Sir Arthur Mitchell MD KCB FRSE LLD (19 January 1826 – 12 October 1909) was a Scottish doctor involved in the study and care of patients with mental illness. He served on several public commissions, and wrote widely on history and anthropology.
Mitchell was born in Elgin, Moray, the son of George Mitchell, a civil engineer, and his wife, Elizabeth Cant. He was educated at Elgin Academy. He studied at the University of Aberdeen, graduating MA in 1845 and MD in 1850. He did further postgraduate studies in Paris, Berlin and Vienna. [1]
From at least 1856 Mitchell was based at Larbert Hospital, Scotland's largest hospital specialising in mental health.[ citation needed ] He lived in Edinburgh, a short train journey away, at 7 Laverockbank Villas in the Trinity district. [2]
In 1857 Mitchell was appointed Deputy Commissioner of Lunacy with the newly established General Board of Lunacy for Scotland, acting as deputy to William A.F. Browne. His appointment closely followed the publication of a report by the Scottish Lunacy Commission that prompted a greater understanding of the care of the insane. [3] [4] The appointment was controversial at the time, as he was still relatively inexperienced. [5] His work included a study of individuals in private care, which was described in his publication The Insane in Private Dwellings. [6] He reported that in 1862 there were 3628 insane persons in private care in Scotland, out of a total of 8207 (44%). While he recorded many cases of neglect and abuse in this setting, he argued that private care, if properly supervised, had an important part to play in the management of the insane. This suggestion was not in accord with the thrust of the 1857 report, which emphasised the role of district asylums. [7] Susser (2010) gives a modern evaluation of this work, describing the methods used as going "well beyond the practices of his own time, and, in some respects, were better than the methods used in such investigations today. [8]
The Insane in Private Dwellings included a great deal of statistical data, and this was to be a feature of much of his work. In 1865-1866 he published a series of papers examining the effects of consanguinous marriages on the offspring. [9] This area was highly contentious, but much of the comment was based on anecdotal evidence or assertions without supporting evidence. [10] While Mitchell's study was not conclusive, he identified the weaknesses of the existing work, and described in clear terms the two types of study that could resolve the issue: the first taking a large, representative, sample of defective births and comparing the parentage with that in the general population; the second, to collect family histories of everyone in a particular locality and follow up those with consaginuous marriages compared to unrelated marriages – in modern terms a cohort study. While he never carried out such studies, his evaluation of the evidence was the best that was available at the time, and his proposals for study design sound. [11]
In 1870 Mitchell became Commissioner for Lunacy, a post he held until 1895. [12] He continued with his research, and in 1877 published Contribution to the statistics of insanity which described a cohort study which followed all first asylum admissions in Scotland in the year 1858 for up to 12 years, to 1870. The total number of patients in the cohort was 1297. The study documented both those who remained within asylums, and those who left. In the latter case re-admissions, recoveries and deaths were recorded as far as possible. This was one of the first prospective studies carried out in psychiatry. [10]
In 1866 he was elected a Fellow of the Royal Society of Edinburgh his proposer being John Hutton Balfour. He served as the Society's Vice President 1889 to 1894 and 1896 to 1902. [1] [10] and in 1880 he was appointed a member of the Commission on Criminal Lunacy in England. In 1885 he became a member of a committee on criminal lunatics in Ireland, and from 1889 chaired a commission to enquire into the lunacy administration of Ireland. [12] In 1908 he was President of the Royal Meteorological Society.
Mitchell's work with the Board of Lunacy required him to travel extensively in Scotland, and he took advantage of this to carry out research in archaeology and anthropology. He reported on several underground buildings, [13] and on a large collection of "rude stone implements" from Shetland. [14] The stone implements were found in a variety of locations: on the surface; in subterranean structures; in the heart of a large tumulus; outside a stone coffin; or inside a kistvaen. Mitchell pointed out that the relatively crude workmanship could indicate an early origin, but could equally represent more recent work as an old skill declined. He used a similar argument in describing some of his finds in the communities of the Scottish islands, for example spindle whorls, and craggans, unglazed and usually undecorated clay pots. Mitchell not only saw these in use, but in several cases saw them being made. He commented that if they were found in an archeological excavation they might well be thought to be from the Stone Age. He also recorded the customs and practices of the places he visited, in particular regional superstitions. [15] In 1876 he became the first Rhind lecturer, and described many of his findings in a series of six lectures, which were published as The Past in the Present: What is Civilisation?. [16] His conclusion was that the difference in the essential characteristics of modern and early peoples are imperceptible, that civilisation was due to accumulated knowledge rather than an inherent superiority of its individual members.
