Culture can affect aviation safety through its effect on how the flight crew deals with difficult situations; cultures with lower power distances and higher levels of individuality can result in better aviation safety outcomes. In higher power cultures subordinates are less likely to question their superiors. The crash of Korean Air Flight 801 in 1997 was attributed to the pilot's decision to land despite the junior officer's disagreement, while the crash of Avianca Flight 052 was caused by the failure to communicate critical low-fuel data between pilots and controllers, and by the failure of the controllers to ask the pilots if they were declaring an emergency and assist the pilots in landing the aircraft. The crashes have been blamed on aspects of the national cultures of the crews.[ citation needed ]

Cultural differences in aviation

Geert Hofstede classified national cultures into six dimensions, two of which can be applied to the flight deck: power distance, which defines the "nature of relations between subordinates and superiors", or "how often subordinates are afraid to express disagreement"; [1] and whether the culture is collectivist or individualist in nature. Western cultures are individualistic and have a low power distance, whereas most Asian and Latin cultures are on the other side of the spectrum. [2] Low power-distance and high individualism in Western culture may have contributed to a better safety record than in Taiwan and India.[ citation needed ] In a more collectivist society like Taiwan, a possible explanation for this might be that personal decision-making skills are not as developed. [3] In Western society, the power-distance is lower in general; "making decisions, implementing them, and taking responsibility for their consequences" is part of their life, making personal decisions easier to make. Tenerife airport disaster

On March 27, 1977, two Boeing 747 passenger jets, KLM Flight 4805 and Pan Am Flight 1736, collided on the foggy runway at Los Rodeos Airport (now Tenerife North Airport), on the Spanish island of Tenerife, Canary Islands, killing 583 people, making it the deadliest accident in aviation history. Before takeoff, the KLM flight engineer expressed his concern about the Pan Am not being clear of the runway by asking the pilots in his own cockpit, "Is he not clear, that Pan American?" The KLM captain emphatically replied "Oh, yes" and continued with the takeoff, snubbing the junior officer's concern. This event led to widespread establishment of crew resource management as a fundamental part of airline pilots' training. [6]

Korean Air Flight 801

On approach to Guam in 1997, Korean Air Flight 801 crashed, mainly due to pilot fatigue and poor communication between the flight crew. The captain made the decision to land despite the junior officer's disagreements, eventually bringing the plane down short of the runway, highlighting how a pilot can contribute to a disaster. [7] In high power distance cultures, it is uncommon for subordinates to question their superiors. "Leaders may be autocratic". [8] [ importance? ] High power distance can be seen as the willingness to be in an unequal position, making it a challenge for an officer lower in the hierarchy to question the decisions of the one in power. At the same time, even in a high uncertainty avoidance culture, with the crew more likely to follow standard operating procedures (SOPs), the crew might react less efficiently to a novel situation. Avianca Flight 052

Avianca Flight 052 from Bogotá to New York crashed after running out of fuel, a problem caused by language and cultural barriers. Both crew spoke Spanish as their primary language, but the first officer had better proficiency in English. "Colombia is a highly masculine, high power distance, and collectivist country", which might have led to the crew's reluctance to ask for help from the New York controllers when they knew they were in trouble. [10]

Japan Air Lines Cargo Flight 1045

In 1977, Japan Air Lines Cargo Flight 1045 (a cargo aircraft) crashed shortly after takeoff from Anchorage en route to Tokyo, killing all 3 crew. The captain was a US national, with the other two being Japanese. Neither Japanese pilot mentioned the captain's intoxication or stopped him from flying the plane. They were reluctant to do so, and given Japan's moderately high power-distance index, their deference to authority could have been a major contributing factor. Had they done so, it would have humiliated the captain, who was clearly their superior, and from there on, it was impossible "to prevent the captain from taking control of the aircraft, even at the cost of an accident." Germanwings Flight 9525

In 2015, Germanwings Flight 9525 was crashed intentionally into the French Alps by first officer Andreas Lubitz after the captain left the cockpit for a bathroom break; the captain was then locked out. Commentators noted cultural factors that indirectly lead to the crash, including Germany's strong protection of privacy and the perception that German regulations could do no wrong. [12] Germany's privacy laws usually inhibit a physician from disclosing a patient's medical history to a third party, as was the case with Lubitz, who never disclosed his mental health issues with his employer Lufthansa (who owned Germanwings). [13]

Other impacts of culture in airline safety

Although crew resource management (CRM) can improve safety in the aviation industry, it is not widely accepted across all cultures. This is likely due to differences in uncertainty avoidance, or "the need for rule-governed behavior and clearly defined procedures". Standard operating procedures are more easily accepted in high uncertainty avoidance cultures, such as Greece, Switzerland, Korea, and some Latin American cultures. In the United States, however, where flexibility is emphasized, pilots may not be as accepting of CRM culture. [14]

