The public health National Support Teams or NSTs [1] constituted a consultancy-style organisational development and change management service provided by the UK Government Department of Health.
Established in 2006, they provided consultancy-style support and assistance to local health economies across England (the UK's devolved administrations – Scotland, Wales and Northern Ireland generally having their own arrangements). Each team had access to national-level expertise on specific public health specialisms, including:
Consultancy and support services were provided free of charge to public-sector clients including local authorities and NHS Trusts. The feedback reports generated as a result of these activities were not published by the Department of Health, although some clients choose to post reports on their own websites.
Each national support team also shared observations with central Government policy advisers on the real-world public health challenges it identified through this work.
The primary activity of each Team was to carry out visiting inputs to local areas across England, more than 450 of which were undertaken during the existence of the NSTs. These visits lasted between two days and one week, using a mix of in-house NST experts and externally sourced advisers where appropriate. Subsequent change management support was offered as appropriate to the changes recommended, often around strengthening strategic coherence, enhancing commissioning capabilities to respond to public health challenges, or improving operational effectiveness of treatment and/or behaviour change services.
The overall style of the national support teams was planned to be in line with Peter Block's 'Flawless Consulting' approach. [2] The diagnostic model employed was broadly influenced by appreciative inquiry, due to its tested relevance to healthcare environments and the emphasis upon positive potential to enhance organisational function (rather than a performance management approach, which initial client feedback suggested would be perceived as judgemental). Recommendations for change were presented at closing plenary sessions to both senior leadership and operational managers with sufficient detail to trigger follow-up action and a delivery style reported by audiences as more memorable than was initially expected of an organisational development input.
NSTs created and published a range of practical guides to achieve public health improvements (several titled as 'high impact changes' guides) as well as summaries of applied learning from their reviews and support work in localities across England. Several of these remain available on the Department of Health website, [3] and a wider range is available from the digital legacy site hosted by the National Archives. [4] The feedback from NSTs to central Government policy teams also informed (and in some cases instigated) the publication of other policy documents including 'white papers' outlining proposed strategic improvements to overall action on public health.
As outlined in the NSTs' own end-of-project report (available at the Department for Health website [5] ) the NSTs achieved significant results in strengthening the performance of a diverse range of NHS and local government public health interventions, resulting in high approval levels on the part of chief executives and directors of public health who considered this to have been one of the most useful services which the Department of Health had made available.
All teams closed in March 2011 as a result of the funding cuts affecting several areas of the public sector in the UK, following a perceived need to reduce public expenditure during the 2007–2008 financial crisis and related recession. Internal communications to NST staff explained the closure as the result of a decision to abolish all posts not covered by a traditional civil service employment contract; as this included almost all NST employees, the service was to close as a consequence – it was stressed that closure was not intended to imply a comment on the success or usefulness of the NST approach.
Specialist medical magazine GP reported the regret voiced by public health leaders following the closure of the NSTs in August 2011. Their stories [6] [7] subsequently picked up by The Daily Telegraph newspaper [8] and BBC News [9] explained how the services of NSTs had been received and questioned the rationale for disbanding them. In response, an unidentified Department of Health spokesperson suggested that the teams were no longer required as they had worked to targets discarded by the incoming new national administration in May 2010, although the teams had continued to provide services used by localities and intelligence brought back into central Government for the rest of that financial year; the spokesperson did not suggest a new means by which continuing aspirations to address public health challenges could now be supported.
The Department of Health and Social Care (DHSC) is a ministerial department of the Government of the United Kingdom. Strategic health authorities (SHA) were part of the structure of the National Health Service in England between 2002 and 2013. Each SHA was responsible for managing performance, enacting directives and implementing health policy as required by the Department of Health at a regional level. Initially 28 in number, they were reduced to 10 in 2006. Along with primary care trusts, they were replaced by clinical commissioning groups and Public Health England in 2013 under the Health and Social Care Act 2012.
NHS Direct was the health advice and information service provided by the National Health Service (NHS), established in March 1998. The nurse-led telephone information service provided residents and visitors in England with healthcare advice 24 hours a day, every day of the year through telephone contact on the national non-geographic 0845 46 47 number. The programme also provided a web based symptom checkers on the NHS Direct website and via mobile, both as apps for iPhone and Android smart phones and a mobile website.