Mitchell's paper On various superstitions in the north-west highlands and islands of Scotland, especially in relation to lunacy [15] encompassed both of his interests in the management of the insane and folk-culture. As Donoho (2012) notes "It is the earliest text specifically devoted to Highland folk-cures for insanity, and it espoused Mitchell's strong views on the matter, which is that he did not find these folk cures remotely palatable or justifiable". [17] Mitchell refers to "disgusting superstitions", for example the burial of a live cock as a cure for epilepsy. But while he regarded such customs as backward and ignorant, he sympathised with the distress of those who sought such cures. Discussing the adoration of wells, where supplicants would bathe a sick child in the waters, drop an offering in the well, and attach a bit of clothing to a nearby bush, he wrote: "Part of a bib or little pinafore tells the sad story of a sorrowing mother and a suffering child, and makes the heart grieve that nothing better than a visit to one of these wells had been found to relieve the sorrow and remove the suffering". [18]
He held positions with the Society of Antiquaries of Scotland, the Scottish Meteorological Society, the Early Scottish Text Society, and council member of the Scottish History Society. He was a professor of ancient history to the Royal Scottish Academy. Other works included A List of Travels in Scotland 1296—1900. [4]
In 1871 he was elected a member of the Harveian Society of Edinburgh. [19] [20] He received an honorary LLD from the University of Aberdeen in 1875. In 1886 Mitchell was made a Companion of the Bath (CB) by Queen Victoria, and was raised to the rank of Knight Commander of the Order of the Bath (KCB) the following year. He became an Honorary Fellow of the Royal College of Physicians of Ireland in 1891.
Unconnected to his medical life he was Chairman of the Scottish Life Assurance Company in Edinburgh. [21] His managerial colleagues included David Paulin as Manager and James Sorley as Treasurer (who together had founded the company).[ citation needed ]
He retired in 1895 and died at home 34 Drummond Place [22] in Edinburgh's New Town, on 12 October 1909. He was buried at Rosebank Cemetery, Edinburgh. [4] The grave lies on the north-facing retaining wall in the centre of the cemetery.
In 1855 he married Margaret Hay Houston (d.1904), daughter of James Houston of Tullochgriban in Strathspey.
For most of his adult life he lived in Edinburgh, first at 6 Laverockbank Villas in the Trinity district of Edinburgh, then at 34 Drummond Place in Edinburgh's New Town. [23] [24]
Their only child was Sydney Mitchell (1856–1930), [25] a successful Edinburgh architect who was commissioned by Arthur to design Craig House as an asylum in 1887.

Lunatic is a term referring to a person who is seen as mentally ill, dangerous, foolish, or crazy—conditions once attributed to "lunacy". The word derives from lunaticus meaning "of the moon" or "moonstruck".
Psychiatric hospitals, also known as mental health hospitals or behavioral health hospitals, are hospitals or wards specializing in the treatment of severe mental disorders, including schizophrenia, bipolar disorder, eating disorders, dissociative identity disorder, major depressive disorder, and others.
Moral treatment was an approach to mental disorder based on humane psychosocial care or moral discipline that emerged in the 18th century and came to the fore for much of the 19th century, deriving partly from psychiatry or psychology and partly from religious or moral concerns. The movement is particularly associated with reform and development of the asylum system in Western Europe at that time. It fell into decline as a distinct method by the 20th century, however, due to overcrowding and misuse of asylums and the predominance of biomedical methods. The movement is widely seen as influencing certain areas of psychiatric practice up to the present day. The approach has been praised for freeing sufferers from shackles and barbaric physical treatments, instead considering such things as emotions and social interactions, but has also been criticised for blaming or oppressing individuals according to the standards of a particular social class or religion.
