The route heads north from Lancaster and runs through suburban and rural areas in northern Lancaster County, passing through Lititz and crossing US 322 in Brickerville. PA 501 continues into Lebanon County and heads into the Lebanon Valley, where it passes through Schaefferstown and intersects US 422 in Myerstown. The route passes through western Berks County, where it has an interchange with Interstate 78 (I-78)/US 22 near the community of Bethel. PA 501 crosses Blue Mountain into Schuylkill County and continues to its northern terminus.
On May 12, 2007, Alec Kreider, a 16-year-old high school student at the time, murdered his classmate and friend Kevin Haines, his father Thomas Haines, and mother Lisa Haines in their home in Manheim Township, Pennsylvania, United States. Kreider was arrested a month later on June 16, 2007, and was charged with three counts of first degree murder. He pleaded guilty to all charges, and was subsequently convicted on all three counts and sentenced to three consecutive life sentences without the possibility of parole. Kreider later became eligible for a re-sentencing hearing following the United States Supreme Court's 2012 decision in Miller v. Alabama, which held that mandatory sentences of life without the possibility of parole are unconstitutional for juvenile offenders.

The Red Caboose Motel is a 48-room train motel in the Amish country near Ronks, in Lancaster County, Pennsylvania, where guests stay in railroad cabooses. The motel consists of over three dozen cabooses and other railroad cars, such as dining cars that serve as a restaurant. It was developed and opened in 1970 by Donald M. Denlinger, who started with 19 surplus cabooses purchased at auction from the Penn Central Railroad. The expanded and renovated property has also hosted railroad-themed events and concerts and dances in its barn.
