Condensing osteitis, also known as focal sclerosing osteomyelitis, is a rare periapical inflammatory condition characterized by the formation of sclerotic bone near the roots of premolars and molars. This condition arises as a response to dental infections, such as periapical pulp inflammation or low-intensity trauma. The lesion typically appears as a radiopacity in the periapical area due to the sclerotic reaction. While most commonly associated with non-vital teeth, condensing osteitis can also occur in vital teeth following occlusal trauma. The condition was first described by Dr. Carl Garré in 1893. [2] [3]
Patients are typically asymptomatic, and the condition is most often detected incidentally during routine radiographic examinations. [3] In rare symptomatic cases, patients may report mild pain. [4]
The cause of condensing osteitis is not clear, but it is thought to happen due to irritation or inflammation that increases osteoblastic activity. [4]
Chronic pulpitis is one of the main factors. This is when bacteria enters the pulp of the tooth, often from untreated cavities. This leads to ongoing mild inflammation that causes bone growth near the root of the tooth. [5] Another main factor is periapical infections. These infections start at the tip of the tooth root, often from dead pulp tissue or occlusal trauma. [6]
Condensing osteitis happens when the bone around the tooth reacts to long-term inflammation. This involves excessive bone growth, leading to the formation of sclerotic bone in the jaw.
Ongoing tooth infections, like pulpitis, release chemicals that attract immune cells and activate osteoblasts. Osteoblasts create extra bone in response to inflammation, making the area look more sclerotic. Unlike typical infections, condensing osteitis doesn't destroy bone or produce pus it just adds more bone to the affected area. [4] [6]
Diagnosis typically involves a clinical examination by a dentist or endodontist, complemented by imaging studies such as cone-beam computed tomography. Radiographically, condensing osteitis presents as a localized radiopaque lesion at the root apex of the affected tooth. [7]
Condensing osteitis is usually benign and asymptomatic. Treatment depends on the underlying cause and severity. Endodontic treatment is the primary intervention in cases of pulpal infection. Other treatment options include Antibiotics that are prescribed for associated bacterial infections and tooth Extraction: Reserved for cases where the tooth is irreversibly damaged due to pulpitis. The prognosis is excellent. Following root canal therapy, the sclerotic bone often remodels, restoring normal bone structure over time. [9] [10]
This affects 4-7% of the human population. It mainly occurs during early adolescence/young adults but can occur at any age and in any gender. [11]
Recent research has focused on the prevalence and management of condensing osteitis and thus has pointed out the need for further investigation into its pathophysiology and treatment. For instance, studies on the frequency and distribution of mandibular condensing osteitis in specific populations, such as the Taiwanese population, have provided useful epidemiological data. These studies indicate that prevalence may be related to cultural, dietary, or even genetic factors, thus constituting a background for conducting research in different population groups. [11]
Case reports, such as the documentation of a female patient aged 35 years with condensing osteitis, further support that there is variability in clinical presentation and outcome. These cases serve to further the understanding not only of the condition itself but also the deficits in both diagnostic and therapeutic approaches, underlining the need for individual treatment strategies, as well as follow-up over a long period of time regarding changes in bone after intervention. Future studies may be directed at the molecular mechanisms of osteoblastic activity and bone sclerotization in response to chronic inflammation. Identifying specific inflammatory or genetic markers may lead to earlier diagnosis and targeted therapies. Clinical practices may also be greatly improved by investigating the place of advanced imaging technologies, such as cone-beam computed tomography, in enhancing diagnostic accuracy and monitoring treatment outcomes.
Condensing osteitis will be more clearly understood when epidemiological data is combined with clinical case studies and molecular research to enhance patient care and outcomes by both dental and medical specialties.

Cemento-osseous dysplasia (COD) is a benign condition of the jaws that may arise from the fibroblasts of the periodontal ligaments. It is most common in middle-aged females of African descent. The three types are periapical cemental dysplasia, focal cemento-osseous dysplasia (Caucasians), and florid cemento-osseous dysplasia. Periapical COD occurs most commonly in the mandibular anterior teeth while focal COD appears predominantly in the mandibular posterior teeth. Florid COD is an extensive variant of periapical COD where lesions occur in multiple quadrants which can encompass the maxilla and mandible, and infrequently can cause jawbone deformity.

A dental abscess is a localized collection of pus associated with a tooth. The most common type of dental abscess is a periapical abscess, and the second most common is a periodontal abscess. In a periapical abscess, usually the origin is a bacterial infection that has accumulated in the soft, often dead, pulp of the tooth. This can be caused by tooth decay, broken teeth or extensive periodontal disease. A failed root canal treatment may also create a similar abscess.

