A senior house officer (SHO) is a non-consultant hospital doctor in the Republic of Ireland and many Commonwealth countries. SHOs usually have a minimum of 1 year post medical school training. SHOs are supervised in their work by consultants and registrars. In training posts these registrars and consultants oversee training and are usually their designated clinical supervisors.
The same structure to junior doctor grades also applied previously in the National Health Service in the UK, and informal use of the term persists there.

In Ireland, physicians typically spend one year as an intern, before becoming a Senior House Officer (SHO). Most doctors spend between 2–4 years working as an SHO. Advancing to registrar level is dependent on experience and aptitude within a specialty; in most cases, becoming a registrar depends primarily on having successfully passed postgraduate examinations such as MRCP/MRCS, although this is not a strict requirement. Most doctors work at registrar level for 1–3 years before being accepted onto a training programme in a particular subspecialty (e.g. Cardiology), after which time, they are known as SpRs (Specialist Registrars). SpR programmes typically last for between 3–6 years. On completion of an SpR programme, doctors are eligible to apply for consultant positions, although the majority opt to undertake 1–2 years of fellowship training – often abroad – prior to appointment at consultant level.[ citation needed ]
Since the introduction of Modernising Medical Careers (MMC), grades in the UK differ from those in the Republic of Ireland.
Before MMC, physicians applied for SHO posts after completing their mandatory pre-registration house officer (PRHO) year after qualifying from medical school. They would typically work as an SHO for 2–3 years, or occasionally longer, before going on to a certain subspeciality where they would take up a specialist registrar post to train as a specialist in that particular field. To qualify for these, SHOs had to be in posts approved by a regional postgraduate dean, as well as passing postgraduate exams (such as the Membership of the Royal College of Physicians, MRCP). SHO jobs typically lasted four or six months in various departments and were often provided in one- or two-year rotations.

In 2002, the Department of Health announced reforms in the training of newly qualified doctors under the banner of "Modernising Medical Careers", [2] [3] merging the PRHO year and the first year of SHO training into a "foundation programme" (FY1 and FY2). This programme was formally introduced in August 2005. [3]
In August 2002, Professor Sir Liam Donaldson, the then Chief Medical Officer, published a report titled "Unfinished Business", which focused on reforming SHO training. [4] The SHO grade was abolished and renamed specialty registrar . This change took place in 2007. Amongst the many changes, many doctors who had completed their Foundation Training were now appointed into a "run-through" training programme that incorporated the previous SHO and specialist registrar grades. The job title changed from "SHO" to "ST1/ST2" (specialist trainee year 1 & year 2). Part of this decision was subsequently reversed in a number of specialties, with competitive entry into the registrar grade ("uncoupled training"), and in these specialties the SHO level posts are referred to as "CT1/CT2" (core trainee year 1 & year 2) and in some "CT3".[ citation needed ]
Some hospitals use the term senior house officer in an unofficial capacity for physicians in FY2 and CT1/2 year, who often have similar working patterns in duty shift rotas.[ citation needed ]
The term still applies to non training posts, those organised by a hospital/trust as opposed to a Deanery, where a physician is employed at a level after full registration with the GMC and before entering higher specialty training. These posts are typically referred to as "Trust SHO", "Junior Clinical Fellow" or other such terms.
In the United States, physicians in training are also sometimes referred to as "senior house officers" in their later years of residency, but the term is more variable in its American than its European usage.
Specializing post-graduate clinical students are still referred to as Senior House Officers in many other countries around the world, most of which were British colonies and adopted the British education system like Uganda.
In the medical profession, a general practitioner (GP) or family physician is a physician who treats acute and chronic illnesses and provides preventive care and health education to patients of all ages. GPs' duties are not confined to specific fields of medicine, and they have particular skills in treating people with multiple health issues. They are trained to treat patients to levels of complexity that vary between countries. The term "primary care physician" is more usually used in the US. In Asian countries like India, this term has been replaced mainly by Medical Officers, Registered Medical Practitioner etc.
Membership of the Royal Colleges of Physicians of the United Kingdom (MRCP(UK)) is a postgraduate medical diploma in the United Kingdom (UK). The examinations are run by the Federation of the Medical Royal Colleges – the Royal College of Physicians of London, the Royal College of Physicians of Edinburgh, and the Royal College of Physicians and Surgeons of Glasgow. The three Royal Colleges of Physicians share this common three part assessment in general medicine which consists of two written parts and one clinical examination. Examinations are held throughout the UK and in overseas centres.
Residency or postgraduate training is a stage of graduate medical education. It refers to a qualified physician, veterinarian, dentist, podiatrist (DPM) or pharmacist (PharmD) who practices medicine, veterinary medicine, dentistry, podiatry, or clinical pharmacy, respectively, usually in a hospital or clinic, under the direct or indirect supervision of a senior medical clinician registered in that specialty such as an attending physician or consultant. In many jurisdictions, successful completion of such training is a requirement in order to obtain an unrestricted license to practice medicine, and in particular a license to practice a chosen specialty. In the meantime, they practice "on" the license of their supervising physician. An individual engaged in such training may be referred to as a resident, registrar or trainee depending on the jurisdiction. Residency training may be followed by fellowship or sub-specialty training.
In the United Kingdom, junior doctors are qualified medical practitioners working whilst engaged in postgraduate training. The period of being a junior doctor starts when they qualify as a medical practitioner following graduation with a Bachelor of Medicine, Bachelor of Surgery degree and start the UK Foundation Programme. It culminates in a post as a consultant, a general practitioner (GP), or some other non-training post, such as a specialty doctor or associate specialist post.
Medical education in Australia includes the educational activities involved in the initial and ongoing training of Medical Practitioners. In Australia, medical education begins in Medical School; upon graduation it is followed by a period of pre-vocational training including Internship and Residency; thereafter, enrolment into a specialist-vocational training program as a Registrar eventually leads to fellowship qualification and recognition as a fully qualified Specialist Medical Practitioner. Medical education in Australia is facilitated by Medical Schools and the Medical Specialty Colleges, and is regulated by the Australian Medical Council (AMC) and Australian Health Practitioner Regulation Agency (AHPRA) of which includes the Medical Board of Australia where medical practitioners are registered nationally.
Modernising Medical Careers (MMC) is a programme for postgraduate medical training introduced in the United Kingdom in 2005. The programme replaced the traditional grades of medical career before the level of Consultant. The different stages of the programme contribute towards a "Certificate of Completion of Training" (CCT). It has been dogged by criticism within and outside the medical profession, and an independent review of MMC led by Professor Sir John Tooke criticised many aspects of it.
Pre-registration house officer (PRHO), commonly refrerred to as house officer and less commonly as houseman, is a former official term for a grade of junior doctor that was, until 2005, the only job open to medical graduates in the United Kingdom who had just passed their final examinations at medical school and had received their medical degrees. The term "house officer" is still used to refer to foundation doctors.