Improvements can be made to CRM by drawing on the strengths of both individualistic and collectivisic cultures. Western assertiveness can be helpful in developing a low power-distance cockpit, while the Eastern interdependence brings cooperation, interdependence, and communication to create a safer flying environment. [15]

Ideally, "CRM represents low power distance (free exchange of information among the crew) and collectivism (recognition and acceptance of crew interdependence), a rare cultural combination." [16]

The Tenerife airport disaster occurred on March 27, 1977, when two Boeing 747 passenger jets collided on the runway at Los Rodeos Airport on the Spanish island of Tenerife. The collision occurred when KLM Flight 4805 initiated its takeoff run during dense fog while Pan Am Flight 1736 was still on the runway. The impact and resulting fire killed all on board KLM Flight 4805 and most of the occupants of Pan Am Flight 1736, with only 61 survivors in the front section of the aircraft. With a total of 583 fatalities, the disaster is the deadliest accident in aviation history.

United Airlines Flight 232 was a regularly scheduled United Airlines flight from Stapleton International Airport in Denver to O'Hare International Airport in Chicago, continuing to Philadelphia International Airport. On July 19, 1989, the DC-10 serving the flight crash-landed at Sioux Gateway Airport in Sioux City, Iowa, after suffering a catastrophic failure of its tail-mounted engine due to an unnoticed manufacturing defect in the engine's fan disk, which resulted in the loss of many flight controls. Of the 296 passengers and crew on board, 112 died during the accident, while 184 people survived. 13 of the passengers were uninjured. It was the deadliest single-aircraft accident in the history of United Airlines.

In aviation, a controlled flight into terrain is an accident in which an airworthy aircraft, fully under pilot control, is unintentionally flown into the ground, a mountain, a body of water or an obstacle. In a typical CFIT scenario, the crew is unaware of the impending collision until impact, or it is too late to avert. The term was coined by engineers at Boeing in the late 1970s.

This includes preventing aviation accidents and incidents through research, educating air travel personnel, passengers and the general public, as well as the design of aircraft and aviation infrastructure. The aviation industry is subject to significant regulation and oversight.

Saudia Flight 163 was a scheduled Saudia passenger flight departing from Quaid-E-Azam Airport in Karachi, Pakistan, bound for Kandara Airport in Jeddah, Saudi Arabia, via Riyadh International Airport in Riyadh, Saudi Arabia, which caught fire after takeoff from Riyadh International Airport on 19 August 1980. Although the Lockheed L-1011-200 TriStar made a successful emergency landing at Riyadh, the flight crew failed to perform an emergency evacuation of the airplane, leading to the deaths of all 287 passengers and 14 crew on board the aircraft from smoke inhalation.

Crew resource management or cockpit resource management (CRM) is a set of training procedures for use in environments where human error can have devastating effects. CRM is primarily used for improving aviation safety and focuses on interpersonal communication, leadership, and decision making in aircraft cockpits. Its founder is David Beaty, a former Royal Air Force and a BOAC pilot who wrote "The Human Factor in Aircraft Accidents" (1969). Despite the considerable development of electronic aids since then, many principles he developed continue to prove effective.

Power distance is the unequal distribution of power between parties, and the level of acceptance of that inequality; whether it is in the family, workplace, or other organizations.

<span class="mw-page-title-main">Pilot error</span> Decision, action or inaction by a pilot of an aircraft

Pilot error generally refers to an accident in which an action or decision made by the pilot was the cause or a contributing factor that led to the accident, but also includes the pilot's failure to make a correct decision or take proper action. Errors are intentional actions that fail to achieve their intended outcomes. The Chicago Convention defines the term "accident" as "an occurrence associated with the operation of an aircraft [...] in which [...] a person is fatally or seriously injured [...] except when the injuries are [...] inflicted by other persons." Hence the definition of "pilot error" does not include deliberate crashing.

<span class="mw-page-title-main">Gulf Air Flight 072</span> 2000 aviation accident

Gulf Air Flight 072 (GF072/GFA072) was a scheduled international passenger flight from Cairo International Airport in Egypt to Bahrain International Airport in Bahrain, operated by Gulf Air. On 23 August 2000 at 19:30 Arabia Standard Time (UTC+3), the Airbus A320 crashed minutes after executing a go-around upon failed attempt to land on Runway 12. The flight crew suffered from spatial disorientation during the go-around and crashed into the shallow waters of the Persian Gulf 2 km (1 nmi) from the airport. All 143 people on board the aircraft were killed.