Cementoma is an odontogenic tumor of cementum. It is usually observed as a benign spherical mass of hard tissue fused to the root of a tooth. It is found most commonly in the mandible in the region of the lower molar teeth, occurring between the ages of 8 and 30 in both sexes with equal frequency. It causes distortion of surrounding areas but is usually a painless growth, at least initially. Considerable thickening of the cementum can often be observed. A periapical form is also recognized. Cementoma is not exclusive to the mandible as it can infrequently occur in the maxilla and other parts of the body such as the long bones.
Pulpotomy is a minimally invasive procedure performed in children on a primary tooth with extensive caries but without evidence of root pathology. The minimally invasive, endodontic techniques of vital pulp therapy (VPT) are based on improved understanding of the capacity of pulp tissues to heal and regenerate plus the availability of advanced endodontic materials. During caries removal, this results in a carious or mechanical pulp exposure from the cavity. During pulpotomy, the inflamed/diseased pulp tissue is removed from the coronal pulp chamber of the tooth, leaving healthy pulp tissue which is dressed with a long-term clinically successful medicament that maintains the survival of the pulp and promotes repair. There are various types of medicament placed above the vital pulp such as Buckley's Solution of formocresol, ferric sulfate, calcium hydroxide or mineral trioxide aggregate (MTA). MTA is a more recent material used for pulpotomies with a high rate of success, better than formocresol or ferric sulfate. It is also recommended to be the preferred pulpotomy agent in the future. After the coronal pulp chamber is filled, the tooth is restored with a filling material that seals the tooth from microleakage, such as a stainless steel crown which is the most effective long-term restoration. However, if there is sufficient remaining supporting tooth structure, other filling materials such as amalgam or composite resin can provide a functional alternative when the primary tooth has a life span of two years or less. The medium- to long-term treatment outcomes of pulpotomy in symptomatic permanent teeth with caries, especially in young people, indicate that pulpotomy can be a potential alternative to root canal therapy (RCT).
Hypercementosis is an idiopathic, non-neoplastic condition characterized by the excessive buildup of normal cementum on the roots of one or more teeth. A thicker layer of cementum can give the tooth an enlarged appearance, mainly occurring at the apex or apices of the tooth. The cellular cementum functions at the bottom half of the tooth roots which contain cementocytes that anchor the tooth into the jaw socket, protect the tooth's pulp, and repair external root resorption.
Pulp necrosis is a clinical diagnostic category indicating the death of cells and tissues in the pulp chamber of a tooth with or without bacterial invasion. It is often the result of many cases of dental trauma, caries and irreversible pulpitis.
Cysts of the jaws are cysts—pathological epithelial-lined cavities filled with fluid or soft material—occurring on the bones of the jaws, the mandible and maxilla. Those are the bones with the highest prevalence of cysts in the human body, due to the abundant amount of epithelial remnants that can be left in the bones of the jaws. The enamel of teeth is formed from ectoderm, and so remnants of epithelium can be left in the bone during odontogenesis. The bones of the jaws develop from embryologic processes which fuse, and ectodermal tissue may be trapped along the lines of this fusion. This "resting" epithelium is usually dormant or undergoes atrophy, but, when stimulated, may form a cyst. The reasons why resting epithelium may proliferate and undergo cystic transformation are generally unknown, but inflammation is thought to be a major factor. The high prevalence of tooth impactions and dental infections that occur in the bones of the jaws is also significant to explain why cysts are more common at these sites.
Periapical periodontitis or apical periodontitis (AP) is an acute or chronic inflammatory lesion around the apex of a tooth root, most commonly caused by bacterial invasion of the pulp of the tooth. It is a likely outcome of untreated dental caries, and in such cases it can be considered a sequela in the natural history of tooth decay, irreversible pulpitis and pulpal necrosis. Other causes can include occlusal trauma due to 'high spots' after restoration work, extrusion from the tooth of root filling material, or bacterial invasion and infection from the gums. Periapical periodontitis may develop into a periapical abscess, where a collection of pus forms at the end of the root, the consequence of spread of infection from the tooth pulp, or into a periapical cyst, where an epithelial lined, fluid-filled structure forms.
Periapical granuloma, also sometimes referred to as a radicular granuloma or apical granuloma, is an inflammation at the tip of a dead (nonvital) tooth. It is a lesion or mass that typically starts out as an epithelial lined cyst, and undergoes an inward curvature that results in inflammation of granulation tissue at the root tips of a dead tooth. This is usually due to dental caries or a bacterial infection of the dental pulp. Periapical granuloma is an infrequent disorder that has an occurrence rate between 9.3 to 87.1 percent. Periapical granuloma is not a true granuloma due to the fact that it does not contain granulomatous inflammation; however, periapical granuloma is a common term used.
