CALSTAR (California Shock Trauma Air Rescue) is a regional air medical services company serving California and northern Nevada. It operates as a nonprofit air ambulance provider on the West Coast. [1]
CALSTAR was founded as a nonprofit public-benefit corporation in 1983 and began flight operations the following year using a leased BK 117 helicopter based at Peninsula Hospital in San Mateo, California. [2] In its first year, they flew 235 patients.
By 2010, CALSTAR’s operations had increased to gain ten helicopters. The EMS bases located throughout Northern and Central California, and a fixed wing program providing inter-facility transport services from the company’s headquarters at McClellan Park (formerly referred to as McClellan Air Force Base) in Sacramento, California.
Since its inception, CALSTAR has provided air medical transport services to over 50,000 critically ill and injured patients and logged over 75,000 accident-free flight hours.
CALSTAR was acquired by for-profit competitor REACH, a subsidiary of Air Medical Group Holdings, in 2016. [3] Air Medical Group Holdings merged with American Medical Response to form Global Medical Response in 2018.
All CALSTAR pilots have a minimum of 3,000 hours of pilot-in-command flight time. CALSTAR operates under its own Federal Aviation Administration (FAA) Part 135 Air Carrier Certificate and Part 145 Repair Station and is directly responsible for all aspects of its flight operations and aircraft maintenance.
CALSTAR operates a fleet of MBB Bo 105LS and MD 902 Explorer helicopters. In 2012, eight Eurocopter EC 135 helicopters were ordered and as of January 2014, six had been delivered. All aircraft are modified with medical interiors, high skid gear, high-intensity search lights and communication systems.
CALSTAR also utilizes King Air B200 light twin-engine airplanes for long-range inter-facility transports. [4]
Each CALSTAR flight crew is staffed with two registered nurses. CALSTAR flight nurses must achieve and maintain Certified Flight Registered Nurse (CFRN), Critical Care Registered Nurse (CCRN), Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications. [5]
CALSTAR flight nurses deliver care under the medical control provided by CALSTAR's medical director, in accordance with the California Nurse's Practice Act of 1974 and specific company procedures and protocols. All medical care provided by CALSTAR is held under scrutiny from the organization's quality assurance/quality improvement program, which is protected under Section 1157.5 of the California Evidence Code.
Since 2001, CALSTAR has been fully accredited by the Commission on Accreditation of Medical Transport Systems (CAMTS), which acknowledges excellence in patient care, safety, and education. [6] Accreditation must be maintained through rigorous evaluation every three years.
In 2009, CALSTAR completed a six-year project to retrofit its fleet of Eurocopter BO-105 LS aircraft with upgraded Rolls-Royce 250-C30P model engines, an enhancement designed to improve safety while significantly reducing emissions and operating costs. The FAA granted CALSTAR a Supplemental Type Certificate (STC) approving the engine retrofit for the entire BO-105 LS fleet nationwide. [7]
CALSTAR worked closely on the project with Rolls-Royce, as well as Steve Fossett. Fossett, a businessman and adventurer, had selected the BO-105 LS for an endurance record he was hoping to set. He was so interested in the C30P engine upgrade that he lent CALSTAR his personal aircraft to conduct all of the required flight testing. [7] The retrofitted BO-105 LS was later nicknamed the "Fossett Special."
CALSTAR also received FAA approval in 2009 for the first three helicopters Wide Area Augmentation System (WAAS) GPS approach procedures in the world. The WAAS procedures were developed by Hickok & Associates, who have since implemented additional helicopter WAAS approaches for air medical providers throughout the United States, Europe and Asia. The Airbus Helicopters H135 is a twin-engine civil light utility helicopter produced by Airbus Helicopters, formerly Eurocopter. It is capable of flight under instrument flight rules (IFR) and is outfitted with a digital automatic flight control system (AFCS). First flying in February 1994, it entered service in 1996. 1,400 have been delivered up to September 2020, to 300 operators in 60 countries, accumulating over 5 million flight hours. It is mainly used for air medical transport (medevac), corporate transport, law enforcement, offshore wind support, and military flight training. Half of them are in Europe and a quarter in North America. The H135M, certified under the name Eurocopter EC635, is a military variant, so the overall design is known as the Airbus Helicopters H135 and the military version, as the Airbus Helicopters H135M. The EC135/H135 is a development of the earlier Messerschmitt-Bölkow-Blohm (MBB) Bo 105.
