A rocker sole shoe or rocker bottom shoe is a shoe which has a thicker-than-normal sole with rounded heel. Such shoes ensure the wearer does not have flat footing along the proximal-distal axis of the foot. The shoes are generically known by a variety of names including round bottom shoes, [1] round/ed sole shoes, [2] and toning shoes, [3] but also by various brand names. [4] Tyrell & Carter identified at least six standard variations of the rocker sole shoe and named them: toe-only rocker, rocker bar, mild rocker, heel-to-toe rocker, negative heel rocker and double rocker. [1]
Rocker soles may replace regular soles on any style of footwear. Some rocker bottom shoes are purpose built to reduce the function or replace the lost function of a joint. For example, a person with a hallux rigidus (stiff big toe) may use a rocker bottom shoe to replace the flexion lost at the metatarsal joint. Rocker bottom shoes are also used to compensate for the lost range of motion, however caused, at the tibiotalar joint (ankle joint). In such cases, the wearer maintains solid and stable footing while standing, but the rock of the heel assists with the propulsive phase of gait, making walking more natural and less painful to the affected joints. Beneficiaries of this type of sole modification include people suffering from arthritis or any other disorder or injury causing pain and/or loss of motion in foot joints.[ citation needed ]
The construction of most varieties of rocker sole shoes mean that the wearer's body weight is shifted behind the ankle and the wearer is required to do more work than would be required in flat-soled shoe to find their center of gravity and remain balanced. [5] In the 2000s, a heel-to-toe rocker sole shoe for the sports footwear market was popularized by brands such as MBT, Shape-Ups by Skechers, and Reebok EasyTone. [5]
Rocker sole shoes have been referenced in publications as early as 1990. [6] Branded generic rocker sole shoes were popularized for the mass market in the late 1990s and 2000s by the Swiss Masai company as Masai Barefoot Technology or MBT. [7] [8] According to Swiss Masai, the market concept originated with engineer and former athlete Karl Müller who intended they would "simulate the challenge of walking barefoot on soft earth". [2] [7] Various other sports footwear companies followed suit with their own branded versions of the heel-to-toe rocker targeted at the exercise equipment market. [2] A news report estimated that 200,000 pairs of modern heel-to-toe rocker sole shoes were sold in the US in 2005. [7]
Proponents of modern heel-to-toe rocker sole shoes claim that because the foot of the wearer is slightly destabilised, certain lesser-worked muscle groups in the leg, such as the core and gluteus, are challenged more than they are normally. As such they are purported to derive health benefits for the wearer, such as improved posture and tighter muscles. [2] [5]
The NICE guidelines in the UK state that healthcare professionals should not offer rocker-bottom shoes as a treatment for people with lower back pain or sciatica. [9] No studies found significant health benefits in the short- or long-term, and several found significant harms in terms of worsened anxiety and depression among people who had lower back pain. [10] One study found that flat-bottomed shoes may be better for people whose lower back pain is aggravated by walking or standing. [11]
Rocker-bottom shoes do not increase muscle use in the legs or burn more calories. [12] A 2010 University of Wisconsin study commissioned by the American Council on Exercise compared exercise wearing rocker bottom shoes and regular running shoes. The study found there was no fitness benefit to wearing rocker sole shoes. [3] The study was cited in a 2011 class-action lawsuits alleging false advertising by New Balance, Reebok, and other manufacturers. [13]
A 2009 study raised concern that use of rocker sole shoes may increase the risk of falls. [14] While research by Wang (2009) indicates that this may not be a problem in able-bodied individuals, it has been demonstrated to be a concern in older individuals who experience balance problems. [15]
When rocker sole shoes were compared with flat sole shoes no benefit was observed favouring rocker sole shoes for any of the reported outcomes in either the short or long term follow-up. It was noted that health-related quality of life was in fact, worse in the rocker sole group at both the short and longer term time points.

Pes cavus, also known as high arch, is a human foot type in which the sole of the foot is distinctly hollow when bearing weight. That is, there is a fixed plantar flexion of the foot. A high arch is the opposite of a flat foot and is somewhat less common.

