Dawn simulation is a technique that involves timing a light, often called a wake-up light, sunrise alarm clock, or natural light alarm clock, in the bedroom to come on gradually, over a period of 30 minutes to 2 hours, before awakening to simulate dawn.

History

The concept of dawn simulation was first patented in 1890 [1] as "mechanical sunrise". Modern electronic units were patented in 1973. [2] Variations and improvements seem to get patented every few years. Clinical trials were conducted by David Avery, MD, [3] in the 1980s at Columbia University following a long line of basic laboratory research that showed animals' circadian rhythms to be exquisitely sensitive to the dim, gradually rising dawn signal at the end of the night.

Clinical use

There are two types of dawn that have been used effectively in a clinical setting: a naturalistic dawn mimicking a springtime sunrise (but used in mid-winter when it is still dark outside), and a sigmoidal-shaped dawn (30 minutes to 2 hours). When used successfully, patients are able to sleep through the dawn and wake up easily at the simulated sunrise, after which the day's treatment is over. The theory behind dawn simulation is based on the fact that early morning light signals are much more effective at advancing the biological clock than are light signals given at other times of day (see Phase response curve).

Comparison with bright light therapy

Dawn simulation generally uses light sources that range in illuminance from 100 to 300 lux, while bright light boxes are usually in the 10,000-lux range. Approximately 19% [4] of patients discontinue post-awakening bright light therapy due to inconvenience. Because the entire treatment is complete before awakening, dawn simulation may be a more convenient alternative to post-awakening bright light therapy. In terms of efficacy, some studies have shown dawn simulation to be more effective than standard bright light therapy while others have shown no difference or shown that bright light therapy is superior. [5] [6] Some patients with seasonal affective disorder use both dawn simulation and bright light therapy to provide maximum effect at the start of the day. [ citation needed ]

Other uses

In an elaboration of the method, patients have also been presented with a dim dusk signal at bedtime, with indications that it eases sleep onset. In addition, the technique has been used clinically[ citation needed ] with patients who suffer from delayed sleep phase syndrome, helping them to awaken earlier in gradual steps, as the simulated dawn is moved earlier.

Non-clinical sleep and wake-up uses

A dawn simulator can be used as an alarm clock. Light enters through the eyelids triggering the body to begin its wake-up cycle, including the release of cortisol, so that by the time the light is at full brightness, sleepers wake up on their own, without the need for an alarm. Most commercial alarm clocks include a "dusk" mode as well for bedtime.

This sleep phase advancement can interfere with daily social and work schedules, and results in shortened sleep duration and excessive daytime sleepiness. The timing of sleep and melatonin levels are regulated by the body's central circadian clock, which is located in the suprachiasmatic nucleus in the hypothalamus.

Nightmare disorder is a sleep disorder characterized by repeated intense nightmares that most often center on threats to physical safety and security. The nightmares usually occur during the REM stage of sleep, and the person who experiences the nightmares typically remembers them well upon waking. More specifically, nightmare disorder is a type of parasomnia, a subset of sleep disorders categorized by abnormal movement or behavior or verbal actions during sleep or shortly before or after. Other parasomnias include sleepwalking, sleep terrors, bedwetting, and sleep paralysis.

Chronotherapy is a behavioural treatment that attempts to move bedtime and rising time later and later each day, around the clock, until a person is sleeping on a normal schedule. This treatment can be used by people with delayed sleep phase disorder (DSPD), who generally cannot reset their circadian rhythm by moving their bedtime and rising time earlier. DSPD is a circadian rhythm sleep disorder, characterised by a mismatch between a person's internal biological clock and societal norms. Chronotherapy uses the human phase response to light or melatonin. The American Academy of Sleep Medicine has recommended chronotherapy for the treatment of circadian rhythm and sleep disorders.

Light therapy, also called phototherapy or bright light therapy is the exposure to direct sunlight or artificial light at controlled wavelengths in order to treat a variety of medical disorders, including seasonal affective disorder (SAD), circadian rhythm sleep-wake disorders, cancers, and skin wound infections. Treating skin conditions such as neurodermatitis, psoriasis, acne vulgaris, and eczema with ultraviolet light is called ultraviolet light therapy.

An alarm clock or alarm is a clock that is designed to alert an individual or group of people at a specified time. The primary function of these clocks is to awaken people from their night's sleep or short naps; they can sometimes be used for other reminders as well. Most alarm clocks make sounds; some make light or vibration. Some have sensors to identify when a person is in a light stage of sleep, in order to avoid waking someone who is deeply asleep, which causes tiredness, even if the person has had adequate sleep. To turn off the sound or light, a button or handle on the clock is pressed; most clocks automatically turn off the alarm if left unattended long enough. A classic analog alarm clock has an extra hand or inset dial that is used to show the time at which the alarm will ring. Alarm clock functions are also used in mobile phones, watches, and computers.

