The Kendrick Extrication Device (K.E.D.) is a device that is used in vehicle extrication to remove victims of traffic collisions from motor vehicles. Commonly carried on ambulances, the K.E.D. is typically applied by an emergency medical technician, paramedic, or another first responder. Typically used in conjunction with a cervical collar, the K.E.D. is a semi-rigid brace that secures the head, neck and torso in an anatomically neutral position. This position reduces the possibility of additional injuries to these regions during extrication. The original K.E.D. was designed by Richard Kendrick in 1978. [1]

Description

Typically there are two head straps, three torso straps, and two legs straps which are used to adequately secure the K.E.D. to the victim. Unlike a long spine board or litter, the K.E.D. uses a series of wooden or polymer bars in a nylon jacket, allowing the responders to immobilize the neck and upper spine and remove the victim from the vehicle or other confined space. Although the K.E.D. can also be used to immobilize infants and children, it is preferable to use specifically designed pediatric immobilization devices whenever possible. If the K.E.D. is used to immobilize an infant or child, appropriate padding must be used to ensure complete immobilization in a manner that does not obscure the thorax and abdomen, thereby preventing continued assessment of these vital areas.

Application

The device can be quickly and easily inserted into the seat of a vehicle by a single rescuer, allows access to the airway and conforms to any body size. [2] The K.E.D. is typically used only on hemodynamically stable victims; unstable victims are extricated using rapid extrication techniques without the prior application of the K.E.D.

Once the K.E.D. is slid into position, it is secured to the victim with straps in order to prevent movement. The first strap that is secured is the middle torso strap. According to the K.E.D. users' manual securing this strap secures the greatest area of the device and therefore provides the greatest stability while securing the rest of the device. Next the bottom torso strap is secured, however the top torso strap is not secured until just prior to moving the patient to the long spine board. This is to allow the patient to breathe easily while the rest of the device is secured. Following the bottom torso strap the leg straps are secured. These may be applied in a "criss-cross" fashion (according to the K.E.D. users' manual this is the most commonly used method), or applied by securing them to their respective sides. If there is any evidence of a groin injury the "criss-cross" method cannot be used. Following application of the leg straps the void between the head and device is padded as needed and the head is secured. Finally, just prior to moving the patient to a long spine board the top strap is secured. [3] Some schools teach this order by remembering the phrase "My Baby Looks Hot Tonight” or "Maori Boys Love Hairy Teke", where the beginning of each word stands for Middle torso strap, Bottom torso strap, Leg straps, Head strap and Top torso strap.

The head pad can bring the head too far forward for the side panels to fully immobilize it. Care must be taken to secure the head properly to maintain neutral immobilization. If the head is too far forward, the head is brought back to meet the K.E.D. unless crepitus, pain or resistance is met. If these symptoms are present, the head is immobilized in the position found.

It is worth noting that there is debate and controversy surrounding the exact order of applying the torso straps, with some saying that the order does not matter, as long as the torso is secured before the head. The K.E.D. users' manual is sometimes used in this justification with the explanation that it states the reason the top strap is last is a function of breathing, and not the process of immobilization itself. [4] There is further controversy about whether the KED still has a place in prehospital care [5] .

In medical forensics the right arm of a corpse is left hanging off the stretcher to let paramedics know it is not a wounded patient. They are also used to hold prisoners during lethal injections in the United States.

Splint (medicine)

A splint is defined as "a rigid or flexible device that maintains in position a displaced or movable part; also used to keep in place and protect an injured part" or as "a rigid or flexible material used to protect, immobilize, or restrict motion in a part". Splints can be used for injuries that are not severe enough to immobilize the entire injured structure of the body. For instance, a splint can be used for certain fractures, soft tissue sprains, tendon injuries, or injuries awaiting orthopedic treatment. A splint may be static, not allowing motion, or dynamic, allowing controlled motion. Splints can also be used to relieve pain in damaged joints. Splints are quick and easy to apply and do not require a plastering technique. Splints are often made out of some kind of flexible material and a firm pole-like structure for stability. They often buckle or Velcro together.

