Coronectomy

When extracting lower wisdom teeth, coronectomy is a treatment option involving removing the crown of the lower wisdom tooth, whilst keeping the roots in place in healthy patients. This option is given to patients as an alternative to extraction when the wisdom teeth are in close association with the inferior alveolar nerve, and so used to prevent damage to the nerve which may occur during extraction. Advantages

Reduces risk of neuropathy [2] compared with full extraction.

The risk of altered sensation is significantly lower than convention surgical removal of mandibular third molars. Approximately 0.65% of individuals encounter postoperative deficits in the Inferior alveolar nerve (IAN) following coronectomy, a significantly lower occurrence compared to the 5.10% observed after conventional extraction procedures. Patients undergoing coronectomy are anticipated to experience easier recovery from IAN deficits compared to those undergoing extractions. Limited studies indicate a 100% recovery rate in coronectomy patients, whereas only 66% of patients undergoing extraction recover within one month. 62.2% of the roots will migrate post-coronectomy, erupting away from the inferior alveolar canal. This makes extraction of the remaining roots safer. [3]

Disadvantages

There is a 5% chance of failure of coronectomy, the root will become mobilized during transection. [4] In 5% of the cases, follicle remnants will form deep periodontal pockets which will lead to infection. [5]

Radiography

A plain film radiograph allows the proximity of the tooth to the inferior alveolar canal to be assessed. The plain film can be assessed to identify the tooth as high risk If there is; loss of the lamina dura, darkening of the canal and grooving of the root. If the mandibular third molar is deemed to be high risk, a cone beam CT (CBCT) is taken in addition to the plain film. The justification of additional radiography can be justified by the surgeon as it allows them to gain further information regarding the tooth roots and the inferior alveolar canal should the roots be mobilised when transecting.

Verbal consent must be attained by the surgeon prior to the procedure of a coronectomy. Additionally consent must be gained if removal of the roots is required due to mobilisation. The patient should be informed of early and late infection meaning the roots may need removing.

Operative technique

  1. Long buccal infiltration and anterior buccal infiltration both with 4% articaine, 1:10000ppm of epinephrine. This can be supplemented with an inferior dental block using 2% lidocaine, 1:80000 epinephrine.
  2. The third molar is exposed by raising a buccal triangular mucoperiosteal fold thickness flap.
  3. A fissure bur is used to create a buccal gutter of bone and to expose the ACJ.
  4. Drill directly into pulp at intersection of buccal groove and ACJ.
  5. This cut is lateralised to create a horizontal groove below the level of the ACJ.
  6. The crown can be fractured off the roots using a small elevator such as coupland number 1 or straight Warwick James.
  7. Care must be taken to not apply too much torque to the tooth so minimising root mobilisation.
  8. Use a rose head bur to remove enamel fragments.
  9. Remaining roots should sit a few millimeters below the alveolar crestal bone level, where the pulp should not be touched.
  10. Then follow up with closing the area and suturing.
  11. Post operative chlorohexidine mouth wash or gel can be prescribed, however antibiotics should not be prescribed unless there is pericoronal infection.

Post operative complications

Early

If the patient presents with dry socket, irrigate with chlorohexidine mouthwash and place resorbable dressing such as Alvogyl. If the patient has recurrent infection, consideration to remove the roots should be noted.

Late

In a few cases the remaining roots may erupt which can minimise the morbidity of the inferior alveolar nerve, however the roots may be in close contact to the inferior alveolar nerve requiring surgical separation. [1]

Dilaceration is a developmental disturbance in shape of teeth. It refers to an angulation, or a sharp bend or curve, in the root or crown of a formed tooth. This disturbance is more likely to affect the maxillary incisors and occurs in permanent dentition. An impacted tooth is one that fails to erupt into the dental arch within the expected developmental window. Because impacted teeth do not erupt, they are retained throughout the individual's lifetime unless extracted or exposed surgically. Teeth may become impacted because of adjacent teeth, dense overlying bone, excessive soft tissue or a genetic abnormality. Most often, the cause of impaction is inadequate arch length and space in which to erupt. That is the total length of the alveolar arch is smaller than the tooth arch. The wisdom teeth are frequently impacted because they are the last teeth to erupt in the oral cavity. Mandibular third molars are more commonly impacted than their maxillary counterparts.

Inferior alveolar nerve block is a nerve block technique which induces anesthesia (numbness) in the areas of the mouth and face innervated by one of the inferior alveolar nerves which are paired on the left and right side. These areas are the skin and mucous membranes of the lower lip, the skin of the chin, the lower teeth and the labial gingiva of the anterior teeth, all unilaterally to the midline of the side on which the block is administered. However, depending on technique, the long buccal nerve may not be anesthetized by an IANB and therefore an area of buccal gingiva adjacent to the lower posterior teeth will retain normal sensation unless that nerve is anesthetized separately, via a (long) buccal nerve block. The inferior alveolar nerve is a branch of the mandibular nerve, the third division of the trigeminal nerve. This procedure attempts to anaesthetise the inferior alveolar nerve prior to it entering the mandibular foramen on the medial surface of the mandibular ramus.

Impacted wisdom teeth is a condition where the third molars are prevented from erupting into the mouth. This can be caused by a physical barrier, such as other teeth, or when the tooth is angled away from a vertical position. Completely unerupted wisdom teeth usually result in no symptoms, although they can sometimes develop cysts or neoplasms. Partially erupted wisdom teeth or wisdom teeth that are not erupted but are exposed to oral bacteria through deep periodontal pocket, can develop cavities or pericoronitis. Removal of impacted wisdom teeth is advised for the future prevention of or in the current presence of certain pathologies, such as caries, periodontal disease or cysts. Prophylactic (preventative) extraction of wisdom teeth is preferred to be done at a younger age to take advantage of incomplete root development, which is associated with an easier surgical procedure and less probability of complications.

<span class="mw-page-title-main">Oroantral fistula</span> Medical condition

Oroantral fistula (OAF) is an epithelialised oroantral communication (OAC). OAC refers to an abnormal connection between the oral cavity and antrum. The creation of an OAC is most commonly due to the extraction of a maxillary (upper) tooth closely related to the antral floor. A small OAC may heal spontaneously but a larger OAC would require surgical closure to prevent the development of persistent OAF and chronic sinusitis.

Alveoloplasty is a surgical pre-prosthetic procedure performed to facilitate removal of teeth, and smoothen or reshape the jawbone for prosthetic and cosmetic purposes. In this procedure, the bony edges of the alveolar ridge and its surrounding structures is made smooth, redesigned or recontoured so that a well-fitting, comfortable, and esthetic prosthesis may be fabricated or implants may be surgically inserted. This pre-prosthetic surgery which may include bone grafting prepares the mouth to receive a prosthesis or implants by improving the condition and quality of the supporting structures so they can provide support, better retention and stability to the prosthesis.

In the dental specialty of endodontics, periradicular surgery is surgery to the external root surface. Examples of periradicular surgery include apicoectomy, root resection, repair of root perforation or resorption defects, removal of broken fragments of the tooth or a filling material, and exploratory surgery to look for root fractures.
