Elinore F. McCance-Katz is an American physician,academic,and government official who served as Assistant Secretary of Health and Human Services for Mental Health and Substance Use from 2017 to 2021. Prior to assuming her current role,she was the chief medical officer for the Rhode Island Department of Behavioral Healthcare,Developmental Disabilities,and Hospitals and a professor at the Alpert Medical School at Brown University. [1] [2] Her nomination was supported by the American Psychiatric Association,which released a statement saying "Dr. McCance-Katz has a wealth of experience in academic and public sector settings in addressing mental health and substance use disorders." [3]
McCance-Katz resigned in the wake of the 2021 storming of the United States Capitol,on January 7,2021. [4]
After her 2021 resignation,McCance-Katz was hired into a senior civil service job by the Drug Enforcement Administration. [5]
McCance-Katz earned a B.A. in biology from Eastern Connecticut State University in 1978. She went on to attend Yale University,where she received an MPhil and Ph.D. She attended medical school at the University of Connecticut School of Medicine,where she earned her M.D. in 1987. She completed her residency in psychiatry at Hartford Hospital in Connecticut. [6] [7]
McCance-Katz was on the faculty of Yale University from 1991 to 1998 and later taught at the University of Texas,the Albert Einstein College of Medicine,Virginia Commonwealth University,and the University of California San Francisco. McCance-Katz was medical director for the California Department of Alcohol and Drug Programs before serving as the first chief medical officer for the Substance Abuse and Mental Health Services Administration,a position which she held from 2013 to 2015. She is a Distinguished Fellow of the American Academy of Addiction Psychiatry. [8] McCance-Katz and her husband hold a patent for a method used to prevent urine specimen substitution in substance use screening. [7] She has conducted substantial research into addiction,specifically opioid addiction. [6]
McCance-Katz's work in the Trump administration was marked by several controversial statements. For example,in 2018,McCance-Katz spoke out against fentanyl test strips for people who use drugs. As she wrote in an HHS-sponsored blog,“The entire approach is based on the premise that a drug user poised to use a drug is making rational choices,is weighing pros and cons,and is thinking completely logically about his or her drug use. Based on my clinical experience,I know this could not be further from the truth.” [9] At the same time,however,a study in the International Journal of Drug Policy came to the opposite conclusion. [10] As pointed out in a 2017 New York Times article,the treatment of drug addiction has long been marked by a split between “a medical model of psychiatry,which emphasizes drug and hospital treatment and which Dr. McCance-Katz has promoted,and the so-called psychosocial,which puts more emphasis on community care and support from family and peers.” [11]
In September 2020,during the height of the Covid epidemic and just prior to the 2020 presidential election,McCance-Katz spoke on a podcast posted on the official HHS website:“What is this nonsense that somehow it’s unsafe to return to school?... There was no agreement to this,to this nonstop restriction and quarantining and isolation and taking away anything that makes people happy.... You can’t go to a movie,you can’t go to a football game.... We shut down the entire country before the virus,in my opinion,had a chance to get around the entire country. …We used a sledgehammer when I think we needed a scalpel.” [12] As she asserted,“I just wish that the media would get honest about its coverage of Covid.... lost in all of this response to Covid and nonstop 24/7 horrors of Covid and if you can't find something to talk about,it appears to me they make things up. It just does.” [13]

This preventable pathology can be fatal if it leads to respiratory depression,a lethal condition that can cause hypoxia from slow and shallow breathing. Other symptoms include small pupils and unconsciousness;however,its onset can depend on the method of ingestion,the dosage and individual risk factors. Although there were over 110,000 deaths in 2017 due to opioids,individuals who survived also faced adverse complications,including permanent brain damage.

This may include disorders involving legal and illegal drugs,gambling,sex,food,and other impulse control disorders. Addiction psychiatrists are substance use disorder experts. Growing amounts of scientific knowledge,such as the health effects and treatments for substance use disorders,have led to advancements in the field of addiction psychiatry. These advancements in understanding the neurobiology of rewarding behavior,along with federal funding,has allowed for ample opportunity for research in the discipline of addiction psychiatry. Addiction psychiatry is an expanding field,and currently there is a high demand for substance use disorder experts in both the private and public sector.

There is an ongoing opioid epidemic in the United States,originating out of both medical prescriptions and illegal sources. It has been described as "one of the most devastating public health catastrophes of our time." The opioid epidemic unfolded in three waves. The first wave of the epidemic in the United States began in the late 1990s,according to the Centers for Disease Control and Prevention (CDC),when opioids were increasingly prescribed for pain management,resulting in a rise in overall opioid use throughout subsequent years. The second wave was from an expansion in the heroin market to supply already addicted people. The third wave,starting in 2013,was marked by a steep tenfold increase in the synthetic opioid-involved death rate as synthetic opioids flooded the US market.
Vanila M. Singh is an American physician and professor with involvement in United States health policy. Singh was a candidate for the United States House of Representatives in 2014. Early in her career she taught at UCLA Medical Center,and she is currently an associate professor of anesthesiology,perioperative and pain medicine at Stanford University Medical Center. On June 12,2017,she was appointed the chief medical officer to the Office of the Assistant Secretary for Health at the U.S. Department of Health and Human Services,a presidential appointment at the Senior Executive Service level. She served as Chair of the Inter-Agency Pain Management Task Force established by the CARA Act of 2016,which released its final report on acute and chronic pain management best practices on May 30,2019. Dr. Singh was also appointed as the Acting Regional Health Administrator in Region 9 in August 2018.

In the United States,people with substance use disorders are often blamed for their disease,which is often seen as a moral failing,due to a lack of public understanding about substance use disorders being diseases of the brain with 40-60% heritability. People with substance use disorders are likely to be stigmatized,whether in society or healthcare.
A disease of despair is one of three classes of behavior-related medical conditions that increase in groups of people who experience despair due to a sense that their long-term social and economic prospects are bleak. The three disease types are drug overdose,suicide,and alcoholic liver disease.
