Water polo has been part of the Summer Olympics program since the second games, in 1900. A women's water polo tournament was introduced for the 2000 Summer Olympics. Hungary has been the most successful country in men's tournament, while the United States is the only team to win multiple times at the women's tournament since its introduction. Italy is the first and only country to win both the men's and women's water polo tournaments.

The event took place in Winnipeg, Manitoba, Canada from May 24 to May 29, 1999. Participating teams were the eight leading teams from the World Championships. The tournament served as a qualifier for the 2000 Summer Olympics in Sydney, Australia. Automatically the first ranked teams from Europe and the Americas, plus one team from the host country, qualified. Other teams could qualify at the 2000 Olympic Games Qualifying Tournament, held from April 22 to April 30 in Palermo, Italy.

The 2004 Men's Olympic Water Polo Qualifying Tournament was a tournament to decide the remaining three competing teams at the 2004 Summer Olympics in Athens, Greece.

The 2008 Women's Olympic Water Polo Qualifying Tournament was a tournament to decide the remaining four competing teams at the 2008 Summer Olympics in Beijing, PR China. Four teams were already qualified: Australia, China, the Netherlands and the United States. The tournament was held at the new Olympic Swimming Pool (2005) in Imperia, Italy from February 17 to February 24, 2008 with twelve competing teams.
