On September 19, 2022, while pitching against the Minnesota Twins, Hentges tossed 2+13 perfect innings to earn his first career save. [11] In 2022, Hentges made 57 appearances out of the bullpen for Cleveland, and went 3–2 with a 2.32 ERA, 0.968 WHIP and 72 strikeouts across 62 innings. [12] Hentges pitched in 56 contests for the Guardians in 2023, compiling a 3–2 record and 3.61 ERA with 56 strikeouts across 52+13 innings pitched. [13]

Hentges began the 2024 campaign with Cleveland, compiling a 3.04 ERA with 27 strikeouts over 25 games. He was placed on the injured list with left shoulder inflammation on July 12, 2024, [14] and was transferred to the 60–day injured list on August 19. [15] On September 10, it was announced that Hentges would require surgery to repair the labrum and capsule in his left shoulder, necessitating a recovery timetable of 12–14 months. [16]
