While efforts towards a Howerdel solo album were started even back in his time in A Perfect Circle during touring in support of their 2003 album Thirteenth Step, commitments to A Perfect Circle and other projects, along with some difficult recording sessions, pushed the album's US release out to April 8, 2008. It was released in Australia on April 19. Its first single, "The Stone", entered radio circulation in January of that year.

The album was produced by Matt Tuck and mixed and mastered by Machine.

On January 10, 2014, after being silent for nearly two years, vocalist JJ Polachek announced the band is working on three potential releases for 2014 and 2015. It was described as a large conceptual work.

The album marked their highest chart positions in the US, debuting at number 29 on the Billboard 200 and number two on the Hard Rock Albums chart. "A Grave Mistake" became the band's first top ten hit on the Billboard Mainstream Rock charts. This is the final album to feature guitarist and vocalist Justin DeBlieck, as well as bassist Justin Morrow, who departed from the band in March 2019 to join Motionless in White.