<span class="mw-page-title-main">Sleep hygiene</span> Set of practices around healthy sleeping

Sleep hygiene is a behavioral and environmental practice developed in the late 1970s as a method to help people with mild to moderate insomnia. Clinicians assess the sleep hygiene of people with insomnia and other conditions, such as depression, and offer recommendations based on the assessment. Sleep hygiene recommendations include: establishing a regular sleep schedule; using naps with care; not exercising physically or mentally too close to bedtime; limiting worry; limiting exposure to light in the hours before sleep; getting out of bed if sleep does not come; not using bed for anything but sleep and sex; avoiding alcohol as well as nicotine, caffeine, and other stimulants in the hours before bedtime; and having a peaceful, comfortable and dark sleep environment. However, as of 2021, the empirical evidence for the effectiveness of sleep hygiene is "limited and inconclusive" for the general population and for the treatment of insomnia, despite being the oldest treatment for insomnia. A systematic review by the AASM concluded that clinicians should not prescribe sleep hygiene for insomnia due to the evidence of absence of its efficacy and potential delaying of adequate treatment, recommending instead that effective therapies such as CBT-i should be preferred.

Non-24-hour sleep–wake disorder is one of several chronic circadian rhythm sleep disorders (CRSDs). It is defined as a "chronic steady pattern comprising [...] daily delays in sleep onset and wake times in an individual living in a society". Symptoms result when the non-entrained (free-running) endogenous circadian rhythm drifts out of alignment with the light–dark cycle in nature. Although this sleep disorder is more common in blind people, affecting up to 70% of the totally blind, it can also affect sighted people. Non-24 may also be comorbid with bipolar disorder, depression, and traumatic brain injury. The American Academy of Sleep Medicine (AASM) has provided CRSD guidelines since 2007 with the latest update released in 2015.

A phase response curve (PRC) illustrates the transient change in the cycle period of an oscillation induced by a perturbation as a function of the phase at which it is received. PRCs are used in various fields; examples of biological oscillations are the heartbeat, circadian rhythms, and the regular, repetitive firing observed in some neurons in the absence of noise.

Negative air ionization therapy (NAIs) uses air ionisers as a non-pharmaceutical treatment for respiratory disease, allergy, or stress-related health conditions. The mainstream scientific community considers many applications of NAIs to be pseudoscience. Many negative ion products release ozone, a chemical known to cause lung damage.

Wake therapy is a specific application of intentional sleep deprivation. It encompasses many sleep-restricting paradigms that aim to address mood disorders with a form of non-pharmacological therapy.

Interpersonal and social rhythm therapy (IPSRT) is an intervention for people with bipolar disorder (BD). Its primary focus is stabilizing the circadian rhythm disruptions that are common among people with bipolar disorder (BD). IPSRT draws upon principles from interpersonal psychotherapy, an evidence-based treatment for depression and emphasizes the importance of daily routine (rhythm).

Circadian rhythm sleep disorders (CRSD), also known as circadian rhythm sleep-wake disorders (CRSWD), are a family of sleep disorders which affect the timing of sleep. CRSDs arise from a persistent pattern of sleep/wake disturbances that can be caused either by dysfunction in one's biological clock system, or by misalignment between one's endogenous oscillator and externally imposed cues. As a result of this mismatch, those affected by circadian rhythm sleep disorders have a tendency to fall asleep at unconventional time points in the day. These occurrences often lead to recurring instances of disturbed rest, where individuals affected by the disorder are unable to go to sleep and awaken at "normal" times for work, school, and other social obligations. Delayed sleep phase disorder, advanced sleep phase disorder, non-24-hour sleep–wake disorder and irregular sleep–wake rhythm disorder represents the four main types of CRSD.

<span class="mw-page-title-main">Sleep deprivation</span> Condition of not having enough sleep

Sleep deprivation, also known as sleep insufficiency or sleeplessness, is the condition of not having adequate duration and/or quality of sleep to support decent alertness, performance, and health. It can be either chronic or acute and may vary widely in severity. All known animals sleep or exhibit some form of sleep behavior, and the importance of sleep is self-evident for humans, as nearly a third of a person's life is spent sleeping.

Designing lighting for the elderly requires special consideration and care from architects and lighting designers. As people age, they experience neurodegeneration in the retina and in the suprachiasmatic nucleus (SCN). Less light reaches the back of the eyes because the pupils decrease in size as one ages, the lens inside one's eye becomes thicker, and the lens scatters more light, causing objects and colors to appear less vivid. These symptoms are particularly common with persons having alzheimer's disease. Older people also have reduced levels of retinal illuminance, such as having smaller pupils and less transparent crystalline lenses. Furthermore, as an individual ages, they begins to lose retinal neurons, which not only compromises the ability to see but also to register a robust daily pattern of light-dark that is needed to maintain biological rhythms. The 24-hour light-dark cycle is the most important external stimulus for regulating the timing of the circadian cycle.

Michael Terman is an American psychologist best known for his work in applying the biological principles of the circadian timing system to psychiatric treatments for depression and sleep disorders. This subspecialty is known as Chronotherapeutics.

Sleep is known to play an important role in the etiology and maintenance of bipolar disorder. Patients with bipolar disorder often have a less stable and more variable circadian activity. Circadian activity disruption can be apparent even if the person concerned is not currently ill.

In chronobiology, photoentrainment refers to the process by which an organism's biological clock, or circadian rhythm, synchronizes to daily cycles of light and dark in the environment. The mechanisms of photoentrainment differ from organism to organism. Photoentrainment plays a major role in maintaining proper timing of physiological processes and coordinating behavior within the natural environment. Studying organisms’ different photoentrainment mechanisms sheds light on how organisms may adapt to anthropogenic changes to the environment.
