Suspected cases have not been confirmed by laboratory tests as being due to this strain, although some other strains may have been ruled out.

An outbreak of an unidentified flu-like illness (sometimes referred to as "Disease X" [4] ) appeared to begin in October 2024 in the southwestern region of Kwango Province, Democratic Republic of the Congo. The outbreak has been primarily in the Panzi rural health zone and has prompted investigation by local and international health organizations. Estimates of fatalities vary from 31 [3] to 143 fatalities by 5 December. [5] [6] On 5 December, the disease was reported as having spread to Katenda, also in the Kwango Province. [7] On 10 December, the disease was reported to have spread to Mai-Ndombe Province. [8] From 5-12 December, 147 new cases were reported, a 30 case increase from the week before. [9]

Initial analysis suggested that cases may largely be malaria combined with high levels of malnutrition. [10]

Outbreak

The first known case was identified on 24 October 2024, [1] with the first recorded fatality recorded on 10 November 2024. [7] Central authorities were only notified later, [1] with the Ministry of Public Health of the Democratic Republic of the Congo reporting the outbreak to the World Health Organization (WHO) on 29 November 2024. [3]

As of 8 December, cases had been reported from 9 health areas (out of 30) in the Panzi health zone: Kahumbulu, Kambandambi, Kanzangi, Kasanji, Kiama, Mbanza Kipungu, Makitapanzi, Mwini ngulu, and Tsakala Panzi. 96% of those cases were from Tsakala Panzi (169 cases), Makitapanzi (142 cases) and Kanzangi (78 cases) health areas. [3] These areas have suffered food shortages in preceding months and have low vaccination coverage. [3] On 10 December, the outbreak was reported by Congolese media to have spread to Mai-Ndombe Province, which is separated from Kwango by the Kinshasa Province, although this has not been confirmed by laboratory tests. The Congolese government has deployed response teams there. [8]

The WHO reported 406 cases from 24 October – 5 December, with a peak in the week ending 9 November 2024. They also report 31 deaths, but that there were further community deaths that were yet to be investigated. [3] According to provincial authorities, including Deputy Provincial Governor Rémy Saki, the death toll ranged from 67 to 143 individuals up to 25 November, several of whom died in their homes due to lack of medical care.

Most cases are in children, particularly those under the age of five, [3] and cases are slightly more common in women rather than men. [1] Most of the dead are under 15 years old. [3]

A man who worked 500 kilometers from the documented outbreak area was hospitalized at the San Luca Hospital in Lucca, Italy, from 22 November to 3 December 2024. [11] He was discharged upon recovery. Samples were collected and will be sent to the Istituto Superiore di Sanità for analysis. [12]

Symptoms

Those afflicted with the unidentified illness suffered from several flu-like symptoms, including severe headache, cough, high fever, and nausea, with the addition of anemia. [5] [7] [13] The 8 December WHO report gave symptom prevalences of fever (96.5%), cough (87.9%), fatigue (60.9%) and a running nose (57.8%). They also noted that the main symptoms associated with fatalities included difficulty in breathing, anemia, and signs of acute malnutrition. [3]

The 8 December WHO report gives 31 deaths out of 406 cases, producing a case fatality ratio of 7.6%, but noted all severe cases were reported to be severely malnourished. [3]

Cause

A WHO report on 8 December 2024 reported ongoing laboratory tests to determine the cause of the disease, but noted that it is also possible more than one disease is involved. Acute pneumonia, influenza, COVID-19, measles, hemolytic uremic syndrome from E. coli , and malaria are being considered as potential causes, with malnutrition as a contributing factor. The WHO report also notes that malaria is a common disease in the area and may otherwise be contributing. [3]

On 10 December, the WHO reported that 10 out of 12 samples tested positive for malaria. Dr Abdi Mahamud, interim director of alert and response coordination for the WHO, noted that malaria is endemic in the region and that there has been an increase in existing respiratory disease with the rainy season. He also said that the data do not show an “explosive increase” in either cases or deaths. [10]