Eastern Air Lines Flight 212 was a controlled flight into terrain accident of a McDonnell Douglas DC-9 during approach to Charlotte Douglas International Airport in North Carolina. The incident occurred on September 11, 1974, killing 72 of the 82 people on board. The scheduled flight was from Charleston Municipal Airport to Chicago O'Hare, with an intermediate stop in Charlotte.

KLM Cityhopper Flight 433 was a Saab 340B, registered as PH-KSH, which crashed during an emergency landing on 4 April 1994 and killing 3 occupants, including the captain. Flight 433 was a routine scheduled flight from Amsterdam, the Netherlands, to Cardiff, Wales. The accident was caused by inadequate pilot training and faulty failure sensor, leading to loss of control during go-around.

Individualistic cultures are characterized by individualism, which is the prioritization or emphasis of the individual over the entire group. In individualistic cultures people are motivated by their own preference and viewpoints. Individualistic cultures focus on abstract thinking, privacy, self-dependence, uniqueness, and personal goals. The term individualistic culture was first used in the 1980s by Dutch social psychologist Geert Hofstede to describe countries and cultures that are not collectivist, Hofstede created the term individualistic culture when he created a measurement for the five dimensions of cultural values.

Maritime resource management (MRM) or bridge resource management (BRM) is a set of human factors and soft skills training aimed at the maritime industry. The MRM training programme was launched in 1993 – at that time under the name bridge resource management – and aims at preventing accidents at sea caused by human error.

<span class="mw-page-title-main">TransAsia Airways Flight 222</span> 2014 passenger plane crash in Huxi, Penghu, Taiwan

TransAsia Airways Flight 222 was a scheduled domestic passenger flight operated by TransAsia Airways from Kaohsiung, Taiwan, to Magong, Penghu Island. On 23 July 2014, the ATR 72-500 twin turboprop operating the route crashed into buildings during approach to land in bad weather at Magong Airport. Among the 58 people on board, only 10 survived.

In aviation safety, threat and error management (TEM) is an overarching safety management approach that assumes that pilots will naturally make mistakes and encounter risky situations during flight operations. Rather than try to avoid these threats and errors, its primary focus is on teaching pilots to manage these issues so they do not impair safety. Its goal is to maintain safety margins by training pilots and flight crews to detect and respond to events that are likely to cause damage (threats) as well as mistakes that are most likely to be made (errors) during flight operations.

Pilot decision making, also known as aeronautical decision making (ADM), is a process that aviators perform to effectively handle troublesome situations that are encountered. Pilot decision-making is applied in almost every stage of the flight as it considers weather, air spaces, airport conditions, estimated time of arrival and so forth. During the flight, employers pressure pilots regarding time and fuel restrictions since a pilots’ performance directly affects the company’s revenue and brand image. This pressure often hinders a pilot's decision-making process leading to dangerous situations as 50% to 90% of aviation accidents are the result of pilot error.

Stress in the aviation industry is a common phenomenon composed of three sources: physiological stressors, psychological stressors, and environmental stressors. Professional pilots can experience stress in flight, on the ground during work-related activities, and during personal time because of the influence of their occupation. An airline pilot can be an extremely stressful job due to the workload, responsibilities and safety of the thousands of passengers they transport around the world. Chronic levels of stress can negatively impact one's health, job performance and cognitive functioning. Being exposed to stress does not always negatively influence humans because it can motivate people to improve and help them adapt to a new environment. Unfortunate accidents start to occur when a pilot is under excessive stress, as it dramatically affects his or her physical, emotional, and mental conditions. Stress "jeopardizes decision-making relevance and cognitive functioning" and it is a prominent cause of pilot error. Being a pilot is considered a unique job that requires managing high workloads and good psychological and physical health. Unlike the other professional jobs, pilots are considered to be highly affected by stress levels. One study states that 70% of surgeons agreed that stress and fatigue don't impact their performance level, while only 26% of pilots denied that stress influences their performance. Pilots themselves realize how powerful stress can be, and yet many accidents and incidents continues to occur and have occurred, such as Asiana Airlines Flight 214, American Airlines Flight 1420, and Polish Air Force Tu-154.

NOTECHS is a system used to assess the non-technical skills of crew members in the aviation industry. Introduced in the late 1990s, the system has been widely used by airlines during crew selection process, picking out individuals who possess capable skills that are not directly related to aircraft controls or systems. In aviation, 70 percent of all accidents are induced from pilot error, lack of communication and decision making being two contributing factors to these accidents. NOTECHS assesses and provides feedback on the performance of pilots' social and cognitive skills to help minimize pilot error and enhance safety in the future. The NOTECHS system also aims to improve the Crew Resource Management training system.

On 14 October 2017, an Antonov An-26 transport aircraft of Valan International Cargo Charter crashed shortly before it was due to land at Félix-Houphouët-Boigny International Airport, Abidjan, Ivory Coast. Four of the ten people on board were killed.