Sir James Crichton-Browne MD FRS FRSE was a leading Scottish psychiatrist, neurologist and eugenicist. He is known for studies on the relationship of mental illness to brain injury and for the development of public health policies in relation to mental health. Crichton-Browne's father was the asylum reformer Dr William A.F. Browne, a prominent member of the Edinburgh Phrenological Society and, from 1838 until 1857, the superintendent of the Crichton Royal at Dumfries where Crichton-Browne spent much of his childhood.
The Association of Medical Superintendents of American Institutions for the Insane, also known as The Superintendents' Association, was organized in Philadelphia in October, 1844 at a meeting of 13 superintendents, making it the first professional medical specialty organization in the U.S.
Robert Gardiner Hill MD was a British surgeon specialising in the treatment of lunacy. He is normally credited with being the first superintendent of a small asylum to develop a mode of treatment in which reliance on mechanical medical restraint and coercion could be dropped altogether. In practice he reached this situation in 1838.
John Thomas Perceval was a British army officer who was confined in lunatic asylums for three years and spent the rest of his life campaigning for reform of the lunacy laws and for better treatment of asylum inmates. He was one of the founders of the Alleged Lunatics' Friend Society and acted as their honorary secretary for about twenty years. Perceval's two books about his experience in asylums were republished by anthropologist Gregory Bateson in 1962, and in recent years he has been hailed as a pioneer of the mental health advocacy movement.
The Alleged Lunatics' Friend Society was an advocacy group started by former asylum patients and their supporters in 19th-century Britain. The Society campaigned for greater protection against wrongful confinement or cruel and improper treatment, and for reform of the lunacy laws. The Society is recognised today as a pioneer of the psychiatric survivors movement.
Dr William Alexander Francis Browne (1805–1885) was one of the most significant British asylum doctors of the nineteenth century. At Montrose Asylum (1834–1838) in Angus and at the Crichton Royal in Dumfries (1838–1857), Browne introduced activities for patients including writing, group activity and drama, pioneered early forms of occupational therapy and art therapy, and initiated one of the earliest collections of artistic work by patients in a psychiatric hospital. In an age which rewarded self-control, Browne encouraged self-expression and may therefore be counted alongside William Tuke, Vincenzo Chiarugi and John Conolly as one of the pioneers of the moral treatment of mental illness. Sociologist Andrew Scull has identified Browne's career with the institutional climax of nineteenth century psychiatry.

Sir John Batty Tuke PRCPE FRSE LLD was one of the most influential psychiatrists in Scotland in the late nineteenth century, and a Unionist Member of Parliament (MP) from 1900 to 1910. Tuke's career in Edinburgh from 1863 to 1910 spanned a period of significant social and political changes in asylum governance and care in Scotland. Tuke's professional success in public and private practice and his powerful role in several prominent medical societies allowed him to influence his colleagues toward a more physiological understanding of mental illness and its treatment.
The lunatic asylum, insane asylum or mental asylum was an early precursor of the modern psychiatric hospital.
Lyttelton Stewart Forbes Winslow MRCP was a British psychiatrist famous for his involvement in the Jack the Ripper and Georgina Weldon cases during the late Victorian era.

Robert Boyd (1808–1883), was a physician and writer on mental illness, who, in 1870 became president of the Medico-Psychological Association.
Sir James Coxe MD FRSE was a Scottish physician and expert on psychiatry. Controversially he linked mental illness with a distancing from religion and with a parallel deterioration of the body. Rather more productively, he was an early campaigner against restraint in asylums, and he advocated greater training of women in the field of medicine.
Audrey Shore Henshall was a British archaeologist known for her work on Scottish chambered cairns, prehistoric pottery and early textiles.
Thomas Duncan Greenlees FRSE was a Scottish psychiatrist who had a strong association with South Africa. He headed the Medical Association of South Africa for many years and was Inspector of Asylums for the Cape of Good Hope. He did much to promote the de-stigmatisation of insanity.
The Lunacy (Scotland) Act 1857 formed mental health law in Scotland from 1857 until 1913.
The Commissioners in Lunacy for Scotland or Lunacy Commission for Scotland were a public body established by the Lunacy (Scotland) Act 1857 to oversee asylums and the welfare of mentally ill people in Scotland.