CRD was established in January 1994, and aims to provide research-based information for evidence-based medicine. CRD carries out systematic reviews and meta-analyses of healthcare interventions, and disseminates the results of research to decision-makers in the NHS.
Emergency medical services in the United Kingdom provide emergency care to people with acute illness or injury and are predominantly provided free at the point of use by the four National Health Services (NHS) of England, Scotland, Wales, and Northern Ireland. Emergency care including ambulance and emergency department treatment is only free to UK residents and a charge may be made to those not entitled to free NHS care.

Local involvement networks (LINks) were launched by England's National Health Service in April 2008 following the Local Government and Public Involvement in Health Act 2007. They replaced the Commission for Patient and Public Involvement in Health (CPPIH) and patient and public involvement forums and existed in every local authority area with a responsibility for NHS health care and social services.
The National Collaborating Centre for Mental Health (NCCMH) is a collaboration between the Royal College of Psychiatrists and the Centre for Outcomes Research and Effectiveness at University College London (UCL). The NCCMH aims to promote the role of evidence synthesis in making informed judgments about healthcare policy. The NCCMH has a history of developing guidelines, conducting systematic reviews and developing implementation guidance for commissioners and service providers. Formed in 2001, on 1 April 2016 a new guideline development centre, the National Guideline Alliance, based at the Royal College of Obstetricians and Gynaecologists took over the clinical guideline programme that had been run by NCCMH.
NHS Evidence was a UK health care evidence search service that enabled users to access clinical and non-clinical evidence and best practice information through its web-based portal. Its purpose was to help users within the National Health Service (NHS), public health and social care sectors in decision-making by fostering evidence-based practices. NHS Evidence was managed by the National Institute for Health and Clinical Excellence (NICE). It was created in 2008 and as of 2024 is no longer in operation; NICE directs would-be users to alternate services on its website.
111 is a free-to-call single non-emergency number medical helpline operating in England, Scotland and Wales. The 111 phone service has replaced the various non-geographic 0845 rate numbers and is part of each country's National Health Service: in England the service is known as NHS 111; in Scotland, NHS 24; and in Wales, NHS111 Wales.
Public Health England (PHE) was an executive agency of the Department of Health and Social Care in England which began operating on 1 April 2013 to protect and improve health and wellbeing and reduce health inequalities. Its formation came as a result of the reorganisation of the National Health Service (NHS) in England outlined in the Health and Social Care Act 2012. It took on the role of the Health Protection Agency, the National Treatment Agency for Substance Misuse and a number of other health bodies. It was an executive agency of the Department of Health and Social Care, and a distinct delivery organisation with operational autonomy.

It operates a membership base of healthcare professionals who work flexible shifts and longer-term placements at more than 130 acute and community NHS Trusts and other health and care organisations across the UK and the Channel Islands. The company reports its Bank Members worked 4.6 million shifts and provided 39 million hours of care in the 2021–22 financial year.
Public Health Wales is an NHS Trust which was established on 1 October 2009 as part of a major restructuring of the health service in Wales. It aims to protect and improve health and wellbeing and reduce health inequalities in Wales.
In England, a sustainability and transformation plan (STP) is a non-statutory requirement which promotes integrated provision of healthcare, including purchasing and commissioning, within each geographical area of the National Health Service. The plans were introduced in 2016 but by 2018 had been overtaken by progress towards integrated care systems.

Exercise Cygnus was a three-day simulation exercise carried out by the UK Government in October 2016 to estimate the impact of a hypothetical H2N2 influenza pandemic on the United Kingdom. It aimed to identify strengths and weaknesses within the United Kingdom health system and emergency response chain by putting it under significant strain, providing insight on the country's resilience and any future ameliorations required. It was conducted by Public Health England representing the Department of Health and Social Care, as part of a project led by the "Emergency Preparedness, Resilience and Response Partnership Group". Twelve government departments across Scotland, Wales and Northern Ireland, as well as local resilience forums (LRFs) participated. More than 950 workers from those organisations, prisons and local or central government were involved during the three-day simulation, and their ability to cope under situations of high medical stress was tested.
