The core conflictual relationship theme, or CCRT, is an analytic tool developed in the late twentieth century by Lester Luborsky, as an aid both to brief psychotherapy in the psychodynamic tradition, and to researching its efficacy.

Origins

Drawing on the Freudian concept of transference, as refined by Jacob Arlow to cover a specific pattern of relating to people established in early life, as well as upon Michael Balint's focal therapy, [1] with its concern to delimit therapy to the exploration of a key theme, [2] CCRT set out to focus therapy on three aspects of a client's central relationship conflict – their core desire, the response to it they typically elicited from other people (RO), and their reaction in turn to that response (RS). [3]

Methods

CCRT is generally employed within the context of a time-limited therapy (perhaps involving 16 or 24 sessions). It focuses on examining core patterns of relating, initially using relationship anecdotes to establish them; [4] and typically involves an exploration of early familial transactions, as manifested through psychological projection and projective identification in outside life, as well as in the transference. [5] Fantasies also are closely linked with the core theme. [6]

Research reveals that accurately interpreting the core patterns of relationship conflict is associated with positive outcomes both within sessions and over the treatment as a whole. [7]

Psychoanalysis is a set of theories and therapeutic techniques that deal in part with the unconscious mind, and which together form a method of treatment for mental disorders. The discipline was established in the early 1890s by Austrian neurologist Sigmund Freud, who developed the practice from his theoretical model of personality organization and development, psychoanalytic theory. Freud's work stems partly from the clinical work of Josef Breuer and others. Psychoanalysis was later developed in different directions, mostly by students of Freud, such as Alfred Adler and his collaborator, Carl Gustav Jung, as well as by neo-Freudian thinkers, such as Erich Fromm, Karen Horney, and Harry Stack Sullivan.

Psychotherapy is the use of psychological methods, particularly when based on regular personal interaction, to help a person change behavior, increase happiness, and overcome problems. Psychotherapy aims to improve an individual's well-being and mental health, to resolve or mitigate troublesome behaviors, beliefs, compulsions, thoughts, or emotions, and to improve relationships and social skills. There are numerous types of psychotherapy designed either for individual adults, families, or children and adolescents. Certain types of psychotherapy are considered evidence-based for treating some diagnosed mental disorders, and other types have been criticized as pseudoscience.

Object relations theory

Object relations theory in psychoanalytic psychology is the process of developing a psyche in relation to others in the childhood environment. It designates theories or aspects of theories that are concerned with the exploration of relationships between real and external people as well as internal images and the relations found in them. It maintains that the infant's relationship with the mother primarily determines the formation of its personality in adult life. Particularly, the need for attachment is the bedrock of the development of the self or the psychic organization that creates the sense of identity.

Countertransference relational aspect of psychoanalysis

Countertransference is defined as redirection of a psychotherapist's feelings toward a client – or, more generally, as a therapist's emotional entanglement with a client.

Transference phenomenon within psychotherapy

Transference is a phenomenon within psychotherapy in which the feelings a person had about their parents, as one example, are unconsciously redirected or transferred to the present situation. It usually concerns feelings from a primary relationship during childhood. At times, this transference can be considered inappropriate. Transference was first described by Sigmund Freud, the founder of psychoanalysis, who considered it an important part of psychoanalytic treatment.

Psychodynamic psychotherapy

Psychodynamic psychotherapy or psychoanalytic psychotherapy is a form of depth psychology, the primary focus of which is to reveal the unconscious content of a client's psyche in an effort to alleviate psychic tension. It evolved from and largely replaced psychoanalysis in the mid-20th century.

Michael Balint

Michael Balint was a Hungarian psychoanalyst who spent most of his adult life in England. He was a proponent of the Object Relations school.

David Huntingford Malan is a British psychoanalytic psychotherapy practitioner and researcher recognized for his contribution to the development of psychotherapy. He promoted scientific spirit of inquiry, openness, and simplicity within the field. He is also noted for his development of the Malan triangles, which became a rubric in which therapists can reflect upon what they are doing and where they are in relational space at any given moment.