A medicalintern is a physician in training who has completed medical school and has a medical degree, but does not yet have a license to practice medicine unsupervised. Medical education generally ends with a period of practical training similar to internship, but the way the overall program of academic and practical medical training is structured differs depending upon the country, as does the terminology used.
In the United Kingdom, Ireland, and parts of the Commonwealth, consultant is the title of a senior hospital-based physician or surgeon who has completed all of their specialist training and been placed on the specialist register in their chosen speciality. Their role is entirely distinct from that of a general practitioner.
A Senior Registrar was a grade of doctor in the United Kingdom or Ireland before being superseded during reforms in the 1990s. The senior registrar post still exists in Australia, whilst in the US, the title of “Senior Registrar” might be applied to a Senior or Chief Resident in Surgery.

A foundation doctor is a grade of medical practitioner in the United Kingdom undertaking the Foundation Programme, a two-year, general postgraduate medical training programme which forms the bridge between medical school and specialist/general practice training. Doctors in the first year of the programme are known as Foundation Year 1 (FY1) doctors, and those in the second year are known as Foundation Year 2 (FY2) doctors. Being a foundation doctor is compulsory for all newly qualified medical practitioners in the UK starting from 2005 onwards. The grade of foundation doctor has replaced the traditional grades of pre-registration house officer and senior house officer.
A specialty registrar (StR), previously known as and still commonly referred to as a specialist registrar (SpR), is a doctor, public health practitioner or dentist who is working as part of a specialty training programme in the UK. This is known as a training grade as these doctors are supervised to an extent, as part of a structured training experience that leads to being able to undertake independent practice in a hospital specialty or working as a general practitioner.
The Medical Training Application Service was an on-line application system set up under the auspices of Modernising Medical Careers in 2007 and used for the selection of Foundation House Officers and Specialty Registrars, and allocating them to jobs in the UK. Its implementation was heavily criticised both in the press and within the medical profession, and its operation was marked by the resignation of key staff and serious security breaches. The system affected junior doctors, and so every qualified doctor in the UK who had not yet attained Consultant status.
Fellow of the Royal Australasian College of Physicians, abbreviated as the post-nominal initials FRACP, is a recognition of the completion of the prescribed postgraduate specialist training programme in internal adult or internal paediatric medicine of the Royal Australasian College of Physicians.
The Foundation Programme is a two-year structured programme of workplace-based learning for junior doctors that forms a bridge between medical school and specialty training. The programme aims to provide a safe, well-supervised environment for doctors to put into practice what they learned in medical school. It provides them with the generalist medical knowledge and skills to meet the requirements of the General Medical Council (GMC) The New Doctor (2007) and the Foundation Programme Curriculum (2007) and prepares them for entry into specialty training. All medical graduates must undertake, and complete the Foundation Programme in order to progress onto specialist or a general practitioner training in the UK.
Non-consultant hospital doctor (NCHD), sometimes also referred to as junior doctor, is a term used in Ireland to describe qualified medical practitioners who work under the supervision of a consultant in a particular speciality. It is an umbrella term, encompassing doctors on formal postgraduate training schemes as well as doctors in "service" roles outside of postgraduate training.
Core Medical Training was the two-year part of postgraduate medical training following Foundation Year 1 and 2, successful completion of which is required to enter higher training in the medical subspecialties. The programme is overseen by the Joint Royal Colleges Postgraduate Training Board, which represents the three medical Royal Colleges in the UK: the Royal College of Physicians of London, the Royal College of Physicians of Edinburgh and the Royal College of Physicians and Surgeons of Glasgow.
In Nigeria, an early career doctor (ECD) is a medical or dental practitioner in the early phase of their professional career post-graduation. This includes house officers, resident doctors and medical officers below the rank of principal medical/dental officer (PMO/PDO). This term is more widely used in Nigeria compared to the junior doctor in the United Kingdom and Australia. Although Junior Doctors may seem synonymous with ECDs, ECDs embraces more cadres of doctors than junior doctors especially as it is in the UK or Australia. They may be engaged in training or non-training position at this point in their professional development. Those in training positions while rendering service include House physician or house surgeon and resident doctors. While medical officers and senior medical officers or the dentistry categories are in the non-training position. Generally, this period usually spans about the start of a career to fifteen years depending on the path an early career doctor chose for their professional development.