Response

Kwango health minister Apollinaire Yumba implemented preliminary containment measures, including advising residents to avoid contact with deceased individuals to prevent potential transmission. Local authorities requested additional medical supplies from the international community. [5] [6] The health ministry warned people not to go to mass gatherings, report any suspected cases or unusual deaths, and to maintain basic hygiene and not to handle deceased bodies. They also stated the reason of the mission is to ensure immediate care, collect samples for the laboratory and to conduct deep investigations to identify the nature of the disease. [14] Due to the rural nature of the province, the disease samples had to be taken to a laboratory 500 kilometers away, delaying results. [7]

The World Health Organization (WHO) was alerted to the outbreak on 29 November, [3] and stated that it was working with the health officials of the Democratic Republic of the Congo to make further investigations as to what the unknown flu-like illness is. [13] The WHO is also sending teams to collect samples. [15] The CDC of Africa also responded and said that they are investigating with the government of the Democratic Republic of the Congo on this issue. [6] [16]

On 5 December 2024, Hong Kong tightened health screenings at airports and at other boundary control points in response to the outbreak. [17] On 6 December Italy's Ministry of Health asked its border offices to pay attention to people coming from the DRC, especially in ports and airports. [18]

Kwango was formed from the Kwango district whose town of Kenge was made the provincial capital and thus gained city status.

Ebola, also known as Ebola virus disease (EVD) and Ebola hemorrhagic fever (EHF), is a viral hemorrhagic fever in humans and other primates, caused by ebolaviruses. Symptoms typically start anywhere between two days and three weeks after infection. The first symptoms are usually fever, sore throat, muscle pain, and headaches. These are usually followed by vomiting, diarrhoea, rash and decreased liver and kidney function, at which point some people begin to bleed both internally and externally. In 2014, an outbreak of Ebola virus disease in the Democratic Republic of the Congo (DRC) occurred. Genome sequencing has shown that this outbreak was not related to the 2014–15 West Africa Ebola virus epidemic, but was of the same EBOV species. It began in August 2014 and was declared over in November of that year, after 42 days without any new cases. This is the 7th outbreak there, three of which occurred during the period of Zaire.

The Kivu Ebola epidemic was an outbreak of Ebola virus disease (EVD) mainly in eastern Democratic Republic of the Congo (DRC), and in other parts of Central Africa, from 2018 to 2020. Between 1 August 2018 and 25 June 2020 it resulted in 3,470 reported cases. The Kivu outbreak also affected Ituri Province, whose first case was confirmed on 13 August 2018. In November 2018, the outbreak became the biggest Ebola outbreak in the DRC's history, and had become the second-largest Ebola outbreak in recorded history worldwide, behind only the 2013–2016 Western Africa epidemic. In June 2019, the virus reached Uganda, having infected a 5-year-old Congolese boy who entered Uganda with his family, but was contained.

<span class="mw-page-title-main">Jean-Jacques Muyembe-Tamfum</span> Virologist known for working with Ebola

Jean-Jacques Muyembe is a Congolese microbiologist. He is the general director of the Democratic Republic of the Congo Institut National pour la Recherche Biomedicale (INRB). He was part of team at the Yambuku Catholic Mission Hospital that investigated the first Ebola outbreak, and was part of the effort that discovered Ebola as a new disease, although his exact role is still subject to controversy. In 2016, he led the research that designed, along with other researchers at the INRB and the National Institute of Health Vaccine Research Center in the US, one of the most promising treatment for Ebola, mAb114. The treatment was successfully tested during recent outbreaks in the DRC, on the express decision of the then DRC Minister of Health, Dr Oly Ilunga, despite advice against this from the World Health Organization.

In 2019, a measles epidemic broke out in the Democratic Republic of the Congo (DRC). The epidemic started in early 2019 in the southeast corner of the DRC and then spread to all provinces. By June 2019 the epidemic was reported to have exceeded the death toll of the concurrent Ebola epidemic. By April 2020, it had infected more than 341,000 people and claimed about 6,400 fatalities. This has primarily affected children under the age of five, representing 74% of infections and nearly 90% of deaths.

The COVID-19 pandemic in the Democratic Republic of the Congo was a part of the worldwide pandemic of coronavirus disease 2019 caused by severe acute respiratory syndrome coronavirus 2. The virus was confirmed to have reached the Democratic Republic of the Congo on 10 March 2020. The first few confirmed cases were all outside arrivals.