The Messerschmitt-Bölkow-Blohm Bo 105 is a light, twin-engine, multi-purpose helicopter developed by Bölkow of Ottobrunn, West Germany. It was the first light twin-engine helicopter in the world, and the first rotorcraft that could perform aerobatic maneuvers such as inverted loops. The Bo 105 features a hingeless rotor system, a pioneering innovation in helicopters when it was introduced into service in 1970. Production of the Bo 105 began at the then-recently merged Messerschmitt-Bölkow-Blohm.
ARCH Air Medical Service is an emergency medical service (EMS) that provides critical care air ambulance service in Missouri, Illinois, and the surrounding regions. Air ambulance programs offer transport by helicopter (rotor-wing) or fixed-wing aircraft. ARCH Air was the twelfth program in the U.S. to offer such services when it began operating in March 1979. Transporting approximately 4,200 patients per year by helicopter, ARCH aircraft are staffed by a pilot, nurse and paramedic. Flights are 80% inter-facility and 20% scene.
Memorial Hermann Life Flight is a hospital-based air medical transport service in Houston, Texas. Prior to 2020, Life Flight flew into 12th floor of the John S. Dunn Heli-Stop atop Memorial Hermann Hospital-Texas Medical Center. In March of 2020, Memorial Hermann-TMC opened the new 18 story Sarofim Pavilion, which serves as the home for Life Flight operations in addition to an expanded helipad atop the pavilion. Life Flight was founded in 1976.
The Shock Trauma Air Rescue Service is a Canadian non-profit helicopter air ambulance organization funded by individual donors, service groups, corporate donors and government contributions. STARS provides rapid and specialized emergency care and transportation for critically ill and injured patients. STARS operates from bases in Calgary, Edmonton, Grande Prairie, Regina, Saskatoon, Winnipeg and formerly Halifax.

The Airbus Helicopters H145 is a twin-engine light utility helicopter developed and manufactured by Airbus Helicopters. Originally designated as the BK 117, the H145 is based upon the MBB/Kawasaki BK 117 C1, which became a part of the combined Eurocopter line-up in 1992 with the merger of Messerschmitt-Bölkow-Blohm's helicopter division of Daimler-Benz into Eurocopter. The helicopter was initially named EC145; an updated version, EC145 T2, was renamed H145 in 2015. The helicopter was significantly updated in the 2020s with first a fenestron replacing the traditional tail rotor, followed later by a 5-blade main rotor head.
Flight for Life is a prehospital care service with many bases of operation across the United States. Flight for Life is primarily known for its emergency medical helicopter transport, but also operates a fleet of land vehicles and fixed-wing aircraft for the transport of critically ill patients to specialized medical care. Helicopter transport is normally reserved for the most critically injured patients or patients who cannot be reached by traditional means of Emergency medical services. Helicopter transport is also especially useful for the transport of critically wounded patients to specialty medical facilities, such as burn, pediatric, or advanced trauma centers, that may be further away from the location of injury. Many rural communities rely on the speed of the helicopter to reach and evacuate their most serious patients to an available medical center. STAT MedEvac "originally STAT Angel One" is a service of the Center for Emergency Medicine of Western Pennsylvania in Pittsburgh, Pennsylvania. The primary function of STAT MedEvac is to provide emergency medevac and air medical transport of critically wounded or sick civilians, either from emergency scenes or between hospitals. As of July of 2017, the organization also operates two ground ambulances for patients with severe injury or critical illness. STAT MedEvac is directed by a consortium of hospitals of the University of Pittsburgh Medical Center (UPMC) that include UPMC Children's Hospital of Pittsburgh, UPMC Altoona, UPMC Hamot, UPMC Mercy and UPMC Presbyterian Shadyside. STAT MedEvac's base of operations is in West Mifflin, Pennsylvania, at the Allegheny County Airport. With 18 air bases across Pennsylvania, Maryland, New York, Ohio, and the District of Columbia, it is one of the largest single operated and dispatched air-medical service organizations in the United States.
Eagle III is an emergency air and ground medical transportation program, in partnership with Bellin Hospital, St. Vincent Hospital and County Rescue Services, all in Green Bay, Wisconsin.
