MV Kaawa is a Lake Victoria ferry that was operated by the Uganda Railways Corporation.
In the early hours of 8 May 2005 Kaawa was en route to Mwanza in Tanzania when she was involved in a collision with MV Kabalega, which was en route to Port Bell in Uganda. [3] Kaawa damaged her bow and Kabalega was damaged below the waterline. [3] Kaawa managed to return to port but a few hours after the collision Kabalega sank about 8 nautical miles (15 km) southeast of the Ssese Islands. [3]
URC's Chairman of Governors Paul Etiang admitted that marine insurance for Kaawa, and Kabalega and their sister ship MV Pemba had expired in December 2004 and not been renewed. [4] After the collision Kaawa was withdrawn from service for repairs to her bow and Pemba was suspended from service.
The collision was at about 0300 hrs when Kaawa's third officer, Stephen Kaliisa, was on watch. [5] Kaliisa told a Ugandan Parliamentary committee that Kaawa had no navigation lights and one of her radars was missing. [5] Maritime convention is that ships passing each other in opposite directions should pass on each other's starboard side. However, Kaliisa told the committee that Kaawa had lacked a starboard navigation light "for a long time". [5]
Kaliisa also told the committee that three years before the collision he had been appointed as third officer despite being only a sailor with no marine training or qualifications. [5] Kaawa's First Officer, Samuel Kyabukulu, told the committee that Kaliisa was not qualified to steer the ship. Despite this, Kaawa's master, Captain Albert Ocaya, told the committee "When I handed over the ship (i.e. to Kaliisa), I did not expect anything strange" and "If Kaliisa had notified me about the trouble, the accident would have been averted". [5] However, as master, Ocaya would have been responsible both for Kaawa sailing in darkness without navigation lights, without one of her radars, and with a third officer who had none of the necessary qualifications.
A commission of inquiry attributed the collision to inadequately trained crew on the bridges of both ferries and a lack of communication equipment. [6] It also noted that both ferries had been involved in previous collisions, a previous incident report had been published a decade earlier but its recommendations had not been implemented. [6] The report also highlighted the absence of a search and rescue unit that could have reached the sinking ferry soon enough. [6]
In September 2005 Uganda's minister of works, John Nasasira, claimed that structural repairs Kaawa's collision damage and to her sister ship Pemba were almost complete but that USh 3.4 billion was needed "to fully repair and maintain, class and insure the vessels". [6] By October 2009 the two ferries were still out of service but Nasasira claimed the Government would recondition both vessels and would return Pemba to service in 2010 and Kaawa in 2011. [7]

MV Queen of the North was a roll-on/roll-off (RORO) ferry built by AG Weser of Germany and operated by BC Ferries, which ran along an 18-hour route along the British Columbia Coast of Canada between Port Hardy and Prince Rupert, British Columbia, a route also known as the Inside Passage. On March 22, 2006, with 101 people aboard, she failed to make a planned course change, ran aground and sank. Two passengers, whose bodies were never found, died in the incident. The ship had a gross register tonnage of 8,806, and an overall length of 125 metres (410 ft). She had a capacity of 700 passengers and 115 cars.

On 10 September 2011, MV Spice Islander I, a passenger ferry carrying over 2,000 passengers, sank off the coast of Zanzibar. The ferry was travelling between Unguja and Pemba, two islands off the coast of mainland Tanzania, when it capsized. Early estimates put the death toll at around 200, but a report published by the Tanzanian government in January 2012 claimed that over 1,500 people had been killed.
MV Baltic Ace was a Bahamian-flagged car carrier, that sank in the North Sea on 5 December 2012 after a collision with the Cyprus-registered container ship Corvus J. Built by Stocznia Gdynia in Poland, the ship had been in service since 2007.
The ferry MV Sewol sank on the morning of April 16, 2014, en route from Incheon towards Jeju in South Korea. The 6,825-ton vessel sent a distress signal from about 2.7 kilometres north of Byeongpungdo at 08:58 KST. Out of 476 passengers and crew, 304 died in the disaster, including around 250 students from Danwon High School in Ansan City. Of the 172 survivors, more than half were rescued by fishing boats and other commercial vessels that arrived at the scene approximately 40 minutes before the Korea Coast Guard (KCG).
At 5:24 a.m. on 21 August 2017, USS John S. McCain, a United States Navy warship, was involved in a collision with the Liberian-flagged tanker Alnic MC off the coast of Singapore and Malaysia, east of the Strait of Malacca. According to a U.S. Navy press release, the breach "resulted in flooding to nearby compartments, including crew berthing, machinery, and communications rooms." Ten US Navy sailors died as a result of the crash, which prompted the Maritime and Port Authority (MPA) of Singapore to start a multi-agency search-and-rescue (SAR) effort as the agency responsible for coordinating SAR operations within Singapore's Maritime Search and Rescue Region (MSRR). The Singapore Transport Safety Investigation Bureau (TSIB) also launched a marine safety investigation following the collision in accordance with the International Maritime Organisation's Casualty Investigation Code in Singapore's capacity as a coastal state, and published its final report on 8 March 2018. The U.S. Navy announced on 24 August 2017 that it had suspended search-and-rescue efforts for survivors in the open sea to focus on the recovery of the remains of the missing sailors still inside the flooded compartments of the ship. By 27 August, U.S. Navy and United States Marine Corps divers had recovered the remains of all ten sailors. On 12 September 2017, the United States' chargé d'affaires Stephanie Syptak-Ramnath expressed thanks for Singapore's support during the SAR operations. The McCain returned to service in June 2020.
MS Estonia sank on Wednesday, 28 September 1994, between about 00:50 and 01:50 (UTC+2) as the ship was crossing the Baltic Sea, en route from Tallinn, Estonia, to Stockholm, Sweden. The sinking was one of the worst maritime disasters of the 20th century. It is one of the deadliest peacetime sinkings of a European ship, after the Titanic in 1912 and the Empress of Ireland in 1914, and the deadliest peacetime shipwreck to have occurred in European waters, with 852 lives stated at the time as officially lost.
