever, or signs of nutritional deficiency

Intervention

The specific nursing interventions will depend on the nature and severity of the risk. Patients should be taught how to recognize the signs of infection and how to reduce their risk. Surgery is a frequent risk factor for infection and a physician may prescribe antibiotics prophylactically. Immunization is another common medical intervention for those who are at high risk for infection. Hand washing is the best way to break the chain of infection.

Candidiasis is a fungal infection due to any species of the genus Candida. When it affects the mouth, in some countries it is commonly called thrush. Signs and symptoms include white patches on the tongue or other areas of the mouth and throat. Other symptoms may include soreness and problems swallowing. Acute septic arthritis, infectious arthritis, suppurative arthritis, pyogenic arthritis, osteomyelitis, or joint infection is the invasion of a joint by an infectious agent resulting in joint inflammation. Generally speaking, symptoms typically include redness, heat and pain in a single joint associated with a decreased ability to move the joint. Onset is usually rapid. Other symptoms may include fever, weakness and headache. Occasionally, more than one joint may be involved, especially in neonates, younger children and immunocompromised individuals. In neonates, infants during the first year of life, and toddlers, the signs and symptoms of septic arthritis can be deceptive and mimic other infectious and non-infectious disorders.

Complications may include backward blood flow in the heart, heart failure – the heart struggling to pump a sufficient amount of blood to meet the body's needs, abnormal electrical conduction in the heart, stroke, and kidney failure.

Cellulitis is usually a bacterial infection involving the inner layers of the skin. It specifically affects the dermis and subcutaneous fat. Signs and symptoms include an area of redness which increases in size over a few days. The borders of the area of redness are generally not sharp and the skin may be swollen. While the redness often turns white when pressure is applied, this is not always the case. The area of infection is usually painful. Lymphatic vessels may occasionally be involved, and the person may have a fever and feel tired.

Fever of unknown origin (FUO) refers to a condition in which the patient has an elevated temperature (fever) for which no cause can be found despite investigations by one or more qualified physicians. If the cause is found, it is usually a diagnosis of exclusion, eliminating all possibilities until only the correct explanation remains.

Chickenpox, also known as varicella, is a highly contagious, vaccine-preventable disease caused by the initial infection with varicella zoster virus (VZV), a member of the herpesvirus family. The disease results in a characteristic skin rash that forms small, itchy blisters, which eventually scab over. It usually starts on the chest, back, and face. It then spreads to the rest of the body. The rash and other symptoms, such as fever, tiredness, and headaches, usually last five to seven days. Complications may occasionally include pneumonia, inflammation of the brain, and bacterial skin infections. The disease is usually more severe in adults than in children.

Pneumococcal infection is an infection caused by the bacterium Streptococcus pneumoniae.

Transmission-based precautions are infection-control precautions in health care, in addition to the so-called "standard precautions". They are the latest routine infection prevention and control practices applied for patients who are known or suspected to be infected or colonized with infectious agents, including certain epidemiologically important pathogens, which require additional control measures to effectively prevent transmission. Universal precautions are also important to address as far as transmission-based precautions. Universal precautions is the practice of treating all bodily fluids as if it is infected with HIV, HBV, or other blood borne pathogens.

Anaerobic infections are caused by anaerobic bacteria. Obligately anaerobic bacteria do not grow on solid media in room air ; facultatively anaerobic bacteria can grow in the presence or absence of air. Microaerophilic bacteria do not grow at all aerobically or grow poorly, but grow better under 10% carbon dioxide or anaerobically. Anaerobic bacteria can be divided into strict anaerobes that can not grow in the presence of more than 0.5% oxygen and moderate anaerobic bacteria that are able of growing between 2 and 8% oxygen. Anaerobic bacteria usually do not possess catalase, but some can generate superoxide dismutase which protects them from oxygen.

<span class="mw-page-title-main">Neonatal infection</span> Human disease

Neonatal infections are infections of the neonate (newborn) acquired during prenatal development or within the first four weeks of life. Neonatal infections may be contracted by mother to child transmission, in the birth canal during childbirth, or after birth. Neonatal infections may present soon after delivery, or take several weeks to show symptoms. Some neonatal infections such as HIV, hepatitis B, and malaria do not become apparent until much later. Signs and symptoms of infection may include respiratory distress, temperature instability, irritability, poor feeding, failure to thrive, persistent crying and skin rashes.

Tertiary peritonitis is the inflammation of the peritoneum which persists for 48 hours after a surgery that has been successfully carried out in adequate surgical conditions. Tertiary peritonitis is usually the most delayed and severe consequence of nosocomial intra-abdominal infection. Patients who acquire tertiary peritonitis are usually admitted to ICU due to the critical, life-threatening nature of the condition which can lead to multi-organ failure despite treatment and has a high mortality rate of 60%. Signs and symptoms of tertiary peritonitis include fever, hypotension and abdominal pain. Diagnosis of the condition is often difficult and treatment intervention should be as early as possible.

<span class="mw-page-title-main">Diabetic foot infection</span> Medical condition

Diabetic foot infection is any infection of the foot in a diabetic person. The most frequent cause of hospitalization for diabetic patients is due to foot infections. Symptoms may include pus from a wound, redness, swelling, pain, warmth, tachycardia, or tachypnea. Complications can include infection of the bone, tissue death, amputation, or sepsis. They are common and occur equally frequently in males and females. Older people are more commonly affected.