Orthopedic cast Medical aid for the treatment of bone fractures

An orthopedic cast, or simply cast, is a shell, frequently made from plaster or fiberglass, that encases a limb to stabilize and hold anatomical structures—most often a broken bone, in place until healing is confirmed. It is similar in function to a splint.

Spinal board

A spinal board, is a patient handling device used primarily in pre-hospital trauma care. It is designed to provide rigid support during movement of a person with suspected spinal or limb injuries. They are most commonly used by ambulance staff, as well as lifeguards and ski patrollers. Historically, backboards were also used in an attempt to "improve the posture" of young people, especially girls.

Vacuum mattress

A vacuum mattress, or vacmat, is a medical device used for the immobilisation of patients, especially in case of a vertebra, pelvis or limb trauma. It is also used for manual transportation of patients for short distances. It was invented by Loed and Haederlé, who called it "shell" mattress.

Vehicle extrication

Vehicle extrication is the process of removing a vehicle from around a person who has been involved in a motor vehicle collision, when conventional means of exit are impossible or inadvisable. A delicate approach is needed to minimize injury to the victim during the extrication. This operation is usually accomplished by using chocks and bracing for stabilization and hydraulic tools, including the Jaws of Life. Standards and regulations for organizations can be found in NFPA 1670 and for individual members in 1006.

Cervical collar

A cervical collar, also known as a neck brace, is a medical device used to support a person's neck. It is also applied by emergency personnel to those who have had traumatic head or neck injuries, and can be used to treat chronic medical conditions.

Cervical fracture

A cervical fracture, commonly called a broken neck, is a fracture of any of the seven cervical vertebrae in the neck. Examples of common causes in humans are traffic collisions and diving into shallow water. Abnormal movement of neck bones or pieces of bone can cause a spinal cord injury resulting in loss of sensation, paralysis, or usually instant death.

A traction splint most commonly refers to a splinting device that uses straps attaching over the pelvis or hip as an anchor, a metal rod(s) to mimic normal bone stability and limb length, and a mechanical device to apply traction to the limb.

Litter (rescue basket)

A litter is a stretcher or basket designed to be used where there are obstacles to movement or other hazards: for example, in confined spaces, on slopes, in wooded terrain. Typically it is shaped to accommodate an adult in a face up position and it is used in search and rescue operations. The person is strapped into the basket, making safe evacuation possible. The person generally is further protected by a cervical collar and sometimes a long spine board, so as to immobilize the person and prevent further injury.

The Milwaukee brace, also known as a cervico-thoraco-lumbo-sacral orthosis or CTLSO, is a back brace most often used in the treatment of spinal curvatures in children but also, more rarely, in adults to prevent collapse of the spine and associated pain and deformity. It is a full-torso brace that extends from the pelvis to the base of the skull. It was originally designed by Blount and Schmidt in 1946 for postoperative care when surgery required long periods of immobilization.

Grady straps are a specific strapping configuration used in full body spinal immobilization.

Helmet removal (sports)

In organized athletics, few situations give rise to greater anxiety than the 'downed athlete'. Obvious causes include head and neck injury, or both, with no immediate means of excluding neck injury in the athlete who may be unable to give a history.
Compounding the problem is the potential for the athlete's airway being compromised. Such a situation requires effective triage with the possibility of the athlete's injury being worsened or its effects being made permanent, if the initial steps taken are not appropriate.
Paradoxically, the equipment designed to prevent or mitigate injury, such as helmets, face masks, neck rolls and shoulder pads, contributes to the complexity of the steps needed to be taken.
Finally, environmental challenges, such as the difficulty of first responders moving on an ice arena, or maneuvering an ambulance through inadequate access routes, will add to the problem if not previously addressed by inspection of facilities and, in the best circumstances, rehearsal.

Airtraq

Airtraq is a fibreoptic intubation device used for indirect tracheal intubation in difficult airway situations. It is designed to enable a view of the glottic opening without aligning the oral with the pharyngeal, and laryngeal axes as an advantage over direct endotracheal intubation and allows for intubation with minimal head manipulation and positioning.
