Newly sober John was unhappy with the US cover art, so the 1991 UK release was issued with new cover art and also replaced "You Gotta Love Someone" and "I Swear I Heard the Night Talkin'" with then-unreleased "Suit of Wolves" and "Understanding Women", the former a B-side to "The One" and the latter later included as a track on the 1992 album The One. The second part of the two-part title track, "The Road to Hell ", is one of Rea's most famous songs, and his only single to reach the UK Top 10. Geffen Records released the album in the US, adding the 1988 re-recording of Let's Dance. Allmusic describes the album as a "modern masterpiece".