The therapeutic relationship refers to the relationship between a healthcare professional and a client or patient. It is the means by which a therapist and a client hope to engage with each other and effect beneficial change in the client.

Child psychotherapy, or mental health interventions for children have developed varied approaches over the last century. Two distinct historic pathways can be identified for present-day provision in Western Europe and in the United States: one through the Child Guidance Movement, the other stemming from Adult psychiatry or Psychological Medicine, which evolved a separate Child psychiatry specialism.

Interpersonal psychotherapy (IPT) is a brief, attachment-focused psychotherapy that centers on resolving interpersonal problems and symptomatic recovery. It is an empirically supported treatment (EST) that follows a highly structured and time-limited approach and is intended to be completed within 12–16 weeks. IPT is based on the principle that relationships and life events impact mood and that the reverse is also true. It was developed by Gerald Klerman and Myrna Weissman for major depression in the 1970s and has since been adapted for other mental disorders. IPT is an empirically validated intervention for depressive disorders, and is more effective when used in combination with psychiatric medications. Along with cognitive behavioral therapy (CBT), IPT is recommended in treatment guidelines as a psychosocial treatment of choice for depression.

Transference focused psychotherapy (TFP) is a highly structured, twice-weekly modified psychodynamic treatment based on Otto F. Kernberg's object relations model of borderline personality disorder. It views the individual with borderline personality organization (BPO) as holding unreconciled and contradictory internalized representations of self and significant others that are affectively charged. The defense against these contradictory internalized object relations leads to disturbed relationships with others and with self. The distorted perceptions of self, others, and associated affects are the focus of treatment as they emerge in the relationship with the therapist (transference). The treatment focuses on the integration of split off parts of self and object representations, and the consistent interpretation of these distorted perceptions is considered the mechanism of change.

Transference neurosis Freudian concept

Transference neurosis is a term that Sigmund Freud introduced in 1914 to describe a new form of the analysand's infantile neurosis that develops during the psychoanalytic process. Based on Dora's case history, Freud suggested that during therapy the creation of new symptoms stops, but new versions of the patient's fantasies and impulses are generated. He called these newer versions "transferences" and characterized them as the substitution of the analyst for a person from the patient's past. According to Freud's description: "a whole series of psychological experiences are revived not as belonging to the past, but as applying to the person of the analyst at the present moment". When transference neurosis develops, the relationship with the therapist becomes the most important one for the patient, who directs strong infantile feelings and conflicts towards the therapist, e.g. the patient may react as if the analyst is his/her father.

Supportive psychotherapy is a psychotherapeutic approach that integrates various therapeutic schools such as psychodynamic and cognitive-behavioral, as well as interpersonal conceptual models and techniques.

The mainstay of management of borderline personality disorder is various forms of psychotherapy with medications being found to be of little use.

Schema therapy was developed by Jeffrey E. Young for use in treatment of personality disorders and chronic DSM Axis I disorders, such as when patients fail to respond or relapse after having been through other therapies. Schema therapy is an integrative psychotherapy combining theory and techniques from previously existing therapies, including cognitive behavioral therapy, psychoanalytic object relations theory, attachment theory, and Gestalt therapy.

Parallel process is a phenomenon noted between therapist and supervisor, whereby the therapist recreates, or parallels, the client's problems by way of relating to the supervisor. The client's transference and the therapist's countertransference thus re-appear in the mirror of the therapist/supervisor relationship.

Malan's triangles – comprising the triangle of conflict and the triangle of persons – were developed in 1979 by the psychotherapist David Malan as a way of illuminating the phenomenon of transference in psychotherapy, both brief and extended.

Golden fantasy is a secret expectation that all of one's problems can be solved by interaction with a perfect and all-caring relationship figure. The fantasy can be found both in psychotherapy and in ordinary life.