This is because of the foot's distance from the central circulation, the heart and its constant exposure to pressures from the ground and the weight of the body.
Flat feet, also called pes planus or fallen arches, is a postural deformity in which the arches of the foot collapse, with the entire sole of the foot coming into complete or near-complete contact with the ground. Sometimes children are born with flat feet (congenital). There is a functional relationship between the structure of the arch of the foot and the biomechanics of the lower leg. The arch provides an elastic, springy connection between the forefoot and the hind foot so that a majority of the forces incurred during weight bearing on the foot can be dissipated before the force reaches the long bones of the leg and thigh.
High-heeled shoes, also known as high heels or pumps, are a type of shoe with an upward-angled sole. The heel in such shoes is raised above the ball of the foot. High heels cause the legs to appear longer, make the wearer appear taller, and accentuate the calf muscle. The term pumps does not strictly refer to shoes with high heels. Pumps may refer to a court shoe.
Sever's disease, also known as calcaneus apophysitis, is an inflammation at the back of the heel growth plate in growing children. The condition is thought to be caused by repetitive stress at the heel. This condition is benign and common and usually resolves when the growth plate has closed or during periods of less activity. It occurs in both males and females. There are a number of locations in the body that may get apophysitis pain. Another common location is at the front of the knee which is known as apophysitis of the tibial tuberosity or Osgood–Schlatter disease.

Hallux rigidus or stiff big toe is degenerative arthritis and stiffness due to bone spurs that affects the metatarsophalangeal joints (MTP) at the base of the hallux.
Pronation is a natural movement of the foot that occurs during foot landing while running or walking. Composed of three cardinal plane components: subtalar eversion, ankle dorsiflexion, and forefoot abduction, these three distinct motions of the foot occur simultaneously during the pronation phase. Pronation is a normal, desirable, and necessary component of the gait cycle. Pronation is the first half of the stance phase, whereas supination starts the propulsive phase as the heel begins to lift off the ground.
The Vibram FiveFingers are a type of minimalist shoe manufactured by Vibram, originally marketed as a more natural alternative for outdoor activities. The footwear is meant to replicate being barefoot and has thin, flexible soles that are contoured to the shape of the human foot, including visible individual sections for the toes. The company settled a lawsuit in 2014 alleging false health claims and set aside $3.75 million to pay refunds of up to $94 to anyone who had purchased the product since March 21, 2009.
Locomotor effects of shoes are the way in which the physical characteristics or components of shoes influence the locomotion neuromechanics of a person. Depending on the characteristics of the shoes, the effects are various, ranging from alteration in balance and posture, muscle activity of different muscles as measured by electromyography (EMG), and the impact force. There are many different types of shoes that exist, such as running, walking, loafers, high heels, sandals, slippers, work boots, dress shoes, and many more. However, a typical shoe will be composed of an insole, midsole, outsole, and heels, if any. In an unshod condition, where one is without any shoes, the locomotor effects are primarily observed in the heel strike patterns and resulting impact forces generated on the ground.

Minimalist or barefoot shoes are intended to closely approximate barefoot running or walking conditions in comparison to traditional shoes. Minimalist shoes are defined as providing "minimal interference with the natural movement of the foot, because of its high flexibility, low heel to toe drop, weight and stack height, and the absence of motion control and stability devices." Minimalist shoes provide more sensory contact for the foot on the ground while simultaneously providing the feet with some protection from ground hazards and conditions. Research shows that wearing a minimalist shoe can help improve running economy, foot strength and arch function.

Running injuries affect about half of runners annually. The frequencies of various RRI depend on the type of running, such as speed and mileage. Some injuries are acute, caused by sudden overstress, such as side stitch, strains, and sprains. Many of the common injuries that affect runners are chronic, developing over longer periods as the result of overuse. Common overuse injuries include shin splints, stress fractures, Achilles tendinitis, Iliotibial band syndrome, Patellofemoral pain, and plantar fasciitis.
Ankle problems occur frequently, having symptoms of pain or discomfort in the ankles.
The toe box is the section of footwear that surrounds the toes on closed-toe shoes. Toe boxes that are too tight can cause injuries and foot deformities, whereas wider toe boxes may be used to treat or prevent common foot conditions such as Morton's neuroma. Toe boxes come in a variety of shapes and styles of construction, some of which are a matter of fashion, and some of which are designed for specialized functions.
