The medley relay swimming events for the 2020 Summer Paralympics took place at the Tokyo Aquatics Centre from September 2 to September 3, 2021. A total of two events were contested. [1]

Nicolas Fink is an American competition swimmer who specializes in breaststroke events. He won three gold medals, two silver medals, and a bronze medal at the 2021 World Short Course Championships in relay and individual breaststroke events. Her total career haul of 11 Olympic medals following the 2020 Olympic Games made her Australia's most decorated Olympian and included one gold medal from the 2016 Summer Olympics in Rio de Janeiro and four gold medals from the 2020 Summer Olympics in Tokyo. With four gold and three bronze medals she was the most decorated athlete across all sports at the 2020 Summer Olympics, and tied for the most medals won by a woman in a single Olympic Games. She has also won 17 medals, including four gold medals, at the World Aquatics Championships; and a record 20 medals, including 14 gold, at the Commonwealth Games.

Evgeny Mikhailovich Rylov is a Russian competitive swimmer and Olympic champion specializing in backstroke events. He won three gold medals at the 2014 Summer Youth Olympics in Nanjing, and a bronze medal at his senior international debut at the 2015 World Championships in Kazan. He also won a bronze medal at the 2016 Summer Olympics in Rio de Janeiro and a gold medal at the 2017 World Championships in Budapest, both were in the 200 metre backstroke event. In 2018, at the 2018 World Short Course Championships, he won gold medals in the 200 metre backstroke and 50 metre backstroke. At the 2019 World Championships, he won a gold medal in the 200 metre backstroke, silver medal in the 100 metre backstroke, and silver medal in the 50 metre backstroke. Denis Tarasov is a Paralympic swimmer from Russia competing mainly in category S8 events. At the 2012 Summer Paralympics in London he won five medals, including gold in the 50 metre freestyle S8 event. He has represented Russia at two IPC World Championships with a total of 12 medals. At the 2015 IPC Swimming World Championships in Glasgow he set four world records, two as part of Russian relay teams and two individual records, in the 50m and 100m freestyle S8 events.

Caeleb Remel DresselOLY is an American professional swimmer who specializes in freestyle, butterfly, and individual medley events. He swims representing the Cali Condors as part of the International Swimming League. He won a record seven gold medals at the 2017 World Aquatics Championships in Budapest, nine medals, six of which were gold, at the 2018 World Swimming Championships in Hangzhou, and eight medals, including six gold, at the 2019 World Aquatics Championships in Gwangju. Dressel is a seven-time Olympic gold medalist and holds world records in the 100 meter butterfly, 50 meter freestyle, and 100 meter individual medley.

Kyle Chalmers, is an Australian competitive swimmer who specialises in freestyle and butterfly events. He is the world record holder in the short course 100 metre freestyle. He is also the Oceanian record and the Australian record holder in the short course 50 metre butterfly and 50 metre freestyle events.

<span class="mw-page-title-main">Michael Andrew (swimmer)</span> American swimmer

Michael Charles Andrew is an American competitive swimmer and an Olympic gold medalist. He was the 2016 world champion in the 100 meter individual medley. At his first Olympic Games, the 2020 Summer Olympics, he won a gold medal and set a world record as part of the 4x100 meter medley relay, placed fourth in the 100 meter breaststroke, fourth in the 50 meter freestyle, and fifth in the 200 meter individual medley. Andrew's swims in 2021 at the 2020 Olympics made him the first swimmer to represent the United States at an Olympic Games in an individual breaststroke event as well as another individual event other than an individual medley in the then-125-year-history of swimming at the Summer Olympics. He has won 76 medals at Swimming World Cup circuits.

Simone Sabbioni is an Italian competitive swimmer and two-time Olympian who competed at the 2016 Olympics and the 2020 Olympics. He is a former Italian national record holder in the short course 50 metre backstroke, 200 metre backstroke, 4x50 metre medley relay and the long course 100 metre backstroke.

Blake John Pieroni is a retired American professional swimmer who specialized in freestyle events. He is a two time Olympian and gold medalist in the 4x100-meter freestyle relay at both the 2016 Summer Olympics and the 2020 Summer Olympics. On the relay in 2016, he swam in the prelims of the race, on the 2020 Olympics 4x100-meter freestyle relay he swam in both the prelims and the final. He also won a gold medal in the 4x100-meter medley relay at the 2020 Olympic Games, swimming the freestyle leg of the relay in the prelims.

The mixed 4 × 100 metre medley relay event at the 2020 Summer Olympics was held in 2021 at the Tokyo Aquatics Centre. These Games marked the first time to feature a mixed-gender swimming event in the program. Each 4-person team features two male and two female swimmers.

Zachary "Zach" Douglas Apple is an American competitive swimmer who specializes in the sprint freestyle events. He currently swims for DC Trident in the International Swimming League. He won his first Olympic gold medal in the 4x100-meter freestyle relay at the 2020 Summer Olympics, swimming in the prelims and the final of the event, and later in the same Olympic Games won a gold medal and helped set a new world record and Olympic record in the 4x100-meter medley relay, swimming the freestyle leg of the relay in the final.

Andrey Dmitriyevich Minakov is a Russian swimmer and Olympian. He is the current world junior record holder in the short course 50 metre butterfly and a former world junior record holder in the long course 50 metre butterfly and 100 metre freestyle. He is the Russian record holder in the long course 100 metre butterfly. At the 2019 World Aquatics Championships, he won silver medals in the 100 metre butterfly and 4×100 metre freestyle relay, and a bronze medal in the 4×100 metre medley relay. In 2021, at the 2020 Summer Olympics, he placed fourth in the 100 metre butterfly, fourth in the 4×100 metre medley relay, seventh in the 4×100 metre freestyle relay, and tenth in the 100 metre freestyle. He also won two gold medals, one silver medal, and three bronze medals at the 2021 World Short Course Championships. In 2022, he won the NCAA title in the 100 yard butterfly.

Joseph Hunter Armstrong is an American swimmer. He is the world record holder in the long course 50 meter backstroke. At the 2020 Summer Olympics, he won a gold medal in the 4×100 meter medley relay, swimming backstroke in the prelims, and finished ninth in the 100 meter backstroke. He won a gold medal as part of the 4×100 meter freestyle relay, swimming in the prelims heats, at the 2022 World Aquatics Championships as well as a gold medal in the 4×100 meter mixed medley relay, swimming backstroke in the final, a silver medal each in the 50 meter backstroke and the 4×100 meter medley relay, and a bronze medal in the 100 meter backstroke.
